Recipients are chosen by a selection committee composed of members of the AFPM Safety & Health Committee. It is the DSA Selection Committee s responsibility to carefully examine the safety performance records of individual plant locations using the specific screening and selection criteria detailed below. See page 2 for screening criteria. ELITE GOLD SAFETY AWARD This award is presented to the top one percent of member company refineries and petrochemical plants that have exhibited superior safety performance and program innovation as determined by the DSA Selection Committee. 4 DISTINGUISHED SAFETY AWARD Screening Criteria In order to qualify for the DSA, a facility must first pass the screening criteria as described below: 1. A location must achieve the following requirements for the calendar year: a. an employee total recordable incidence rate (TRIR) of 0.3 or less, b. a contractor total recordable incidence rate (TRIR) of 0.3 or less, 2. A location must have had no employee workplace related fatalities and no non-employee (contractor, visitor, etc.) workplace related fatalities for the previous five consecutive years. A location must answer no to the following questions: 1. Did the location experience a community evacuation or sheltering event, or worker/ contractor hospitalization (admitted for treatment) during the evaluation period? 2. Did the location experience a recordable injury where any of the following caused or contributed to the injury during the evaluation year? Failure to isolate hazardous energy (lockout/tagout) Opening process equipment not properly prepared for maintenance Improperly prepared or hazardous confined Space Improperly prepared or hazardous excavation Any falls from height (>4 feet) Any Tier 1 or 2 Process Safety Event (PSE) Contact/exposure to hazardous materials (physical and chemical) H2S, HF, Hydrocarbons, Corrosives, Electricity, Radiation, Excessive Heat, etc. Any crane or rigging failure 3. Did the location have any API RP Tier 1 Process Safety Events (PSE) that required notification to the National Response Center during the evaluation year? Selection Criteria After passing the screening criteria, the candidate facility s overall safety performance record will be evaluated by the selection committee based on the applicant s written application. Those sites that are in the running for the DSA and Elite Gold awards participate in in-person interviews with representatives of the DSA Selection Committee. SAFETY ACHIEVEMENT AWARD (OPERATING MEMBERS) Screening Criteria In order to qualify for the Safety Achievement Award, a facility must first pass the following screening criteria. 1. The awards are part of a comprehensive program developed by the AFPM Safety and Health Committee to promote safe operations in the refining and petrochemical industries and to recognize facilities with outstanding occupational and process safety records. In our industries, safety is paramount, and something in which we take great pride and hold ourselves to the highest standards, Chet Thompson, president and CEO of AFPM noted. He added, The U.S. refining and petrochemical industries continue to have the lowest rates of injury and illness among major industrial sectors and are leading the way in process safety innovation and practices. The recipients represent leaders within our industries, and we are pleased to acknowledge these facilities for their excellent safety records, proactive programs, and impressive safety cultures. These safety awards recognize their outstanding work and exemplify the gold standard for others to emulate. This year s most prominent award, the 2017 Distinguished Safety Award, has been awarded to three facilities: the ExxonMobil Chemical Company-Baytown Chemical Plant, LyondellBasell Industries-Bayport Complex, and Phillips 66-Bayway Refinery. The Elite Gold Award, which recognizes facilities with safety performance in the top one percentile, went to the LyondellBasell Industries-Chocolate Bayou Polymers Facility, LyondellBasell Industries-Clinton Complex, and Phillips 66-Sweeny Refinery.
