1 COE Installation Quality Bulletin Installation Quality Bulletin Number: To: All Installation Suppliers / Central Office Operations Representatives / Design Engineering Representatives / Sr. Technical Support Manager (formerly Quality Managers) / Field Engineering Representatives / Planning and Engineering Guidelines (formerly AMC Representatives) Date: Monday, August 15, 2011 From: CenturyLink Installation Quality Assurance Subject: New Standard CenturyLink Toll Equipment and Power Anchor Bolt Assemblies. Action Required: YES: NO: Description of Bulletin: The purpose of this Installation Quality Bulletin is to document the new standard anchor bolt assemblies approved for use within Toll/Switch/Power areas of CenturyLink Central Offices and equipment facilities. Effective 8/15/2011, the CenturyLink approved anchor manufacturer will

2 change to Seisco LLC and new orders will provide either the Toll/Switch Anchor Kit, SE12125, or the Battery Stand/Power Anchor Kit, SE12125-B. COE-FM Q&As will be revised to order the correct anchor bolt assemblies. Any remaining Hilti anchors (Refer to Installation Quality Bulletins and 06-06) will be utilized until the existing stock is exhausted, but will not be reordered. The new Seisco torque indicating expansion anchor bolt kits are designed for embedment in floor thicknesses 4 or greater. Where the facility floor design will not allow for installation of the CenturyLink standard Seisco anchor kits, contact the CenturyLink Real Estate representative responsible for the building to obtain a structurally safe custom solution for anchoring equipment frames, cabinets or battery stands. Approved Central Office Toll/Switch Floor Anchors Kit Part Number: Seisco anchor kit Part Number SE12125 Each kit includes (2) complete anchor bolt assemblies, square washers, and installation instructions. The manufacturer Common Language Material Code (CLMC) is BOYK. Approved Applications for Use: Anchoring all telecommunications equipment frames/cabinets/stanchions installed in Toll, Power, and/or Switch areas of the CO and equipment facilities. All Zone Heavy and Light earthquake zones. All floor thicknesses 4" (or greater). Adherence to the followin g Seisco Installation Instructions is mandatory: Installation Instructions For SEISCO Toll/Switch Floor Anchors (Part # SE12125) 1. Drill an insertion hole using an 18mm masonry drill bit to a depth 70mm (2-3/4 ) Note: Only use an 18mm drill bit, DO NOT substitute (Non-metric bits are not approved). 2. Remove dust and debris using a High Efficiency Particulate Arrestor (HEPA) vacuum equipped with an equally-rated HEPA filter. Dispose of floor debris per TP Par Anchors come preassembled. Refer to Figure 1 for the following instructions: a) Insert the expansion shield assembly into the hole and tap down until the top is flush with the floor.

3 b) Slide the cabinet into it s final position over the anchor holes. c) Insert the threaded rod flat washer and torque nut through the hole of the square plate and into the installed expansion sleeve assembly and hand-tignten. d) Using a ¾ socket wrench, position the torque setting to 60 ft. lbs. Tighten the torque nut until the top bolt shaft twists off. Note: The remaining intact red slip ring indicates that the anchor was placed to the proper torque. The top of the bolt shaft twists off suddently and gloves are recommended. Figure 1 Approved Battery Stand Power Room Floor Anchors Kit Part Number: Seisco anchor kit Part Number SE12125-B. Each kit includes (2) complete anchor bolt assemblies, round washers, and installation instructions. The manufacturer Common Language Material Code (CLMC) is BOYK. Approved Applications for Use: Anchoring all battery stand and power equipment frame/cabinets/stanchions installed in Power areas of the C.O. and equipment facilities. All earthquake zones. All floor thicknesses 4" (or greater). This power anchor does not allow shimming for uneven concrete floors in the C.O. If shimming is required, the Qwest approved Toll anchor bolt must be used.

4 Adherence to the followin g Seisco Installation Instructions is mandatory: Installation Instructions For SEISCO Battery Stand Floor Anchors (Part # SE12125-B) 1. Drill an insertion hole using an 18mm masonry drill bit to a depth 70mm (2-3/4 ) Note: Only use an 18mm drill bit, DO NOT substitute (Non-metric bits are not approved). 2. Remove dust and debris using a High Efficiency Particulate Arrestor (HEPA) vacuum equipped with an equally-rated HEPA filter. Dispose of floor debris per TP Par Anchors come preassembled. Note: Do not tap or strike the torque bolt shaft. Disassemble the anchor by removing the torque bolt shaft and flat washer. 4. Refer to Figure 2 for the following instructions: e) Insert the expansion shield assembly into the hole and tap down until the top is flush with the floor. f) Slide the battery stand into it s final position over the anchor holes. g) Insert the spacer sleeve into the anchor hole provided in the battery stand floor mounting flange and tap down until it s flush the top of the battery stand floor mounting flange. h) Place the plate washer and the flat washer over the hole and insert the torque bolt and hand-tighten. i) Using a ¾ socket wrench, position the torque setting to 60 ft. lbs. Tighten the torque nut until the top bolt shaft twists off. Note: The remaining intact red slip ring indicates that the anchor was placed to the proper torque. The top of the bolt shaft twists off suddently and gloves are recommended.
