9 NPRA Safety Awards Program The presentation of the NPRA Safety Award plaques is part of a comprehensive safety awards program which the Association s Safety & Health Committee has developed to promote accident prevention in the petroleum refining, petrochemical manufacturing, and contracting industries and to publicly recognize the excellent record of safety in operations which the industries and contractors have achieved. NPRA Safety Awards are based on records kept for employees in accordance with OSHA record keeping requirements as defined by law and entered on the OSHA 300A summary form. The Safety Awards Program honors NPRA Regular member companies operating U.S. refineries and petrochemical manufacturing plants as well as Associate member contractors working in those facilities. The program consists of the following awards. 7
10 Awards Distinguished Safety Award The NPRA Distinguished Safety Award, (DSA) NPRA s most prestigious award, recognizes those member company refineries and petrochemical plants that have attained a superior level of safety performance in the domestic refining and petrochemical manufacturing industries. Recipients are chosen by a selection committee represented by members of the NPRA Safety & Health Committee. It is the selection committee s responsibility to carefully examine the safety performance records of individual plant locations using the specific screening and selection criteria detailed below. Screening Criteria In order to qualify for the DSA, a facility must first pass the screening criteria as described below: 2. A location must achieve a contractor total recordable incidence rate of 0.6 or less and no contractor lost workday cases involving days away from work, restriction or transfer (DART) for the evaluation period. 3. A location must have had no employee workplace related fatalities and no non-employee (contractor, visitor, etc.) workplace related fatalities for five consecutive evaluation periods. Selection Criteria After passing the above screening criteria, the candidate facility s overall safety performance record will be evaluated by the Selection Committee based on the applicants written application. 1. A location must achieve an employee total recordable incidence rate of 0.6 or less, no employee lost workday cases involving days away from work, restriction, or transfer (DART) or the equivalent of a Tier 1 Process Safety Event that results in days away and/ or a restricted case of an employee, contractor, or subcontractor, a fatality of a third-party individual, or an officially declared community evacuation or community shelter-inplace for the evaluation period.* * Evaluation period refers to a single calendar year. 8
11 Awards Meritorious Safety Performance Award This award recognizes facilities which have achieved an employee total recordable incidence rate of 0.8 or less with no workplace related fatality of an employee or non-employee or the equivalent of a Tier 1 Process Safety Event that results in days away and/ or a restricted case of an employee, contractor, or subcontractor, a fatality of a third-party individual, or an officially declared community evacuation or community shelter-in-place during the calendar year Award for Safety Achievement 1,000,000+ Employee Hours This award acknowledges facilities for operating one million or more employee hours without an employee lost workday case involving days away from work, no workplace related fatalities of an employee or non-employee, and no incidents that are the equivalent of a Tier 1 Process Safety Event that results in days away and/or a restricted case of an employee, contractor, or subcontractor, a fatality of a third-party individual, or an officially declared community evacuation or community shelter-in-place during the calendar year Award for Safety Achievement 1+ Years This award acknowledges facilities for operating one or more years without an employee lost workday case involving days away from work, no workplace related fatalities of an employee or nonemployee, and no incidents that are the equivalent of a Tier 1 Process Safety Event that results in days away and/ or a restricted case of an employee, contractor, or subcontractor, a fatality of a third-party individual, or an officially declared community evacuation or community shelter-in-place during the calendar year Contractor Award for Meritorious Safety Performance This award recognizes those contractors working a minimum of 20,000 hours per calendar year at a Regular member facility who have achieved a total recordable incidence rate of.08 or less with no workplace related fatality during

13 NPRA Safety Award Recipients The 19th Annual Safety Awards Dinner honors 44 Regular member companies represented by 58 refineries and 48 petrochemical plants with a grand total of 217 awards. Forty-one Associate member companies are recognized for their excellent safety performance with 372 awards for their efforts in 120 facilities.
