1 ADDENDUM NO. 4 RFP-DOT-14/ TB FLORIDA DEPARTMENT OF TRANSPORTATION ADDENDUM NO. 4 DATE: August 18, 2014 RE: RFP NUMBER: RFP-DOT-14/ TB RFP TITLE: MAINTENANCE AND REPAIR OF FDOT VEHICLES AND EQUIPMENT AT THE BROWARD OPERATIONS CENTER. QUESTIONS AND S QUESTION 1. Small Equipment- Can you provide us a list of the approximately 120 pieces of small equipment. See Attachment J BELOW - Copy of Small Equipment Inventory. QUESTION 2. Utilities - Internet Will FDOT provide an internet connection in the maintenance facility? Vendor must make his own arrangements for internet access on his own computers. DOT computers will have internet access. QUESTION 3. Utilities-Solid Waste-Will FDOT provide solid waste removal/service? This question does not apply to hazardous waste. Vendor must make his own arrangements for dumpster/waste removal at his expense. QUESTION 4. Generators is an annual load test required? Yes. QUESTION 5. New unit up fitting Is the requirement for the contractor to provide first aid kits and fire extinguishers part of the contract price or is this Special Services expense? Under Basic Services. However, existing first aid kit contents that become expired may be obtained from the FDOT warehouse without cost to the vendor. 1

2 ADDENDUM NO. 4 RFP-DOT-14/ TB QUESTION 6. Can you provide us a detailed report of the expenses that were billed to the Special Services account for the last 12 months? Please note the general category of the expense and the amount paid in each of the categories. Categories could be tires, towing, internal non-contract labor, accidents, direct work, sublet work, etc. Out of the Special Services Budget: Directed Work; 72%. Accidents; 5%. Towing; 1%. Tires; 13%. Extended Life Repairs; 6%. Emergency Work; 0%. QUESTION 7. What are the current hours and days of operation for the shop? Monday through Friday, 7:00 AM to 5:30PM. Also refer to Scope of Services, page A-9. QUESTION 8. Attachment F provides PM intervals for equipment under 1 ton, can you provide us the PM intervals for all other equipment listed on Attachment F? PM schedules can be found in the Mobile Equipment Manual. QUESTION 9. Can you provide the incumbents current staffing levels that are on-site for the performance of the contract? Please provide headcount and job title/classification for each of the employees. They have the shop manager, an office / clerk position, and four mechanics (6 people total). QUESTION 10. FLEET QUESTIONS Unit DOT07790, Fleet Class 8854 is listed on F18458 ATTACHMENT E2 EXCLUDED INVENTORY, but is listed on F15481 EXHIBIT C Price Proposal (in category 9400 < 9475 Core Drill Trucks). Should a Basic Services Monthly Cost be entered on the Price Proposal for Unit DOT07790? Should be a part of Excluded Inventory; no Basic Services. 2

DOT Downing RPM DISP D Striping and Mixers TRL MTD Please provide a description of this unit This is a unit that mixes and dispenses adhesive for the application of raised pavement markers (RPM s). Vendors MUST acknowledge receipt of this Addendum by completing and returning to the Procurement Office, by no later than the time and date of the replies due date. Failure to do so may subject the vendor to disqualification.
