Infrmatin cntained herein applies t typical instances and may nt address all circumstances. FILE REVIEW 1. ENVIRONMENTAL SITE ASSESSMENT Submitted and apprved? If required, a Phase I Envirnmental Site Assessment shall be apprved prir t issuance f any grading, demlitin r cnstructin permits. Verify if site is cntaminated. 2. FEES Permit fees entered in Permits Plus. 3rd r greater checks require an hurly fee fr the review. DOCUMENTATION 3. Grundwater Wells Are wells being remved? All wells being remved r destryed require separate permit frm the Snma Cunty Permit and Resurces Management Divisin - Well & Septic (707) ). Prvide cpy f PRMD permit t the SRFD. 5. Undergrund Service Alert Plan shall include lcatins f all utilities. Cntact Service Alert at prir t the start f any excavatin. 6. Subsurface Cntaminatin - Ensure subsurface is nt cntaminated Refer t Phase I reprt 7. Site Security Verify site security plan is in place. 8. Training Verify training certificatins fr OSHA Hazwpper are submitted with packet. 9. Site Safety Plan The plan shall cver: PPE Persnal prtective equipment. Health and safety fficers. Installatin cntractrs must be recertified as required by the equipment manufacturers r every 3 years. 14. ICC Certificatins Verify at least ne persn t be n site must be certified by the Internatinal Cde Cuncil as a CA UST SYSTEM INSTALLER and as a CA UST SYSTEM SERVICE TECHNICIAN. TANKS 15. SECONDARY CONTAINMENT TITLE 23 CCR 2631 (a) - All new undergrund strage tanks including assciated piping used fr the strage f hazardus substances shall have primary and secndary cntainment. Primary cntainment shall be prduct-tight. Secndary cntainment may be manufactured as an integral part f the primary cntainment r it may be cnstructed as a separate cntainment system. Secndary cntainment systems shall be designed and cnstructed such that the secndary cntainment system can be peridically tested 18. UL LISTED - TITLE 23 CCR 2631(b) Design and cnstructin f primary cntainment is apprved by an independent testing rganizatin (e.g., UL). 19. MANUFACTURER - TITLE 23 CCR 2635(a)(6) Tank systems will be installed in accrdance with manufacturers written installatin instructins 20. LOADING - TITLE 24 CFC #1 - Tanks are lcated with respect t existing fundatins and supprts such that the lads carried by the latter cannt be transmitted t the tank. 21. SPACING TITLE 24 CFC #2 - Tanks are lcated nt less than 3 feet frm the nearest wall f a basement, pit, cellar, r lt line. 22. TANK SPACING - TITLE 24 CFC #3 - Tanks are separated by at least 1 ft, measured shell-t-shell. 23. SPILL CONTAINER - CHSC (f), (e), 25291(c), TITLE 23 CCR 2635(b), A spill cntainer having minimum 5 galln capacity and drain valve allwing drainage f cllected spills t the primary tank is prvided at each tank fill lcatin. 24. OVERFILL - HSC (f), (e), 25291(c), TITLE 23 CCR 2635(b), An apprved verfill preventin device is prvided at each tank fill lcatin. LOCATIONS SUBJECT TO FLOODING TITLE 24 CFC Where a tank is lcated in an area where it is subject t buyancy because f a rise in the water table, flding r accumulatin f water frm fire suppressin peratins, uplift prtectin shall be prvided in accrdance with Sectins and f NFPA 30. PIPING 27. Verify submittal includes the fllwing: 28. PRODUCT TIGHT - HSC (c)(1), (c)(1), 25291(a)(1), TITLE 24 CFC #1, NFPA Piping is prduct tight and cmpatible with the material(s) intended t be stred; is f adequate strength and durability t withstand the pressure, structural stress, and expsure t which it will be subject; and cmplies with ASME B UL LISTED - TITLE 23 CCR 2631(b), TITLE 24 CFC Design and cnstructin f piping is apprved by an independent testing rganizatin (e.g., UL). 30. MANUFACTURE INSTALL - TITLE 23 CCR 2636(c)(2) - Piping systems will be installed in accrdance with manufacturers written installatin instructins. 31. FLEX LINE - TITLE 24 CFC Undergrund liquid, vent, and vapr return piping is prvided with listed and apprved flexible jints at the fllwing pints: 32. Where piping cnnects t undergrund tanks; 33. Where piping ends at dispensing islands and vent risers; At pints where differential mvement in the piping can ccur. VENTS LOCATION - TITLE 24 CFC Tank vent piping discharges t a safe pint utside f buildings and away frm adjacent walls, eaves, r ther bstructins t assist in vapr dispersin, with the fllwing minimum distances: feet abve adjacent grund level; feet t any lt line f a prperty that can be built upn r pening int a building. 37. SHEAR VALVE - CFC Liquid supply piping is prvided with an apprved shear/fusible link emergency shutff valve at the base f each dispenser supplied by a remte pump. SECONDARY CONTAINMENT [Nte: Laminated, cated, r clad primary cntainment is cnsidered single-walled, and des nt satisfy requirements fr secndary cntainment.] 37. TANKS - CHSC (c) Verify secndary cntainment is prvided fr tanks. 38. PIPING - CHSC (c), (c), 25291(a), TITLE 23 CCR 2636 Verify secndary cntainment is prvided fr piping as fllws: Page 3 f 7

4 UST s installed n r after 7/1/2003 Secndary cntainment is required fr all prduct and remte fill lines; and all undergrund vent lines, vapr recvery lines, and riser piping cnnected t tank primary cntainment. 39. UL LISTING - TITLE 23 CCR 2631(b), 2631(d) - *Nte the design and cnstructin f each integral secndary cntainment system is apprved by an independent testing rganizatin (e.g., UL). Each secndary cntainment system which is nt an integral part f primary cntainment is designed and cnstructed accrding t an engineering specificatin apprved by a stateregistered Prfessinal Engineer r accrding t a natinally recgnized industry cde r engineering standard which includes the cnstructin prcedures. 40. PRODUCT TIGHT CHSC (c)(2), (c)(2), 25291(a)(2) Verify secndary cntainment is prduct tight and cnstructed t prevent structural weakening as a result f cntact with any hazardus substance released frm primary cntainment. 41. WATER INTRUSION - HSC (c)(3), (c)(3) Verify all secndary cntainment systems will be cnstructed t prevent any water intrusin int the system by precipitatin, infiltratin, r surface runff. [Nte: Required fr tank systems installed n r after 7/1/2003.] 42. UDC - HSC (c), (c), 25291(a), TITLE 23 CCR 2636(g) Verify a SWRCB apprved under-dispenser cntainment (UDC) sump r pan is prvided fr each dispenser. CORROSION PROTECTION 43. TANKS - TITLE 23 CCR 2635(a)(2) Verify tanks are prtected frm crrsin. 44. PIPING - TITLE 23 CCR 2636(b) Verify crrdible undergrund piping, if in direct cntact with backfill, is prtected against crrsin. BURIAL AND COVER 45. TITLE 24 CFC Verify plans shw detail f burial and cver prcedures. Tanks are t be set n a firm fundatin and surrunded by a minimum 6 inches f nn-crrsive inert material, such as clean sand r pea gravel. A certificatin, stamped by a registered engineer, that flding will nt ccur and that grundwater cnditins d nt warrant additinal engineering t cunteract tank buyancy, is included with this applicatin. [Alternative: Attach buyancy calculatins, stamped by a registered engineer and based upn the assumptin that each tank lies cmpletely submerged.] DISPENSERS 46. Verify the fllwing are included in the plans: 47. VEHICLE IMPACT TITLE 24 CFC Cncrete islands at least 6 inches high are prvided (r ther apprved methd f vehicle impact prtectin). 48. LOCATIONS TITLE 24 CFC Dispensers are sited with the fllwing minimum distances: 1.) 10 feet t any lt line; 2.) 20 feet t any fixed surce f ignitin; 3.) 10 feet t buildings having cmbustible exterir wall surfaces r buildings having nncmbustible exterir wall surfaces that are nt 1-hur rated r buildings having cmbustible verhangs; 4) such that all prtins f vehicles being fueled will be n the premises f the facility. 49. 5 50. HOSE LENGTH TITLE 24 CFC Dispenser hses are listed and apprved, and are n mre than 18 feet in length. When nt in use, hses will be reeled, racked, r therwise prtected frm damage. 51. BREAK-AWAY TITLE 24 CFC Each dispenser hse is prvided with an apprved emergency break-away cnnectr designed t retain liquid n bth sides f the breakaway pint. 52. KNOZZEL LISTED TITLE 24 CFC Each dispenser hse is prvided with a listed autmatic-clsing-type nzzle valve. 53. UN-MANNED TITLE CFC If dispensing is unsupervised, the fllwing are prvided: Operating instructins shall be cnspicuusly psted in apprved lcatin n every dispenser and shall indicate the lcatin f the emergency cntrls. VISIBLE TITLE 24 CFC Switch r sign is visible frm every dispensing lcatin. MONITORING Verify the fllwing are included in the plans: TANK MONITORING - CHSC (e) 59. Duble-walled tank with cntinuus mnitring using cntinuus vacuum, pressure, r hydrstatic mnitring f the annular space; AND 60. Any leak shall initiate an audible and visual alarm that can immediately be detected by the UST peratr PIPING MONITORING - CHSC (e) - All UST piping shall be duble-walled with cntinuus mnitring f the secndary cntainment using cntinuus vacuum, pressure, r hydrstatic including: 61. SUMP MONITORING - CHSC (d) - If a new UST tank is installed then the UST sumps shall be cntinuusly mnitred by ne f the fllwing methds: 65. Single-walled sump with single-walled piping inside sumps. The interir sump space shall be cntinuusly mnitred using cntinuus vacuum r pressure 66. Single-walled sump with cntinuusly mnitred duble-walled piping inside sumps using cntinuus vacuum, pressure, r hydrstatic 67. Duble-walled sump cntinuusly mnitred by using cntinuus vacuum, pressure, r hydrstatic mnitring f the annular space f the duble-wall. The internal sump space must have cntinuus liquid leak sensrs fr any leaks. 68. A leak shall initiate an audible and visual alarm that can immediately be detected by the UST peratr. 69. All sumps must have water-tight lids 70. Sumps must be installed n all UST riser piping and man-ways UNDER DISPENSER CONTAINMENT (UDC) MONITORING - HSC (d) - If a new UST tank is installed then the UDC shall be cntinuusly mnitred by ne f the fllwing methds: 71. Single-walled UDC with cntinuusly mnitred duble-walled piping inside UDC using cntinuus vacuum, pressure, r hydrstatic mnitring f the duble-walled piping. The dublewalled piping must extend all the way t the shear valve. 72. Duble-walled UDC cntinuusly mnitred by using cntinuus vacuum, pressure, r hydrstatic mnitring f the annular space. Inside the UDC, piping may be single walled but the internal space f the UDC must have a cntinuus liquid leak sensr fr any piping jint leaks, fuel filter, r ther leaks in the UDC as fllws: 1. Liquid flat sensr with audible and visual alarm and psitive shut dwn OR 2. Mechanical flat cnnected t shear valve 73. Any leak shall initiate an audible and visual alarm that can immediately be detected by the UST peratr, except 3, the mechanical flat shut-ff AUTOMATIC LINE LEAK DETECTORS (LLD s) fr PRESSURIZED PIPING - 23 CCR 2636(f)(2) - All pressurized piping must have autmatic line leak detectrs apprved by an independent testing rganizatin (e.g. UL Listing) fr its particular use. Must, at a minimum, detect release within 1 hur equivalent t 3.0 gph at 10 psi with a 95% prbability f detectin and 5% prbability f false alarm. Autmatic line leak detectrs must be capable f restricting the flw r shutting dwn the pump when a leak is detected. [Exceptin: Nt required fr Emergency Generatr Tank Systems (EGTS) meeting the requirements f TITLE 23 CCR 2636(f)(6).] 74. AUDIBLE VISUAL ALARM: 23 CCR2632(c)(2)(B), 2634(b) - Alarm panel prvides bth audible and visual alarms.
