3 CITY OF ELKO INVITATION TO BID Sealed bids will be received until 3:00 P.M., local time, on Thursday, August 11, 2016 at the office of the Elko City Clerk; 1751 College Avenue; Elko, Nevada 89801, for the following: Proposals received after that time will not be accepted. Bidders mailing their bids assume the risk of late delivery. The submitted bids will be opened at the Elko City Hall immediately after the published deadline for acceptance of said bids with City staff present. All interested parties are invited to be in attendance at the bid opening. The City Council may formally accept or reject the bids at their regular meeting after 4:00 P.M., local time, Tuesday, August 23, Specifications and other information may be obtained at the office of the Elko City Clerk. The City of Elko reserves the right to accept or reject any or all items specified in the bid request as is deemed in the best interest of the City of Elko. 4 CITY OF ELKO BIDDING INFORMATION The City of Elko reserves the right to accept or reject any or all items specified in the bid request and reserves the right to waive any minor technicalities in the preparation of these bidding documents or specifications. The award may be made to the lowest responsive and responsible bidder. The lowest responsive and responsible bidder will be judged based on price, warranty, conformance to specifications, materials, or equipment offered and their adaptability to the required purposes, and the best interest of the public--each of such factors being considered. A. Sealed bids are to be filed with the Office of the City Clerk; 1751 College Avenue; Elko, Nevada; prior to 3:00 P.M., local time, Thursday, August 11, B. 5 CITY OF ELKO SPECIAL TERMS AND CONDITIONS A. Bidder is required to submit descriptive data or printed specifications describing unit offered. Failure to comply with this request may be cause for non-acceptance of bid. B. ADHERENCE TO SPECIFICATIONS: Deviations to any of the requirements set forth in this bid shall be so stated in written form in space provided or attached on separate sheet if additional space is required. Delivery: Unit shall be transported to the City of Elko Facilities Department, 975 Water Street, Elko, Nevada; completely furnished and ready for operation. E. Warranty: Minimum warranty shall be one year on the Aerial Lift Unit and Manufacturer s standard warranty for the cab and chassis. Copy of warranty must be furnished with submitted bid. Successful bidder shall provide warranty service within twenty-four (24) hours after notification that such service is required on any vehicle that is inoperative due to failures covered by warranty. In the event such service is not provided within the specified time, the City of Elko may elect to have the repairs made or make the repairs in their own shop and bidder shall reimburse the City of Elko for all costs incurred for such repairs. All warranty and recall service shall be performed at an authorized service facility or on-site if applicable. F. Bidder shall be prepared to give a complete demonstration of the merits of the vehicle offered under the normal working conditions of the City of Elko Facilities Department any time prior to an Award being made. G. Questions pertaining to bidding documents shall be directed to the City of Elko Public Works Director; 1751 College Avenue; Elko, NV or by calling (775) H. 7 CITY OF ELKO, NEVADA SPECIFICATIONS FOR GENERAL: The City of Elko, Nevada is accepting bid proposals for One (1) New One-Man Articulating Telescopic Aerial Lift Truck with Utility Body. Equipment furnished under this specification shall be the manufacturer's current conventional design for the type and the allied equipment specified. All necessary accessories customarily furnished whether stipulated herein or not, with such modifications and attachments as may be necessary to enable the equipment to function reliably and efficiently in sustained operation. Further, all equipment furnished under this specification shall comply with the most current Federal Regulations and Laws along with all requirements of Nevada Revised Statutes. Any use of specific names and/or model numbers in the specifications is not intended to restrict the bidder or any seller or manufacturer, but is included solely for the purpose of indicating the type, size, and quality of the equipment considered best adapted to the use of the City of Elko. The unit will be used in Elko, Nevada at an altitude of approximately 5,000 to 6,000 feet. UNIT DESCRIPTION FOR: The following are required minimum options: GENERAL: This specification describes a minimum 40 Foot Hydraulic Telescopic Aerial Device or approved equivalent. Prototypes are NOT acceptable. Basic requirements shall include, but are not limited to the following: Equipment bid must conform to all State of Nevada and/or Federal regulations, whichever is more stringent, as relates to Safety, Pollution Control (exhaust emission and noise), etc. 5

Telescopic Upper Boom - Telescopic Upper Boom shall have ANSI A92.2 C-46 kv rating J. Boom Rest and Platform Support Unit shall be equipped with a boom rest and a platform support K. Platform Leveling System - Unit shall have a hydraulic platform leveling system L. Platform: Fiberglass non-insulating platform for use with or without insulating liner (per ANSI A92.2). M. Upper and Lower Controls Unit shall have full pressure upper and lower controls N. Controls - Boom and articulating arm functions shall be controlled with a single handle control. O. Outrigger/Boom Interlock System Unit shall be equipped with an Outrigger/Boom Interlock System to prevent boom from being unstowed until outriggers have been at least partially deployed. P. Outrigger/Unit Selector Control: Located near the outrigger controls, allows operator to divert hydraulic oil from machine circuit for outrigger operation. This reduces the potential for inadvertent outrigger movement during machine operation if outrigger controls are bumped. Q. Outrigger Motion Alarm Unit shall be equipped with an Outrigger Motion Alarm that provides audible alarm when any of the outriggers are in motion. R. Back-up Alarm Unit shall be equipped with a back-up alarm S. Diagnostic Pressure Test Quick Disconnect Couplings Unit shall be equipped with Diagnostic Pressure test Quick Disconnect Couplings. T. ANSI Category C, 46 kv and below dielectric rating. Upper boom must be extended approximately 22 inches. U. Manuals Two (2) Operator s and two (2) Maintenance/Parts manuals containing instructional markings indicating hazards inherent in the operation of an aerial device shall be provided. V. Paint: Painted white to match cab and chassis. 6

Unit shall be equipped with non-skid surfaces with non-skid treatment applied on all walking surfaces and the tops of the bins. 6. Unit shall be equipped with rear access steps. 7. Unit shall be equipped with a tail shelf installed at the rear of the body. 8. Unit shall be equipped with grab handles - one each side at rear of tail shelf. 9. Unit shall be equipped with a security door lock system with provisions for locking all compartments with central locking points. 11. Unit shall be equipped with heavy duty pintle hook with chassis frame reinforcement and two (2) safety chain rings. 12. Unit shall be equipped with lights and reflectors to include four corner LED strobe lights. 13. Unit shall be equipped with a trailer receptacle installed at rear 14. Unit shall be painted white to match cab and chassis 19. Unit shall have all safety and instructional signs installed. 20. Successful bidder is to supply a self-directed, computer based training (CBT) program. This program will provide basic instruction in the safe operation of this aerial device. This program will also include and explain ANSI and OSHA requirements related to the proper use and operation of this unit. 21. Bidder is to supply a copy of the manufacturer s warranty with the bid. ADDITIVE ALTERNATE 1 Yes No ISO-Grip TM System: The Altec ISO-Grip TM (U.S. Patent No.,416,053) System includes the following boom tip components that can provide an additional layer of secondary electrical contact protection. This is not a primary protection system. Control Handle: A single handle controller incorporating that is dielectrically tested to 40 kv AC with no more than 400 microampers of leakage. The control handle is green in color to differentiate it from other non-tested controllers. The handle also includes an interlock guard that reduces the potential for inadvertent boom operation. Auxiliary Control Covers: Non-tested blue silicon covers for auxiliary controls. Control Console: Non-tested non-metallic control console plate. Boom Tip Covers: Non-tested non-metallic boom tip covers. The covers are not dielectrically tested, but they may provide some protection against electrical hazards. 8
