1 GALESBURG-AUGUSTA COMMUNITY SCHOOLS REQUEST FOR PROPOSALS PROJECT: OWNER: SCHOOL BUS PURCHASE Bids Due by 5 p.m., Monday, January 18 th, 2016 Galesburg-Augusta Community Schools 1076 North 37 th Street Galesburg, Michigan INSTRUCTIONS AND PROCEDURES 1. Bids for the School Bus Purchase, as described in these specifications, will be accepted until Monday, January 18 th, 2016 at 5 PM EDT. 2. Bids will be accepted by mail or by personal delivery to Galesburg-Augusta Community Schools Administration Building at 1076 North 37 th Street, Galesburg, Michigan Should a vendor elect to mail bids to the Galesburg-Augusta Community Schools Administration Building, it is the responsibility of the vendor to ensure that the bid is delivered to the reception desk of the Superintendent s office at 1076 North 37 th Street, Galesburg, Michigan by the date and time indicated above. 4. All bids must be printed and placed in an opaque envelope with the following plainly marked in the lower left corner of the envelope: a. Text reading SEALED BID - SCHOOL BUSES b. the name of the bidder c. the date of the bid opening 5. All bids must be addressed to: Dania Bazzi, Superintendent Administration Office 1076 North 37 th Street Galesburg, MI Any bid submitted to the District may be withdrawn prior to the scheduled time for opening bids.

2 6. Bids received after Monday, January 18 th, 2016, at 5 PM EDT, and bids sent by fax or electronic mail will not be accepted. 7. It is the sole responsibility of the vendor to ensure that the bids are delivered to the Galesburg-Augusta Community School District by the stated date and time. 8. All bids must be submitted with the BID RESPONSE FORM (Attachment B) and must be signed by an authorized representative of the vendor. 9. Bids not submitted with the BID RESPONSE FORM or bids not signed and/or dated will not be considered and will be determined to be a non-bid. 10. A notarized copy of Public Act 232 of 2004, Familial Relationship Disclosure, (Attachment C) must accompany all bids. Bids submitted without the notarized Public Act 232 of 2004, Familial Relationship Disclosure, will be determined to be a non-bid. 11. A notarized copy of Public Act 517 of 2012, Iran-linked Business Disclosure (Attachment D) must accompany all bids. Bids submitted without the notarized Public Act 517 of 2012, Iran-linked Business Disclosure, will be determined to be a non-bid. 12. A notarized copy of a Non-Collusive Affidavit (Attachment E) must accompany all bids. Bids submitted without the notarized Non-Collusive Affidavit will be determined to be a non-bid % Bid Bond: A certified check or bank draft payable to Galesburg-Augusta Community Schools, or a satisfactory bid bond executed by the bidder and a surety company in the amount equal to not less than five percent of the maximum bid amount shall be submitted with each proposal. 14. The Galesburg-Augusta Community School District reserves the right to withdraw this Request for Bid at any time and for any reason and to issue such clarifications, modifications and/or amendments as it may deem appropriate. 15. The Galesburg-Augusta Community School District Board of Education reserves the right to accept any bid, waive any informality in or reject any part of a bid. 16. All bids shall be opened publicly on Monday, January 18 th, 2016 at 6:30 p.m. at Galesburg-Augusta High School (HS Media Center/Library). All bidders and other interested persons are invited to be present when the bids are opened. 17. All bids will be accepted until 5:00 p.m. on Monday, January 18 th, Bids received after 5:00 p.m. on Monday, January 18 th, 2016 will not be considered. 4 ATTACHMENT A GALESBURG AUGUSTA COMMUNITY SCHOOLS SCHOOL BUS PURCHASE Bus Bid Specifications Conventional Style School Bus 71/72 Passenger Bus Galesburg-Augusta Community Schools requests sealed bids on a per-unit price for a 71 passenger, conventional style school bus chassis and body. Requirements are for new models and or used no older than a 2014, as described in these specifications and must indicate warranty and miles on the bus. Minimum specifications are listed below. Each bidder must state immediately below the listed specifications (or on an attached page with reference to the specification line item number), the compliance details that pertain to the specification line item indicating size, quality, range and information as necessary to determine unit quality. Bidder specifications must meet, be comparable or exceed minimum specifications and must be listed in detail to be considered a qualified bidder. The "Iran Economic Sanctions Act" (P.A. 517 of 2012) makes an Iran-linked business ineligible to submit a bid on a RFP with a public entity.
