1 PROBEM COMPAINT FIED COMPAINT TOYOTA PRIUS CARY I JTDKB20U T*THE CONTACT OWNS A 2004 TOYOTA PRIUS. WHIE DRIVING IN ANY TYPE OF WEATHER, ONE HEADIGHT WOUD DIM FOOWED BY THE OTHER. HOWEVER, BOTH HEADIGHTS COUD DIM SIMUTANEOUSY. THE FAIURE OCCURS INTERMITTENTY. THE VEHICE'S SPEED WAS NOT A FACTOR. DURING THE FAIURE, A OF THE OTHER IGHTS WERE OPERATIONA. THE CONTACT HAS HAD THE IGHTS REPAIRED DUE TO SAFETY CONCERNS. THE CURRENT MIEAGE IS 90,000 AND FAIURE MIEAGE WAS 70, TOYOTA PRIUS YONS KS JTDKB20U I HAVE ONY HAD THE 2007 PRIUS SINCE SO ESS THEN A MONTH OD WHEN I PUED INTO GARAGE AFTER DRIVING WITH HEADIGHTS ON AND NOTICED THE DRIVERS SIDE HEADIGHT WAS OUT. PASSENGERS WORKS FINE. I TURNED SWITCH OFF THEN ON AND IT CAME BACK ON. TOOK IT TO DEAERS THEY SAID THEY TESTED IT AND NO PROBEMS FOUND. 3 DAYS ATER I PUED UP IN FRONT OF A REFECTIVE WINDOW AND OW AND BEHOD MY DRIVERS SIDE HEADIGHT WAS OUT AGAIN. I GO IN WED, TO HAVE IT OOKED AT, I TOD THEM I WANT THE WHOE SOCKET REPACED. NO INJURY OR TICKET FROM POICE YET... BUT I IVE IN DEAR COUNTRY AND I NEED GOOD IUMINATION TO SEE WHERE I AM GOING SO PEASE POST THERE MAY NEED A RECA ON THIS PART. *TR TOYOTA PRIUS HAZEHURST WI JTDKB20U T*THE CONTACT OWNS A 2006 TOYOTA PRIUS. THE CONTACT STATED THAT THE HEADIGHTS WOUD NOT IUMINATE. SHE TURNED THE IGHTS OFF AND THEN ON AND THE IGHTS IUMINATED. THE FAIURE OCCURRED INTERMITTENTY. SHE ASO STATED THAT WHIE DRIVING 50 MPH, THE HIGH BEAM IGHTS DIMMED. THE DEAER WAS UNABE TO DIAGNOSE THE CAUSE OF FAIURE. THE POWERTRAIN AND FUE SYSTEM WERE UNKNOWN. THE FAIURE MIEAGE WAS 35,000 AND CURRENT MIEAGE WAS 37,800. Page 1

DRIVER SIDE HID HEADIGHT WOUD RE-IGNITE AFTER TURNING HEADIGHT SWITCH OFF THEN ON. *JB Page 2

HEADIGHT(S) WOUD GO OUT WHIE DRIVING AT NIGHT. DEAER SAID IT WAS HID COMPUTER AND THE REPAIR BI IS $1140!!!! RESEARCHING ON INTERNET REVEAS THIS IS A COMMON PROBEM. IT IS DEFINITEY SAFETY REAYED AND I FEE TOYOTA SHOUD MAKE THE REPAIR AT NO CHARGE. I'M NOT EXCITED ABOUT REPACING THE COMPUTER NOT ONY BECAUSE OF THE COST, BUT CAN I EXPECT THE PROBEM TO REPEAT IN A FEW YEARS? *TR TOYOTA PRIUS NEWARK DE JTDKB20U PROBEM STARTED IN AUGUST 07 WITH DRIVER'S SIDE HEADIGHT INTERMITTENTY SHUTTING OFF. NO HIGH OR OW BEAM, OFTEN VERY DIM. HEADIGHT BUB REPACED DUE TO SHORT CIRCUIT. REPACED DRIVER SIDE HEADIGHT ASSEMBY. MODUE HAS ASO BEEN REPACED. DEAER CURRENTY SUGGESTS THAT IT CANNOT BE FIXED IF PROBEM CANNOT BE DUPICATED. DEAER SAID THEY HAVE SPOKEN WITH TOYOTA AND THEY ARE NOT AWARE OF OTHER PEOPE HAVING THIS PROBEM. DEAER DOES NOT CONSIDER THIS PROBEM A SAFETY ISSUE. PROBEM HAS YET TO BE RESOVED AND THE CAR WI AGAIN BE GOING INTO THE SHOP. *TR Page 3

EXACT NUMBER OF FAIURES AND FIRST FAIURE BEOW IS AN ESTIMATE, AS IT IS NEARY IMPOSSIBE UNDER MOST CONDITIONS FOR THE DRIVER OF THE CAR TO KNOW THAT ONE OR BOTH HEADIGHTS ARE OUT--INCREASING THE DANGER OF THIS SITUATION. RAINY OR DUSK CONDITIONS ARE A PARTICUAR PROBEM. TOYOTA IS APPARENTY AWARE OF THIS SITUATION; THE DEAER TO WHOM I TOOK IT FOR REPAIR ACKNOWEDGED THIS AND SAID THAT IT AFFECTS A IMITED RANGE OF VIN NUMBERS IN 2006 S ONY. OCA DEAER HAS REPACED BOTH HEADAMPS AND SAYS THIS WI FIX THE PROBEM. THIS IS A SERIOUS SAFETY HAZARD ABOUT WHICH DRIVERS MAY BE UNAWARE; DRIVERS SHOUD BE NOTIFIED BY THE MANUFACTURER SINCE TOYOTA AND THE NHTSA ARE AWARE OF THIS PROBEM. I SEE THAT THERE HAVE BEEN AT EAST THREE PREVIOUS COMPAINTS REGARDING THIS PROBEM ON THE ODI WEBSITE. *TR TOYOTA PRIUS MORENO VAEY CA JTDKB20U HEADIGHTS ON 2006 TOYOTA PRIUS INTERMITTENTY SHUT OFF. FIRST RIGHT SIDE, THAN EFT. ONCE IGHTS SHUT OFF, THE SWITCH CAN BE TURNED OFF AND ON AND THEREAFTER IGHTS WI STAY ON FOR A SHORT WHIE THAN SHUT OFF AGAIN. FROM MY READING ON THE WEB, THIS IS NOT AN ISOATED CASE, AS OTHER OWNERS OF THE 2006 ARE HAVING THE SAME IDENTICA PROBEM. *TR TOYOTA PRIUS CARSON CA JTEDP21A CONTINUA PROBEM WITH HEADIGHTS ON MY 2006 PRIUS GOING OUT. I TOOK MY CAR INTO CABE TOYOTA IN ONG BEACH A FEW TIMES AND THEY COUD NOT FIND THE PROBEM OR RECREATE IT. I HAVE BEEN HAVE TROUBE WITH IT FOR 6 MONTHS, FINAY ON MY AS VISTS TOYOTA TECH DEPARTMENTS TOD THEM TO CHECK GROUNDING AND IF NOT AT FAUT JUST TO REPACE THE AMP ASSEMBY NOW THE OTHER HEADIGHT S GOING OUT RANDOMY. REPACING THE HEADIGHT I DO NOT BEIEVE WI SOVE THIS SAFETY REATED ISSUE. I AM STARTING TO HEAR THAT THIS IS A WIDE SPREAD PROBEM WITH THE 2006 PRIUS, TOYOTA IS RATHER SIENT ON THIS ISSUE. *TR TOYOTA PRIUS WINTHROP HARBO I I OWN A 2006 TOYOTA PRIUS. I AM HAVING PROBEMS AS WE AS MANY OTHER 2006 PRIUS OWNERS WITH THE HEADIGHTS TURNING OFF WHIE DRIVING. THE ISSUE IS RESOVED FOR A SHORT TIME BY TURNING THE IGHTS OFF AND BACK ON AGAIN- UNTI THE IGHTS TURN OFF AGAIN A FEW MINUTES ATER. I HAVE HAD IT IN TO 2 TOYOTA DEAERS AND THEY ARE STATING THAT IT IS BAD IGHTBUBS. IN ORDER TO REPACE THESE IGHTBUBS (2) IT IS $900!! AND MIGHT I ADD THAT THE TOYOTA DEAERS NO NOT CARRY THESE IGHTBUBS IN STOCK. THEREFORE, THIS EADS ME TO BEIEVE THAT THESE IGHTBUBS ON A 2006 NEW PRIUS SHOUD NOT BE GOING OUT WHIE DRIVING IN THE DARK AND ARE NOT A REGUAR REPACEMENT ITEM IF TOYOTA DOES NOT EVEN KEEP THE IGHTBUBS IN STOCK. I CURRENTY CAN NOT DRIVE MY CAR AT NIGHT BECAUSE IT IS A SAFETY HAZARD. MY IGHTS COUD TURN OFF AT ANY SECOND AND I COUD EASIY GET HIT/AND OR KIED IN AN ACCIDENT DUE TO FAIING HEADIGHTS. I AM CURRENTY IN THE PROCESS OF DEAING WITH TOYOTA AND THIS ISSUE. I ASO HAVE THE EXTENDED WARRANTY ON MY CAR WHICH I HAVE SPENT A OT OF MONEY FOR Page 4

THE FIRST COUPE OF TIMES I THOUGHT IT WAS THE PASSENGER'S SIDE BUT I DID NOT STOP TO FIND OUT. THE AST TIME IT WAS THE DRIVER'S SIDE FOR CERTAIN. TURNING THEM OFF AND BACK ON FIXES THE PROBEM. SOMETIMES IT HAPPENED SEVERA TIMES ON ONE TRIP (ESS THAN ONE HOUR). I OOKED ON PRIUS ONINE THEN HERE. WHEN I SAW OTHER INCIDENTS I DECIDED TO MAKE A COMPAINT. I WI TAKE THE CAR TO THE DEAER IN THE NEXT MONTH FOR SERVICE. *TR THE DEAER SERVICE MANAGER CONTACTED TOYOTA BEFORE DOING ANY WORK ON MY VEHICE. THE TOD HIM TO REPACE BOTH HID BUBS, WHICH HE DID. HE WAS TOD THAT THE BUBS OVERHEAT AND CUT OUT. THE PROBEM NOONGER OCCURS, BUT AS OF THIS WRITING IT HAS ONY BEEN A MONTH TOYOTA PRIUS REMSENBURG NY HID HEADIGHTS GO OUT WITHOUT WARNING. SOMETIMES JUST ONE, SOMETIMES BOTH. SHUTTING THE IGHTS OFF AND BACK ON TURNS THEM BACK ON. SOMETIMES THIS HAPPENS FREQUENTY, OTHER TIMES NOT AT A OR INFREQUENTY. CAR IS NO ONGER UNDER WARRANTY. TO HAS NOT BEEN CORRECTED AS THOSE WHO HAVE HAD IT CORRECTED ADVISE OF VERY HIGH COSTS (THOUSANDS) AND SOME HAVE HAD SIMIAR FAIURES WITH THE REPACEMENTS. *TR TOYOTA PRIUS TEMECUA CA JTDKB20U HID HEADAMPS FAI INTERMITTENTY WHIE DRIVING. FAIED HEADAMP CAN BE REIGNITED BY CYCING THE HEADAMP SWITCH. /R SIDE FAIURES ARE INDEPENDENT OF EACH OTHER. DEAER ADVISED THAT BOTH ECU AND BUBS MUST BE REPACED AT COST OF $1,000 PER SIDE. OBVIOUS SAFETY ISSUE WITH WIDESPREAD FAIURES AMONG PRIUS OWNERS WITH HID AMPS INDICATING CATASTROPHIC DESIGN AND/OR PARTS ISSUES. *TR Page 5

IGHT COUD BE RE-IT BY TURNING OUT AND TURNING ON HEADIGHTS AGAIN. COST OF REPACEMENT OF IGHT, AMP HOUSING AND ECU, IN EXCESS OF $ AT 60,000 MIES, RIGHT HEADIGHT SHOWING SAME PROBEM. REPACEMENT COST OF BUB IN EXCESS OF $ OSS OF HEADAMPS ONE AT A TIME OR BOTH AT THE SAME TIME MAKES NIGHT TIME DRIVING HAZARDOUS. UNEXPECTED REPEATED OSS OF IGHTS IS UNACCEPTABE. *TR Page 6

NEEDED TO STOP DRIVING AT NIGHT DUE TO THE SAFETY RISK UNTI I COUD GET THEM FIXED. IT TURNED OUT TO BE AN HID COMPUTER MAFUNCTION AND BOTH THE COMPUTER AND HEADAMP NEEDED TO BE REPACED. THE COST WAS ABOUT $750 FOR EACH COMPUTER/HEADAMP COMBINATION; IE, $1,500 TOTA. THE MAKE OF MY CAR IS A 2006 TOYOTA PRIUS, PURCHASED IN ATE '05. *TR TOYOTA PRIUS FAIRFIED CA JTDKB20U IN SEPT 2006, MY EFT HEADIGHT BEGAN TO GO OUT INTERMITTENTY. IT CAN BE RESTORED BY CYCING THE IGHT SWITCH OFF AND BACK ON. TOYOTA AT FIRST SAID THAT THEY COUD NOT DUPICATE THE PROBEM. THE FREQUENCY INCREASED, AND FINAY, THEY (1) ASKED ME IF I HAD HAD ANY ACCIDENTS (NO), THEN SUGGESTED THAT THE BUB WAS DEFECTIVE. IT COSTS ~$1, TO REPACE IT. I DIDN'T AT THE TIME, BUT EVENTUAY THE SHERIFF CAUGHT ME AND I HAD TO FIX IT. IT WAS OK FOR A WHIE, BUT NOW THE RIGHT SIDE AMP IS ASO DOING THE SAME THING, AND OCCASIONAY, BOTH AMPS GO OUT AT THE SAME TIME.. I CAN'T AFFORD ANOTHER $1000 IGHT BUB. THIS IS REAY GETTING TO BE A *)#*. AND, IT'S UNSAFE. (I SPEND 5 HOURS A DAY IN THE COMMUTE, AND EVENTUAY IT'S GOING TO GET ME KIED). *TR TOYOTA PRIUS SPRINGFIED VA JTDKB20U PASSENGER HEADIGHT ON 2004 TOYOTA PRIUS GOES OUT INTERMITTENTY. I HAVE NOTICED THIS ABOUT 8 TIMES. ON ONE OCCASION I WAS IN A VEHICE IN FRONT OF THE PRIUS AND THE HEADIGHT WENT OFF AFTER 15 MINUTES OF OPERATION. IT CAME BACK ON AFTER THE VEHICE WAS TURNED OFF AND RESTARTED. I TOOK IT TO THE DEAER TODAY, EXPECTING A DIAGNOSIS AND THEY PUT IN A NEW HID BUB ($324.00). I HAD THEM SAVE THE OD BUB BECAUSE I AM DOUBTFU A NEW BUB WI SOVE WHAT APPEARS TO BE AN EECTRICA SHORT CIRCUIT PROBEM. NOT HAVING BOTH HEADIGHTS OPERATIONA IS A SAFETY PROBEM THAT NEEDS TO BE ADDRESSED WITH THE ROOT CAUSE ASCERTAINED AND REPAIRED BY THE MANUFACTURER. *TR Page 7

IF YOU TURN THEM OFF AND THEN BACK ON THEY COME BACK ON FOR A WHIE THEN GO OUT AGAIN. I HAVE HAD BOTH THE BUBS AND CONTRO MODUE REPACED ANT THE TUNE OF $500 BUT THE PROBEM STI PERSIST. *TR TOYOTA PRIUS CORVAIS OR JTDKB20U INTERMITTENT PROBEM WITH HID HEAD IGHTS. BOTH RIGHT AND EFT HEAD IGHTS HAVE GONE OUT INDIVIDUAY (NOT AT THE SAME TIME) DURING OPERATION OF THE VEHICE, AND THEN BY TURNING IGHT SWITCH OFF AND THEN ON AGAIN THE HEAD IGHTS WORK. ON A DIFFERENT TRIP, EITHER HEAD IGHT MAY STOP WORKING AGAIN. THIS DOESN'T HAPPEN ON EVERY TRIP, AND IT SEEMS TO HAPPEN DURING OPERATION OF THE HEADIGHTS (I.E., TURN THE IGHTS "ON" AND BOTH HEAD IGHTS WORK, AND THEN AFTER SOME PERIOD OF TIME ONE OF THE HEAD IGHTS STOPS WORKING). THIS HAS HAPPENED MUTIPE TIMES DURING THE AST MONTH. *TR Page 8
