3 SKILL SHEET # 1 Preventive Maintenance Performance Evaluation: The evaluator shall verify the candidate has the ability to perform and document routine tests, inspections, and servicing functions on the systems and components specified in the performance steps, so that the operational status of the vehicle is verified and deficiencies are reported. JPR: 4.2.1, 4.2.2, NFPA 1002, 2014 Edition Instructor s Statement Tasks: Verify the operational status of the aerial apparatus and report any deficiencies. The candidate, given a fire department aerial apparatus, it s manufacturer specifications, maintenance and inspection forms, policies of the jurisdiction, shall perform and document routine tests so that the listed performance steps are completed. Equipment Required: Fire department aerial apparatus, apparatus manufacturer specifications, maintenance and inspection forms, policies and procedures of respective jurisdiction. Critical Skills are identified by a bold type C. Any candidate that fails to complete a critical skill shall receive an unacceptable completion for the entire Practical Skill Sheet. Safety: A safety violation is grounds for automatic failure. All proctors present shall review the safety violation. STEPS FOR SKILL COMPLETION 1. Identifies correct hand tools for use during inspection; uses appropriately. C 2. Inspects tires, sufficient tread and inflated to manufacturer s specifications. C 3. Inspects batteries as clean, corrosion free, appropriate electrolyte levels, and secured. C 4. Inspects engine oil, fluid level within manufacturer s specifications. C 5. Inspects fuel level, fuel above ¾ tank. C 6. Inspects coolant fluid level; fluid level is within manufacturer s specifications. C 7. Inspects belts for tightness and excessive wear (engine not running) C 8. Inspects braking system for air/fluid leaks. C 9. Inspects steering system linkage for wear, fluid level within manufacturer s specifications. 10. Inspects fluid level in transmission, fluid level within manufacturer s specifications. 11. Inspects pumping systems. C 12. Inspects electrical system, all running and emergency lights are operational, electronic audible devices are operational, voltage regulator is operational. C 13. Inspects water tank and other extinguishing agent levels. C 14. Inspects foam systems (if applicable) C 15. Inspects breathing air systems: adequate air in storage cylinders, inspect cylinders, gauges, hoses, regulator, connections, confirm all cylinders are within current hydrostatic Initial Retest Yes No Yes No

5 SKILL SHEET #2 (if applicable) Vacuum Test Performance Evaluation: The evaluator shall verify the candidate has the ability to perform a primer, seals and tightness test so that the apparatus pumping system is evaluated. JPR: NFPA 1002, 2014 Edition Instructor s Statement Tasks: Perform a primer, seal and tightness test. The candidate, given a fire department aerial apparatus equipped with a pump, it s manufacturer specifications, maintenance and inspection forms, and policies of the jurisdiction, shall perform and document a primer, seals and tightness test so that the listed performance steps are completed. Equipment Required: Fire department aerial apparatus equipped with a pump, apparatus manufacturer specifications, maintenance and inspection forms, policies and procedures of respective jurisdiction. Critical Skills are identified by a bold type C. Any candidate that fails to complete a critical skill shall receive an unacceptable completion for the entire Practical Skill Sheet. Safety: A safety violation is grounds for automatic failure. All proctors present shall review the safety violation. STEPS FOR SKILL COMPLETION 1. Ensures apparatus is in park, brakes engaged, wheels chocked. 2. Drains water from the pump. 3. Closes all valves on the pump and caps steamer connection on the intake manifold. 4. Operates primer to achieve a dry vacuum on the pump (18 to 22 of Hg). 5. Shuts engine down and sets a timer for 5 minutes. 6. Walks around apparatus/pump; listens for potential air leaks, looks for signs of air leaks. 7. At the end of required time, checks master intake gauge for amount of remaining vacuum (at least 10 Hg). 8. Opens strainer and steamer cap to check for water in the pump. C 9. 6 SKILL SHEET #3 Pressure Control Device Test Performance Evaluation: The evaluator shall verify the candidate has the ability to perform pressure control device test so that the apparatus pumping system is evaluated. JPR: NFPA 1002, 2014 Edition Instructor s Statement Tasks: Perform a test on the pressure control device. The candidate, given a fire department aerial apparatus equipped with a pump, it s manufacturer specifications, maintenance and inspection forms, and policies of the jurisdiction, shall perform and document a pressure control device test so that the listed performance steps are completed. Equipment Required: Fire department aerial apparatus equipped with a pump, apparatus manufacturer specifications, maintenance and inspection forms, policies and procedures of respective jurisdiction. Critical Skills are identified by a bold type C. Any candidate that fails to complete a critical skill shall receive an unacceptable completion for the entire Practical Skill Sheet. Ensures apparatus is in park, brakes engaged, wheels chocked. 2. Places apparatus in pump gear with engine running. 3. Opens tank to pump (tank suction) valve. 4. Opens tank fill valve completely. 5. Ensures pressure control device is in the off position. 6. Adjusts throttle to develop 150 psi on the master discharge gauge. 7. Correctly engages the pressure control device to maintain 150 psi on the discharge header. 8. Closes tank fill valve and checks for excessive pressure increase on the master discharge gauge. 9. JPR: NFPA 1002, 2014 Edition Instructor s Statement Tasks: Perform inspections of aerial device components. The candidate, given a fire department pumping apparatus, it s manufacturer specifications, maintenance and inspection forms, and policies of the jurisdiction, shall perform and document an inspection of aerial device components so that the listed performance steps are completed. Equipment Required: Fire department aerial apparatus, apparatus manufacturer specifications, maintenance and inspection forms, policies and procedures of respective jurisdiction. Critical Skills are identified by a bold type C. Any candidate that fails to complete a critical skill shall receive an unacceptable completion for the entire Practical Skill Sheet. Inspects turntable assembly; visually inspect drive pinion and turntable gear teeth for damage, proper meshing and alignment, evidence of wear and adequate lubrication. 4. Inspects lower control pedestal; checks components for signs of wear or damage, ensures all controls move freely and automatically return to their neutral position, checks electrical connections. 5. Inspects platform control console (if applicable). C 6. Inspects aerial device communications system. C 7. Inspects extension/retraction system; checks for signs of wear or damage, checks hydraulic cylinders for signs of damage, fluid leakage and secure connections. Inspects all surfaces that come into contact with cable system. 8. Inspects elevation/lifting cylinders; checks for signs of fluid leakage and secure connections. C 9. Inspects aerial device sections; checks all beams, base rails, hand rails, slides, rollers, locks, ladder rungs, alignment systems, and truss work for signs of wear, cracks in welds, Initial Retest Yes No Yes No
8 loose or missing parts, physical damage or improper alignment. Checks for signs of rust on steel ladders, chipped paint or oxidation corrosion on aluminum ladders (white powder). 10. Inspects elevating platform component if so equipped; railings, heat shields, kick plates, control pedestal, nozzles, gates/doors, leveling system. 11. Inspects piped waterway system if so equipped; checks for damage to piping connections and seals, signs of leakage, proper alignment, operation of pin system if applicable. C 12. 9 SKILL SHEET #5 Operational Testing of Aerial Device Performance Evaluation: The evaluator shall verify the candidate has the ability to perform operational testing of the aerial device so that the pumping system is evaluated. JPR: Tasks: Perform operational readiness testing of the aerial NFPA 1002, 2014 Edition device. The candidate, given a fire department aerial apparatus, it s manufacturer specifications, maintenance and inspection forms, Instructor s Statement and policies of the jurisdiction, shall perform and document operational tests so that the listed performance steps are completed. Equipment Required: Fire department aerial apparatus, apparatus manufacturer specifications, maintenance and inspection forms, policies and procedures of respective jurisdiction. Critical Skills are identified by a bold type C. Any candidate that fails to complete a critical skill shall receive an unacceptable completion for the entire Practical Skill Sheet. Safety: A safety violation is grounds for automatic failure. All proctors present shall review the safety violation. STEPS FOR SKILL COMPLETION 1. Ensures apparatus is in park, brakes engaged, wheels chocked. 2. Transfer power from the drive train to the aerial device hydraulic system. 3. Check to make sure the transfer indication light is on. 4. Deploy apparatus stabilizers, check for signs of damage or leaking hydraulic fluid. 5. Inspect apparatus to determine if it maintains balance, or leans to one side (indicating hydraulic leakage) once stabilizers are deployed. 6. Check indicator lights to ensure apparatus is safely stabilized. 7. Move the hydraulic system selector valve control to the aerial device position. 8. Elevate the device to full elevation. 9. Rotate the device, ensure operations are smooth with no unusual sounds while rotation is in progress. 10. Extend the device to maximum extension. Check for smoothness of extension, no unusual noises or binding of components. 11. Test operation of auxiliary equipment: remote control nozzle, video cameras, floodlights. 12. Test the emergency power unit according to manufacturer s recommendations. (note* main hydraulic system should be disengaged when testing EPU) 13. Stow the aerial device and stabilizers, prepare apparatus for road travel. C 14. Documents the performed actions and results. Total Initial Retest Yes No Yes No Score needed to pass 8
