Bidder: Signature: Date: Procurement: City of Glasgow does abide by Residential Bidder Preference per KRS 45A : KAR 5:400. This is designed to give in-state bidders an advantage against out-of-state bidders whose states have own bidder preference laws. Prior to awarding a contract, City of Glasgow must provide a preference to resident bidders of KY against non-resident bidders, IF those non-resident bidders are registered in a state that gives or requires its own residential bidder preference. Preference must be equal to preference given or required by state of non-resident bidder. If tie between resident and non-resident, preference must go to resident. Page 1 of 7

Late bids will not be accepted. No extension of the bidding period will be made. The City reserves the right to award a bid to multiple bidders when more than one item appears in a bid package. Total Bid: The City of Glasgow Glasgow Public Works reserves the right to reject any and all bids. Bids must be good for 90 days. In compliance with the above, the undersigned offers and agrees, if this bid is accepted within ninety (90) calendar days from the date of opening, to furnish any and all of the items upon which prices are quoted, at the price set opposite each items, delivered at the designated point(s) within the time specified in the schedule. Items on bid are exempt from federal excise tax and Kentucky sales and use tax. Title shall pass to the City upon actual receipt and acceptance of the items. 4 High-Speed Grinder Bid Specification It is the intent and purpose of this specification to describe the requirements of a high-speed grinder to be used at the City-owned landfill facility. The unit shall be new and unused, the latest model in production and shall have all standard equipment normally furnished on such models including the following: Product Information: Engine and Transmission: Engine location: Engine to be located on opposite side of machine from the mill to minimize dust and fines build up in engine area. Engine compartment: Engine compartment doors to include sound suppression material for quieter operation. Engine Accessibility: Engine compartment to be accessible from all four sides of machine with pivoting radiator to enable complete access to engine for maintenance. Additional door to provide access from above the engine. Engine Manufacturer: Mercedes-Benz or equivalent Type: OM 926 LA Horsepower: Fuel Consumption: 10 GPH - 12 GPH maximum Cooling System: Automatic Reversing Fan Systems for both engine cooling and hydraulic cooling system. Safety: Automatic shutdown activated by sensor monitoring of high coolant temperature, low coolant temperature, low engine oil pressure, low hydraulic oil level, high hydraulic oil level, high hydraulic oil temperature, high fluid coupling temperature and air filter restriction. Acoustic starting indication. Transmission: Hydrodynamic fluid coupling power transfer (no friction clutch) with v-belt drive.
