The Proposal Submission Due Date is Changed to March 8, 2016 (2:00 p.m.). This addendum is divided into the following parts: Part 1: Answers to questions. The State does not accept this recommendation. This information is proprietary to the incumbent contractor s network. The current contractor s reports are proprietary, and comprise Call Center statistics. Please refer to Part 2 of this document for Exhibit H, Attachment-Call Center Statistics. Overnight stays out of state requires the State Contract Manager s pre-approval, and the costs are passed through to the State. The Contractor pays for the room in advance, and the costs are passed through to the State.

Please elaborate on these types of trips Paragraph 2: It appears as though the data provided in this paragraph is outdated and attached data only goes thru in May Significant program changes have occurred this then including folding in the expansion population. Can the Agency please update this data through the most recent fiscal year at a minimum? Can the State provide the total annual ride counts for SFY 2013, SFY 2014 and SFY 2015? What challenges has the current vendor faced in the provision of MAV bariatric services and trips out of state? Exhibit E. Any data prior to June 2013 will not be sufficient in determining proper program costs due to Medicaid expansion. Please provide the following data for last two calendar years (2014 and 2015): a. Total number of eligible members by county and month b. Total number of paid trips, by month of service and type/level of service (LOS), including mileage reimbursement if utilized. If mileage reimbursement is included within the ambulatory category, please provide the percentage of ambulatory trips that are provided via mileage reimbursement. c. Total miles driven by LOS and month of service. Yes. An example of a non-state Agency is a County Agency. The State cannot elaborate on these trips. Local utilization is under cooperative purchasing partner agreement, and is not known to the State. Please refer to revised Exhibits in Part 2 of this document. Please refer to revised Exhibit E in Part 2 of this document. Challenges have resulted in extra costs for the current Contractor and more difficult logistics. Please refer to revised Exhibit E in Part 2 of this document.

4 Background Background Background Background Background Background Table after 4: Can the Agency at a minimum provide the Specialty and Out of State transports for the latest fiscal year (July 1, 2014 thru June 30, 2015), including air ambulance? What is the expected timeline for the State to make sure that the selected vendor has at least 120 days to perform start-up for the new contract? The current contract expires 6/30/16. The RFP proposal is due 2/10/16 with start date to be 120 days after award. The award will have to be completed by 2/28/16 to accomplish a start date of 7/1/16. Is this timeline accurate? The data table referencing out-of state transports uses five months of data from 2014, please provide an additional table that includes the following additional data points: average trip distance, how many trips also include lodging and meals reimbursement by state (NY, PA and DE), trip by level of service provided by state (NY, PA and DE) and average blended cost for trips by State (NY, PA and DE). Paragraph 2: Please provide all of the authorized trip data for all types of trips as outlined by category in Exhibit E for all months from May 2014 to the present. The current vendor should have reported this information monthly and it is required to submit a competitive bid for the response. As stated in the RFP, The location of providers and their practice sites may include, but is not limited to, practitioner offices, hospital outpatient departments, independent clinics, dialysis facilities, ambulatory surgery centers, and federally qualified health centers. Where could additional providers and locations be? Please refer to Part 2 of this document for Exhibit I, Attachment Out of State Trips. Does New Jersey have a law about the position of the wheelchair ramps for service vehicles, i.e.: rearloading or side-loading? Will the Department accept hand delivery of proposal responses, i.e., Fed Ex? Is the bidder required to submit a response through the Electronic Bidding (EBID) application? The RFP lists Ground Ambulance Service (GAS) as a mode of transport, along with Basic Life Support (BLS) and Specialty Care Transport (SCT), on pages 5 and 22. The definition is provided on page 17. Please explain the difference between GAS and BLS? Based on the size and scope of this contract, would DHS consider extending the implementation period to 180 days to ensure a smooth transition for non-incumbent vendors? The link for forms for this RFP is not valid. Please provide correction for: hase/bid/summary/17-x shtml Does the State have a file size limitation? May the electronic copy be submitted on a USB drive instead of a CD or DVD? The RFP does not provide details about how to submit a redacted copy of the proposal. Will the State please clarify where contractors can specifically address redaction and how to submit redacted copies? Section 2- Page limited to 25 pages. Is this page limitation 25 double-sided pages or is this 25 single-sided pages? Can the bidder use a 10 point font in graphics and tables? We recommend allowing the bidder to use a smaller font as long as the readability is not affected. Please refer to Part 2 of this document for Exhibit I, Attachment Out of State Trips. No. Yes. These sections appear very similar; could DHS more fully explain the difference between the two sections? Based on the page limitation, would it be possible to combine these two sections? Please confirm that the schedules and plans to be submitted for these sections can be submitted as attachments and therefore will not count against the 25-page limit for section Please clarify what the State means by contract schedule? Is the contract schedule only to cover the implementation period, or are you looking for a contract schedule that covers the entire 5 year period till transition. Yes. 7 Contract Schedule, Mobilization and Implementation Plan Resumes Please clarify the differences between the content of Contract Schedule and the Mobilization and Implementation Plans to be submitted. Please confirm we do not need to include resumes for transportation providers. The Mobilization and Implementation Plan covers the period one-hundred and twenty (120) calendar days from contract award. The Contract Schedule covers the contract period as set forth in RFP Section 5.2 Contract Term and Extension Option. The Bidder should provide adequate insight into the Bidder s Ability to Mobilize and Implement and complete the contract. Confirmed, resumes do not need to be included for transportation providers.

8 Financial Capability of the Bidder Financial Capability of the Bidder Considering the RFP due date is Feb 10th, most companies (public traded or private) would not have their audits completed for their December 2015 year end and since the cost associated with adding a review and preparing financial statements and footnotes is considerable while already incurring the cost of a full audit, not to mention the number of human resource folks needed to close out a full year in 30 days, can you please consider the following: a) If the bidding entity has multiple years of certified audits (more than two years), can it be allowed to provide the 2014, 2013 and 2012 audited financial statements with the understanding that full audited financial statements will be provided by either March 15, 2016 (after earnings release) and or date of actual contract execution? or b) In the case of a public company where audited financial statements cannot be released prior to earnings release due to SEC rules, would it be possible to provide the last quarter unaudited statements (Sept 30, 2015) for the bidding entity and a bridge to the publicly released Parent company 10-Q filing, which includes a Balance Sheet, Income Statement, Statement of Cash Flow and Footnotes identifying the bidding entity as a separate segment in these already released financials, also with the understanding that the bidder will provide full financial statements for the most recent year by March 15th or as soon as the companies earning release is made public? Given the timing of this RFP, an entity with a calendar year end will not have yet completed an audit of the most recent year nor have a completed "reviewed or compiled statement." Will it be considered deficient to provide a trail balance of the most recent year and audited statement of the previous year? A&B: In absence of audited/ reviewed/ compiled financial statements for the current year, prior year certified financial statements are acceptable. The Bidder may provide year-todate (YTD) unaudited financials, however, certification from Bidder s authorized management shall be included.

Please clarify who will have the right to final proposal acceptance. Is there a detailed evaluation criteria/scoring sheet, including possible points per evaluated section, that the reviewers will be using available to RFP respondents now? If so, please provide. We would like to write to those criteria as a way of ensuring we are fully responsive to each question. Please explain how the proposals will be evaluated? Will there be an area in scoring for possible Contractors that will illustrate a company s financial and resource strengths, ability to perform for the duration of the contract and that the company is not over extending itself either financially or having adequate resources? What process will the state undertake to determine the financial strength and resources a company has to undertake a project of this size?
