3 1. OPENING AND WELCOME The chairman welcomed all present to the meeting. Mr. Z. Mohamed 2. ATTENDANCE An attendance register was circulated and apologies noted. All 3. Corrections and approval of the Minutes of previous Meetings (August 2016) Minutes were reviewed and changes were noted. 4. ACTION LIST FEEDBACK: 5. Action Register Item 1: Not completed. Nosibusiso to obtain presentation and share with the committee Scribe Nosibusiso Mfuywa Item 2: DWS Water quality results to be captured and shared via the Reservoir website.. Capacity constraints resulted in May- July 2016 results not being shared at the forum. Nosibusiso Mfuywa Item 3: To be removed from the list. Item 4. Rephrase to Water Quality guidelines versus actual monitoring results to be presented at the next meeting Item 5. It was brought to everyone s attention that Metsimaholo Municipality needs to attend the Forum meetings as they are currently not sending

4 any representative to the quarterly forum meetings. DWS to engage with MLM (Mc Botha) to ensure availability at ltf rephrase action required DWS Item 6. Zain indicated that the still has to see the bank manager with Wally Kingeberf preferably at the 2 nd week of December 2016 in order for him to share with Forum the LTF financial status. Mohamed 7 Pollution Incidents All industry

5 7. Sharing of Pollution Incidents representatives 7.1 Industry Lethabo: 23 October 2016 Ferric chloride Pollution incident. Ferric Chloride leak from PVC resulted in approximately hundred liters of oil spillage. Rehabilitation completed and now Lethabo is dealing will contaminated soil disposal. Sasol Industry: Sewage works is overloaded and there are currently Bioworks problems as the sewage works takes a lot of effluent from Municipality and Industries. The reason to this,is that it was noted that a lot of storm water is getting into the system.this causes problems as the system was meant to only take sewage. 7.3 Community/NGO s No representation 7.4 Department of Water and Sanitation To enquire about communication strategies from Drought.
