1 June 26, 2018 Addendum No. 02 File Reference Number: RFQ Title: Propane Powered Forklift RE: Clarifications/Questions Please note the following clarifications regarding the above referenced RFQ: 1. Question: Can Warranty references be amended? Answer: (1) The warranty period for the Equipment provided by the Supplier shall be as described in the Supplier s Submission. Unless otherwise identified by the Supplier in its submission, the Supplier will repair or replace any material and workmanship which is defective, at its sole expense, during the warranty period as described in the Supplier s Submission. (2) At the time of delivery and for the duration of the warranty period, the Supplier warrants that the Equipment shall: (a) be free of all claims and encumbrances (including any liens); (b) be of good quality and workmanship, incorporate only new material and be free of deficiencies and defects (including latent defects) of design, material, quality, non-infringement, workmanship and title; (c) comply with industry accepted standards and practices; (d) comply with all applicable legislation and all specifications, drawings, samples, and other description provided by ONTC; and (e) be fit for the intended purpose. The warranties do not apply to normal wear and tear or Equipment that have been subject to misuse or abuse by ONTC. (3) ONTC shall benefit from all legal and manufacturer s warranties available in connection with the Equipment, equipment, machinery, tools and consumables incorporated in the Equipment and/or Services and the Supplier shall assign all such warranties to ONTC on the same terms in which these warranties have been provided to the Supplier. The Supplier shall cooperate with ONTC in the enforcement of the assignment and the warranties against the manufacturers. 2. Respondents shall replace the specification issued with the RFQ Part 3 RFQ Specifications, Schedule 3-A, Scope of Work with the revised RFQ Part 3 RFQ Specifications, Schedule 3- A Scope of Work included with this addendum as an attachment below.

Respondents shall replace Part 4, Form of Proposal, Proposal Form 3, Acknowledgment to comply with the Schedule 3-A Scope of Work with the revised Part 4, Form of Proposal, Proposal Form 3, Acknowledgment to comply with the Schedule 3-A Scope of Work included with this addendum as an attachment below. 4. Submission Requirements ONTC has revised the format for submissions. Respondents may proceed with submitting per the RFQ documents or submit electronically. If submitting your quotation electronically, please ensure the subject line is clearly marked RFQ Propane Powered Forklift Submission. Electronic submissions shall be ed to: 5. 3 Introduction PART 3 RFQ SPECIFICATIONS SCHEDULE 3-A SCOPE OF WORK Ontario Northland Transportation Commission (ONTC) is seeking proposals for the supply of One (1) Propane Powered Forklift. Scope of Work Ontario Northland Transportation Commission (ONTC) requests that the Respondent submit a Quotation for the supply of One (1) Propane Powered Forklift that meets the following requirements: Between 100 and 150 horsepower Load capacity - minimum of 6000 pounds and maximum of 9000 pounds Side shift cage Pneumatic tires Turning radius under 8 feet Width under 4 feet Propane Fueled Power steering Enclosed cab with removable doors Adjustable seat Cab heater Cab windshield wiper Backup beeper Fire extinguisher Amber flashing light Block heater Electric Horn Capable of driving through a 91 high doorway Standard 48 long pallet forks with 48 high load backrest, high visibility Hydraulic lift/lower and tilt functions Heavy duty, industrial, propane fueled engine, naturally aspirated and liquid cooled, complete with cold start options. Quote base engine and any optional engines available. Full hydrostatic power steering utilizing an axle beam with integral double acting steer cylinders. High efficiency replaceable element dry type engine air cleaner, complete with precleaner and service indicator. High capacity radiator with coolant recovery system.

4 Complete instrumentation package with the following indicators as a minimum requirement: Coolant Temperature Transmission Oil Temperature Engine Oil Pressure Battery Charging Engine Hour Meter Transmission: Power reversing full synchromesh torque converter transmission designed for maximum reliability under severe operating conditions High capacity transmission oil cooler Full parts and service warranty for 1 year FOB Delivered to Cochrane, Ontario by September 30, 2018
