2 SAFETY PRECAUTIONS AND IRIS DIAGNOSTICS CONTACT INFORMATION SAFETY PRECAUTIONS: Make sure the customer observes all Material Safety Data Sheets and associated safety precautions when working with hazardous materials or when working inside the ichem VELOCITY System Analyzers. Make sure the customer observes all precautions when working with the ichemvelocity Automated Urine Chemistry Analyzer and when working around moving parts. CAUTION and SAFETY symbols are used in this document prior to steps in the procedure where customers must be informed about safety precautions. 3 Preparations before working with the ichemvelocity Analyzer Before You Begin: 1. Read and follow all instructions. CAUTION / SAFETY 2. Make sure the customer is informed about all safety precautions before beginning this procedure. Wear all required Personal Protective Equipment when working with the ichemvelocity Analyzers. (Example: Gloves, Lab Coat, Eye Protection, etc ). Turn off the instrument by turning off the front door power switch before opening the ichemvelocity front door. Make sure the customer does not operate the instrument with the door open.

4 When this procedure will be performed This procedure should be performed during the following condition: Customer reports CB control failure; false positive on one or more analytes (Example: CB false positive blood). AND The chemistry strips and controls have been replaced, but the customer still experiences the same error. When this condition is met, the recommendation is to perform the following: Replace the ichemvelocity waste filter If the customer does not have a spare waste filter, or if replacing the waste filter does not resolve the problem, perform the waste line flush Run controls. Verify status of control run.

5 UA Call Center Learning Objective The objective of this How To document to ensure Call Center Associates trained on the ichemvelocity Analyzer are able to effectively perform the following: Perform a Velocity waste filter replacement with the customer over the phone in a safe and effective manner. Perform a Velocity waste line flush over the phone with the customer in a safe and effective manner. Resolve CB control failures over the phone to reduce Emergency Service Call dispatches, improve the phone fix rate, and improve customer satisfaction.

Place paper towels under the waste filter. Slowly remove the tubing from the bottom waste filter fitting.

6 Removing and replacing the waste filter Note: If customer does not have a spare waste filter, go directly to slide number 8. Turn off the instrument from the front door power switch. Open the front door. Unscrew and remove the splash guard or front panel assembly. Place paper towels under the waste filter. Slowly remove the tubing from the bottom waste filter fitting. Slowly remove the tubing from the top waste filter fitting. Remove the existing waste filter. Install the new waste filter Dispose of the old waste filter. Reinstall the lower front panel / splash guard and close the front door.

7 Run CA/CB/CC Controls Make sure all old chemistry strips have been removed from the Strip Provider Module. Load new chemistry strips in the Strip Provider Module. Make sure that the desiccant is replaced when loading new chemistry strips. Pour fresh CA/CB/CC controls in positions 8, 9, and 10 of the control rack. Shield controls from light and allow to come to room temperature. Make sure there is an adequate amount of iwash solution in the external iwash bottle. Turn on the front door power switch. Make sure the instrument initializes and goes to Standby Mode. Run CA/CB/CC controls when the controls are at room temperature. Verify results. If CB controls fail in the same manner (CB false positive), proceed to slide number 8 and perform the Velocity waste line flush.

Enter Chemistry Service Mode - Make sure the instrument in Standby Mode.

CAUTION / SAFETY: Confirm with the customer that the instrument door is closed.

- Click on the Maintenance Tab - Click on Chemistry Service - The instrument will enter Chemistry Service

8 Enter Chemistry Service Mode - Make sure the instrument in Standby Mode. - Log Off the current Operator and Log On Operator as [IRIS]. The password is iris2k1 - Go Off Line. CAUTION / SAFETY: Confirm with the customer that the instrument door is closed. Inform the customer Do not open the door when performing this procedure. - Click on the Maintenance Tab - Click on Chemistry Service - The instrument will enter Chemistry Service Mode and the service screen will appear. Note: If the Chemistry Service mode screen does not appear, press the Windows key between the Ctrl and Alt keys on the keyboard. Then click on the Chemistry service icon on the task bar.

Perform waste line flush using DFS Home and DFS Wash Commands 1.When the Chemistry Service Screen appears, look under the Routines column and click on DFS Home. 2.

This takes approximately 5 seconds. Repeat the DFS wash command ten times. 4.Turn off front door power switch. 5.Click on the on top right part of the service screen to exit chemistry service mode. 6.

9 Perform waste line flush using DFS Home and DFS Wash Commands 1.When the Chemistry Service Screen appears, look under the Routines column and click on DFS Home. 2.Allow the DFS Home command to execute. This takes approximately 30 seconds. After the analyzer has executed the DFS Home command, under the Routines column, click on DFS wash. 3.Allow the DFS wash command to execute. This takes approximately 5 seconds. Repeat the DFS wash command ten times. 4.Turn off front door power switch. 5.Click on the on top right part of the service screen to exit chemistry service mode. 6.A pop-up box will appear stating to turn off the front door power switch before clicking o.k. Verify that the front door power switch has been turned off and click on o.k. After clicking o.k., the User Interface screen will appear. Note: If the Temperature button is clicked, the feedback should be around 60. This indicates that the front door power switch is turned on and there is power for the motors to perform the DFS Home and DFS Wash Commands.

10 Run CA/CB/CC Controls Make sure all old chemistry strips have been removed from the Strip Provider Module. Load new chemistry strips in the Strip Provider Module. Make sure that the desiccant is replaced when loading new chemistry strips. Pour fresh CA/CB/CC controls. Shield controls from light and allow to come to room temperature. Run CA/CB/CC controls when the controls are at room temperature.

11 Verify Status of Control Run Verify controls have passed. If controls passed, return the instrument to normal operating condition. If controls did not pass, the call will need to be dispatched.
