CEO Greater Ouachita Water Company PO Box 1257 West Monroe LA, Dear Mr. Sparks: Severn Trent Environmental Services is pleased to present the attached Monthly report. Included is a detailed report of operation and maintenance of GOWC Wastewater, Water Treatment Plants and Pump Stations and, Billing and collections. 3 Executive Summary Most quality issues during the month of January have been on the purchased water systems where water is bought from the City Of Monroe and sold to GWOC customers. This is due to the City of Monroe s annual Chlorine Burn in wrapping up. Their anticipated stop date is January 30, 2016 and will return to Chloramine disinfection. During this time GOWC has been working with Corona Environmental to install automated controls to boost disinfection level to stay at the required levels throughout the systems. These have been installed on the North Monroe system and are now in service. The controls system are being installed on the Lakeshore system and expected to be operation by the end of March Water main replacements are in full swing in the Walters st. areas West Monroe, and in the Tangle wood Subdivision in Monroe. These are both areas that are plagued with main breaks due to very old and brittle lines. Both projects are nearing completion and will provide better services to the customers in those areas. During the month of January 2016 we had 1108 Various Work orders that required staff to go out and follow up. These include reconnects, new services, water leaks, meter rereads, service disconnects, and of those work orders we had reported 17 water quality calls. Along with the normal work orders we conducted 580 line locates throughout the parish. Field services called in 64 locates for our own work to repair 46 leaks, repaired 2 Fire Plugs, terminated 3 service taps and 18 new service taps completed. Operations Highlights WWTP: Millhaven Lagoon 1. Adjusted aerators Country Heritage WWTP 1. Repaired Blowers 2. Replaced blower motor Valley Hill WWTP 1. Repaired aerators Heritage WWTP 1. Replaced some grating on the catwalks of the plant Diamond WWTP 1. Cleaned plant clarifiers 2. Cleaned effluent structure 3. Repaired sludge piping Presidential WWTP 1. Replaced piping 2. Installed new pump on plant 3. Replaced a breaker on the control panel Tangle Wood WWTP 1. Adjusted aerators US 165 South Region WWTP 1. Repaired aerators Monthly Report Page 1

Replaced overloads on the Clarifiers 3. Replaced float on lift station WTP: Arkansas Road WTP 1. Worked with contractors to get the new GAC filtration online 2. Marked all valves taking the old sand filters offline and converting to Carbon. HWY 80 WTP 1. Replaced chlorinator pump 2. Worked with contractors to get the new GAC filtration online 3. Marked all valves taking the old sand filters offline and converting to Carbon. Riser St. Well 1. Replaced communication chip on the Missions box Spell ST Well 1. Replaced communications chip on the Missions box 2. Replaced a blown transformer on the well control board Oliece St Well 1. Replaced communications chip on the Missions box Tichelli WTP and well 1. 5 Monticello LS, changed belts and adjusted pumps Shadow oaks LS, cleaned out station and pulled and cleaned pumps Frenchman s bend #3 LS, repaired pumps WOIP #4 LS, raised motor base to prevent flooding Coon trail LS, Replaced capacitors and checked pump, repaired rails Joe White LS, Repaired pump controls WOIP #4 LS, repaired pumps Faith Drive LS, pulled and cleaned both pumps Tangelwood LS, replaced belts Treasure island, repaired electrical controls on pump Eastwood LS #2, Repaired phase box and replaced capacitor due to lightning strike Lincoln park LS, adjusted belts Thatcher point LS, Changed fuses out Field Services Field crews completed 18 new service tap Fixed 46 water leaks 3 killed service taps 1 sewer tap 2 fire plug repairs Billing and Collections Month end closing had 22,857 customers and billed for 119,855,000 gallons of water throughout GOWC systems. Total of 314 new customers Total of 247 customers closed out Technical Services Line replacements in the Tangle Wood subdivision Designs are complete for Carbon instillation on the Brownsville system and is anticipated to start construction in early Line replacement on Walters and Lynette is underway and is slightly behind schedule but nearing completion Environmental Information Rainfall for January 2016 totals throughout the Parish was a total of 4.72 inches. In Comparison to January 2015 had an total of 4.91 inches Safety Performance October all GOWC plants, we had 0 LTIs, 0 OSHA recordable injuries, 55 Behavioral KPIs (Near Misses, Unsafe Conditions and Indications of Interdependent Culture), and 0 vehicle incidents. There were 68 hours of in-house safety training. Safety topics were on electrical safety and chlorine safety. What is a KPI? Severn Trent finds that awareness of our surroundings is a key to promoting safety. Each month, each employee is to find a condition - whether at work Monthly Report Page 3
6 or at home - that is unsafe or a near miss and report it to the safety coordinator. Taking immediate corrective action is an important step in this process, and this is one of the ways we prevent potential accidents.
