2 DMU Project Officer to audit DMU Project Officer to audit 01. Slips and trips Tools, plant and equipment from contractor can be a slip and trip hazard. Trailing cables, Uneven floor surface, Spilled liquids, Poor housekeeping Site / work zone to be appropriately segregated with barriers / heras fencing. No trailing cables at any time, Floor to be maintained in good condition throughout project. Spills to be cleaned up immediately, to keep work area tidy. 02. Work at Height Falls from height, Work at height to be subject to a risk assessment. Work equipment to be selected in accordance with the Work at Height Regulations. All equipment to be used and inspected by a competent person. Any site specific issues to be identified by DMU / contractor at tender / quotation. not to use DMU equipment without permission from the project officer. All work on an unprotected roof is subject to a PTW. Appropriate edge protection, toe boards, netting etc to be used. 02. Noise Nuisance noise from contractor Work to be conducted and programmed to minimise noise levels and disruption, DMU staff in surrounding area to be notified. Allowable noise levels will depend on time of day, day of week, neighbours, internal or external work. Residential area etc Lighting Levels Reduced lighting in area due to work Local users to be advised and relocated or task lighting provided. to provide task lighting for contractor area as required Access / Egress Restricted access / egress for DMU staff. Unauthorised access to site. Signing in / out Staff to be relocated if safe access cannot be gained. If access / egress routes are changed faculty / dept lead to be informed before the change and notices posted. Barriers / heras fencing / hoarding to be erected in accordance with the nature of the project. On arrival the contractor must report to the ESB reception and sign in. Upon signing in the contractor and all employees will be issued with site rules. On departure the contractor and their employees must report to the ESB reception and sign out. All keys must be returned when signing out. 08. Confined Spaces Confined space working can lead to a loss of consciousness caused by an increase in body temperature or, loss of consciousness or asphyxiation caused by gas, fumes, vapours or the lack of oxygen. Drowning from inflowing liquids or asphyxiation or entrapment by free flowing solid (e.g. lime, polyelectrolyte etc) All work in a confined space to be subject to a detailed / specific risk assessment and method statement. All work in Confined Spaces must be carried out under DMU Confined Space Work Permit Procedure, including paired working, rescue arrangements, ventilation etc

Permission for skips must be gained from the DMU project officer before a skip is brought to site. A skip must be lockable or enclosed in heras fencing compound and located a minimum of 3m from the building Plant and Equipment s equipment All plant and equipment brought to site must be suitable for purpose and maintained in accordance with PUWER. All machinery to be used by trained and authorised personnel. Guards and barriers must not be removed / defeated. 13. Plant and Equipment DMU owned machinery Permission to use DMU owned machinery must be requested in advance and will not normally be granted 18. Electric shock, fires and burns Isolations All electrical circuits to be isolated and locked off in accordance with the electricity at work regulations, HSE guidance and DMU local rules. Warning signs to be posted. Live working is not permitted on DMU electrical systems.

Overhead / underground cables Excavations: Contact with underground cables Excavation must be carried out by competent persons and planned appropriately, giving due account to ground conditions, weather and depth of excavation. No excavations using mechanical equipment will take place without full CAT scan. Manual excavation by hand is permitted without CAT scan if operation is carried out on the basis of around live services ; with use of CAT scan as preferable. DMU project officer to provide contractor with information on services (where known) before contractor produces their risk assessment and method statement. 23. Manual handling operations Musculoskeletal injuries to train operatives in Manual Handling techniques and safe practice. Manual Handling Assessments and inform operatives of personal limits. to consider all methods of alternative handling to reduce risk of injury by provision of lifting aids Lifting Equipment Cranes All crane activities are subject to a lifting plan which must be submitted before the crane is brought to site.
6 DMU Project Officer 25. Hazardous Substances Lead pigments (old paint mediums, old solvent mediums) If the presence of hazardous materials is suspected report to the DMU project officer. Tests to be conducted ascertain if hazard is present. Carry out the work in such a way as hazard is avoided or minimised. (e.g. stripping of paint with chemical strippers instead of burning off. DMU will notify contractor of any known or suspected hazardous materials likely to be encountered during the contract before work commences Hazardous Substances Asbestos DO NOT disturb any material containing or suspected of containing asbestos. If asbestos is detected or suspected cease work and contact the DMU Project officer. All contractors to have received a minimum of Asbestos Awareness training. If a Class 3 Asbestos Survey has been carried out for the works, the survey results will be provided to the contractor before work commences. Copies of the Class 2 asbestos survey are available to view at the ESB Reception at any time during normal working hours and should be viewed by the before commencing work or pricing work. Asbestos must only be removed by a licensed contractor. 25. Hazardous Substances Decontamination of area Area to be allocated as a contractor zone / area to be decontaminated before occupation and clearance handover certificate produced 25. Hazardous Substances Flammable Substances All highly or extremely flammable materials are to be secured/stored in appropriate flameproof containers outside buildings location to be agreed. When in use the minimum quantity should be in the building at any one time, and removed from the building when not in use

DMU provide additional copies of the DMU induction as required. For sites not handed over to the contractor sll contrctors staff to be inducted by DMU Project Officer Fire Fire routes blocked (As part of project) - Fire routes blocked (Unintentional) Alternative fire route to be provided, signage installed and local users informed. The contractor must ensure that all escape routes are maintained clear of all obstructions Fire Hotwork All Hotwork - inside and outside buildings to be subject to the DMU Hotwork permit, Appropriate fire extinguishers to be used / provided. Ihr fire watch to be conducted. Fire detectors to be impaired as appropriate Fire Fire; Smoke Inhalation, Burns, Property damage to ensure fire safety of contractor area / zone

If lone working is to occur contractor must have adequate controls for contact, support and emergency. Any out of hours working is subject to a PTW

The contractor is to ensure that their out of hours procedure and Assessment cover the increased risks Other Laboratories and specialized scientific facilities can contain radiological, biological and chemical hazards or energy emitting or producing equipment (e.g. lasers, microwaves, magnetic devises etc) Work in laboratories and other special areas, will only be permitted either after safety briefing by Area Safety manager, issue of work permit if required, Issue of Clearance Handover Certificate to confirm the area has been decontaminated and made safe for contractors. 27. Other Protection of the Public Segregation DMU is a busy city centre campus with over 23,000 students and 3,000 staff Barriers / heras fencing / hoarding to be erected in accordance with the nature of the project. 27. Other Traffic routes, routes, Reversing vehicles Pedestrian Deliveries, Where practicable there is to be complete segregation of vehicles and pedestrians with separate walkways and appropriate barriers / fencing. to produce a vehicle pedestrian plan in conjunction with DMU project officer in relative to the project and degree of risk. Consideration must be given to: loading, one way systems, reversing, banksman, speed limits (5mph on DMU property), deliveries. Roadways and footpaths to be kept clean and clear from mud and other debris
