3 1) Purpose 6 NYCRR Part 596 Hazardous Bulk Storage Regulations requires the owner or operator of any facility with an above ground tank storing a hazardous substance with a capacity of one hundred eighty five gallons (185) or greater must register the tank and prepare and maintain a spill prevention report for preventing and responding to spills, releases and accidents at the facility. 2) General Information a) Name of Facility: Lift Station, (LS-1A) CBS Registration # b) Facility Owner: Village of Clayton 425 Mary Street Clayton, NY c) Facility Operator/Response Coordinator: Jeff Overstrom (DANC) d) Facility Phone: (315) , Operator s Cell Phone e) Nearest water body: St. Lawrence River feet 3) Report Certification and Approval a) Engineer Certification This Spill Prevention Report (SPR) has been developed under the guidance of a registered professional engineer in accordance with the standard and care typical of good engineering practices to meet or exceed the provisions required by 6NYCRR Parts 595, 596, 597, 598 and 599. The Village of Clayton is committed to the implementation of this plan as described herein in order to prevent accidental discharges of chemicals to the environment. Upon request by the department, an owner must provide evidence of financial responsibility for corrective action and for operating, maintaining or closing storage tanks. Norma Zimmer, Mayor 3
4 4) Facility Description LS-1A is as sewage pump station that receives untreated wastewater from the Cedar Point State Park pumping station and Cape Vincent Correctional Facility pumping station. To minimize the generation of hydrogen sulfide in the 6.6 mile forcemain between this liftstation and the Village s Wastewater Treatment Plant (WWTP) the sodium hypochlorite is injected at LS-1A. The pumping station has a separate building for storage of sodium hypochlorite. The tank and secondary containment are crosslinked polyethylene tanks with the primary tank being vented to the outside of the building. The grades surrounding the facility generally slope to the southeast. A major water body the St. Lawrence River lies approximately 1,600 feet to the North. 5) Summary and Leaks, Spills and Releases There were no leaks spills or releases at the LS-1A pump station since the station went online in 2007, as determined by a review of existing records for the facility. 6) Inspection and Records The following inspections must be completed and files maintained as noted. Daily Inspections A daily inspection is required by 6NYCRR Part 598.7(a) of the tank and piping, for spills, leaks, that drains are closed and that there are no unpermitted discharges. The recommended form is found in Appendix C. Annual Inspections - The Operator shall perform annual comprehensive inspections as detailed in the annual inspection form located in Appendix C. Annual Inspections will be kept with this report for a minimum of 5 years from the date they were created. Annual Status Report on Compliance - This report is prepared following the Annual Inspection. A template to aid in completing this report is provided in Appendix B. Annual Status Reports will be kept with this report for a minimum of 5 years from the date they were created. Five Year Inspection - Facilities with Tanks less than 5,000 gallons that are underlain with an impervious barrier are required to perform at a minimum, a thorough external inspection every 5 years. Five year inspections will be kept with this report for a minimum of 10 years from the date they are created. Clayton s tank is 520 gallons and meets these requirements. 7) Tank Information and Deliveries a) The operator or designee, when on the premises or when in control of a hazardous substance transfer, is responsible for transfer activities. If the operator is not on the premises or is not in control of a hazardous substance transfer, the carrier is responsible for transfer activities. 4
5 b) Tank 1 has a design capacity of approximately 520 gallons and a working capacity of 500 gallons. This tank shall not be filled above the working capacity labeled on the tank and at the fill point. c) Prior to the delivery, the operator or carrier must determine that the receiving tank has available capacity to receive the amount to be transferred and all tank valves and flow control devices are in the appropriate position to accept delivery. d) Throughout the entire period of transfer and while the tank is connected to the loading device, the operator or carrier must at all times supervise, monitor and control the transfer to prevent overfilling and spilling. e) The operator or carrier must be trained in the proper transfer procedures, must monitor and control the delivery and must take immediate action to stop the flow when the working capacity of the tank has been reached or should an equipment failure or emergency occur. f) If a leak is discovered during the transfer, measures must be taken immediately to stop the leak and clean up the material which has been leaked. 8) Spill Response Plan A Spill Response Plan (SRP) must be included in the Spill Prevention Report and shall be reviewed and updated annually. a) Initial Response (1) Assess the scene carefully to determine if it is safe. If emergency response personnel are needed call 911. (2) Restrict access and keep people away from the spill. (3) Prevent further release if it is safe to do so. b) Spill Reporting Procedures Contact the facility response coordinator and be prepared to answer the following Name of material spilled Location of spill Approximate amount spilled Time of discovery Has release been stopped Weather conditions Any immediate receptors (persons, surface waters) 5
6 The facility response coordinator or designee shall report all spills of sodium hypochlorite solution greater than 62 gallons 1 to the New York State Spill Hotline within 2 hours of the release. An Emergency contact list is located in Appendix D. c) Health and Safety Considerations A Safety Data Sheets (SDS) for Sodium Hypochlorite is included in Appendix E. The SDS contains health and safety information associated with this chemical. Employees working with Sodium Hypochlorite are familiar with the SDS and trained on the dangers of this chemical through the Village s Hazard Communication program. d) Potential Spill Hazard Summary Potential Event Spill Description Potential Volume of Release Spill Rate Complete failure of a full tank Confined within secondary containment 500 gallons Instantaneous Partial Failure of a full tank Confined within secondary containment 1 to 500 gallons Gradual to instantaneous Tank Overfill Confined within secondary containment several ounces to several gallons Up to 1 gpm Delivery vehicle leak or failure Gravel driveway 1 to 8,000 gallons e) Flow prediction Gradual to instantaneous Spills from the facility are not likely to enter any watercourse. Immediately north of the facility is a small gravel pit that may receive runoff from a spill. Figure 1, LS-1A Site Map, depicts the area around the facility and provides arrows for the predicted direction of flow based on the grade. The grade in the vicinity of the facility generally slopes to the southeast where there are no immediate receptors. f) Spill cleanup considerations For spills or releases of less than 62 gallons dilute with water and absorb using a noncombustible absorbent. For spills greater than 62 gallons notify NYSDEC Spill hotline. 9) Training The Facility Operator will ensure that employees are properly trained in procedures for preventing transfer spills, overfill and releases. 1 Reportable Quantity calculated using 15.6 % Sodium Hypochlorite solution 6

Does building foundation show any signs of deterioration? All items marked with a yes will be described and documented with any corrective actions taken and attached to this inspection. Tank shall be removed from service if: The circumferential measurement is greater than the baseline (151 inches) by more than 2% or 154 inches. There is a two inch or more crack with a depth of more than 1/3 of the thickness of the tank shell in the area of the bottom 25% of the height of the tank;(tank thickness is a minimum 3/16 of an inch.) There is a four inch or more crack with a depth of more than 1/3 of the thickness of the tank shell in the area of the top 75% of the height of the tank.
