Background During the Fiscal Year, the SMCRA assisted in funding of both the design and construction of the now completed SW 66 th Infrastructure and Streetscape Improvement Project, Phase I (See Exhibit A) located east of SW 59 th Place. During the prior fiscal year, the SMCRA entirely funded the design and construction drawing portion for Phase II of the project located west of SW 59 th Place. The construction of Phase II of the project has now been recently completed (See Exhibit B). Improvements included in both Phase I and II of the project involved the installation of new curbs and gutters, complete road resurfacing, the installation of decorative crosswalks and landscaped traffic calming bulb-outs at strategic locations along SW 66 th Street. As is typical with most completed infrastructure and streetscape improvement projects, engineering "harmonization" is required to repair areas affected by new roadway construction. In this instance, the area located just west of 5990 SW 66 th Street is in need of both ADA and driveway harmonization. This condition was due the overall height new curbing far in excess the existing pavement height directly south of the completed improvements (See Exhibit C). Staff was recently made aware of the condition via correspondence from the City's Capital Projects Division identifying the issue and recommending repair. Capital Projects Manager, Mr. Keith Ng also forwarded the estimate from Acosta Tractors Inc. required to complete the necessary ADA harmonization repairs (See Exhibit D). Approval of the attached resolution shall authorize a funding disbursement to Acosta Tractors Inc. in the total amount of $5, to complete the above recommended ADA harmonization repairs. Recommendation Staff recommends approval of the attached resolution authorizing a funding disbursement in an amount not to exceed $5, to City of South Miami approved contractor, Acosta Tractors Inc. to complete final ADA infrastructure harmonization repairs as part of the SW 66 th Street Infrastructure & Streetscape Improvement Project, Phase II; and charging the total amount to Account No (SW 66 th Street Infrastructure Improvement Account). A resolution relating to infrastructure improvements; authorizing a funding disbursement in an amount not to exceed $5, to City of South Miami approved contractor, Acosta Tractors Inc. to complete final ADA infrastructure harmonization repairs as part of the SW 66 th Street Infrastructure & Streetscape Improvement Project, Phase II"; and charging the total amount to Account No (SW 66 th Street Infrastructure Improvement Account). Whereas, during the Fiscal Year, the SMCRA assisted in funding of both the design and construction of the now completed SW 66 th Infrastructure and Streetscape Improvement Project, Phase I (See Exhibit A) located east of SW 59 th Place; and Whereas, during the prior fiscal year, the SMCRA entirely funded the design and construction drawing portion of Phase II of the project located west of SW 59 th Place which was recently completed; and Whereas, Improvements included in both Phase I and II of the project involved the installation of new curbs and gutters, complete road resurfacing, the installation of decorative crosswalks and landscaped traffic calming bulb-outs at strategic locations along SW 66 th Street; and Whereas, as is typical with most completed infrastructure and streetscape improvement projects, engineering "harmonization" is required to repair areas affected by new roadway construction; and Whereas, in this instance, the area located just west of 5990 SW 66 th Street is in need of both ADA and driveway harmonization repairs due the overall height new curbing being far in excess the existing pavement height just south of the completed improvements; and Whereas, staff was recently made aware of the condition via correspondence from the City's Capital Projects Division identifying the issue, recommending repair and providing an estimate in the amount of $5,40501 to facilitate the recommended improvements; and Whereas, the SMCRA Board desires to continue to fulfill the goals and objectives contained in the adopted redevelopment plan including the improvement of damaged, inadequate infrastructure in the SMCRA Area. Now therefore be it resolved by the Community Redevelopment Agency of the City of South Miami that: Page 1 of2

11 From: Martinez, Grizel Sent: Thursday, February 16, :00 AM To: 'Goodson, Letitia S. (PHCD)' Cc: Ng, Keith Subject: FW: SW 66 St Ph II - Change Order for Handrail and Harmonization Letitia, Please see the attached change order to harmonize one of the properties and install a handrail adjacent to one of the ADA ramps on SW 66 Street Phase II. Upon inspection during a punchlist walkthrough, it was determined by the engineer that the installation of a handrail would address safety concerns posed for one of the newly constructed ADA ramps. The change order includes the work to harmonize the back of sidewalk with the existing grade of the church located on SW 60 th Avenue. Original copies of the change order are being sent to your office for formal review and approval. In light of the safety concerns, we would like to request an advanced concurrence from your office to proceed with the completion of the work. Thank you, Grizel Martinez Engineering & Construction Division City of South Miami 4795 SW 75 Avenue Miami, FI / / fax
