JONES II, INTERIM DIRECTOR OF ORKS DATE: NOVEMBER 19, 2013 SUBJECT: NOTICE OF COMPLETION FOR THE ANNUAL SIDEWALK REPAIR PROGRAM, FY (SUPPORTS 2013 CITY COUNCIL GOAL #4, PUBLIC INFRASTRUCTURE- CITYWIDE STREET MAINTENANCE) REVIEWED: CAROLYN LEHR, CITY MANAGER ~ Project Manager: Melissa Murphy, Associate Engineer ~ RECOMMENDATIONS 1. Accept the work as complete. 2. Authorize the City Clerk to file a Notice of Completion with the L.A. County Registrar Recorder Office and if no claims are filed within 35 days after recordation, and upon the contractor posting an acceptable warranty bond, notice the surety company to exonerate the Performance bond. 3. Authorize the Director of Public Works to release the 5% retention payment in the amount of $5, to John Simich Construction, Inc. 35 days after recordation of the Notice of Completion by the L.A. County Recorder contingent upon no claims being filed on the project, and the contractor posting an acceptable warranty bond. BACKGROUND Every year the City implements the sidewalk repair program where damaged sidewalk is repaired at various locations throughout the City. The City is sub-divided into nine maintenance zones and as requests are received regarding uplifted and/or damaged sidewalk throughout the year, these locations are inspected and then logged, and a database of repair areas is maintained until the quantity of repairs is sufficient to initiate the project. Increased focus was given to maintenance areas 3 and 5 for this project so that concrete repairs could be made in these areas before the streets were resurfaced. H-1

2 NOC for the Annual Sidewalk Repair Program, FY November 19, 2013 Page 2 DISCUSSION The project's scope included demolition of uplifted concrete sidewalk panels, root trimming, and pouring of new concrete panels. A few locations also included curb and gutter and driveway apron work. A great deal of roots were encountered during the root trimming process, which were underneath portions of the sidewalk and therefore not visible when the project was originally estimated. Nonetheless, due to the reasonable bid prices and expedient completion of the work by John Simich Construction, Inc., the project was completed within the time allotted and the amount budgeted, yielding a net savings of $2,082.60, enabling the necessary repairs to be completed in time before the resurfacing of the streets commenced.
