DEFECTS The Simard steering double knuckle arm on certain Sterling 8X4 front axles may not have been manufactured to the correct material specification and could fracture under normal loads. Letters will be sent to affected owners. Owners are advised to contact their authorised Mecedes-Benz dealership to have the steering arm inspected and, if necessary, replaced free of charge. AUSTRALIA Isuzu N Series vehicles PRODUCT DESCRIPTION N Series vehicles produced in Japan between October 2007 and October 2010, fitted with MYY/MZZ transmissions. IDENTIFYING FEATURES See attached ISOVIN list. Campaign number: 11-Z-03 TARGET NUMBER: 8,116 SUPPLIER Isuzu Australia Limited HAZARDS Reverse lamps and reverse buzzer (where equipped) may become inoperable. DEFECTS The positioning of the wiring harness bracket can induce a resonance in the wiring harness. The resonance can potentially cause the wiring harness to disconnect and render the reverse lamps and reverse buzzer (where equipped) inoperable. Owners will be contacted by Isuzu Australia Limited directly by registered letter. Owners are asked to contact their dealer as soon as possible for inspection and reposition of the wiring harness and replacement of the reverse switch where necessary. 3 AUSTRALIA Toyota Lexus RX400h (MHU38) PRODUCT DESCRIPTION Lexus RX400h produced between 1st August 2006 through to 7th August IDENTIFYING FEATURES JTJ HW31U# CAMPAIGN NUMBER: 1525F TARGET NUMBER: 17 SUPPLIER Toyota Motor Corporation Australia Limited. HAZARDS In the worst case, the power supply fuse may open circuit resulting in the hybrid system stopping whilst the vehicle is being driven. DEFECTS The inverter assembly is part of the hybrid system. The transistors on the control boards located inside the inverter assembly in some of the involved vehicles were inadequately soldered, and could be damaged by heat that occurs from large current flows that are typical under high-load driving conditions. If this occurs, various warning lamps will be illuminated on the instrument panel and in most instances the vehicle will enter a fail-safe driving mode, resulting in reduced power. All owners of affected vehicles will be contacted by Lexus Australia by phone and / or recall campaign letter at their last known address. Owners are advised to contact their authorised Toyota Dealership for inspection and rectification. AUSTRALIA Toyota FJ Cruiser (GSJ15) PRODUCT DESCRIPTION FJ Cruiser produced between 1st November 2010 through to 18th May IDENTIFYING FEATURES JTE BU11F# 0K ~ 0K Campaign number: 1524F TARGET NUMBER: 454 SUPPLIER Toyota Motor Corporation Australia Limited. HAZARDS In the event of a side collision, there is a possibility that impact detection may be slightly delayed and the SRS side airbag and curtain shield airbag may not operate as intended. DEFECTS In the SRS side airbag and curtain shield airbag system of the involved vehicles, due to the incorrect side airbag sensor polarity, the G- force threshold to detect a side impact may be incorrectly set to the improper direction. All owners of affected vehicles will be contacted at their last known address by recall notification letter. Owners are advised to contact their authorised Toyota Dealership for inspection and rectification. Page 3

4 AUSTRALIA Honda VT750CS Motorcycles PRODUCT DESCRIPTION 2010 year model Honda VT750CS motorcycles IDENTIFYING FEATURES VIN range: JH2RC50U*AK JH2RC50U*AK (* denotes check digit) TARGET NUMBER: 95 SUPPLIER Honda Australia MPE Pty Ltd HAZARDS Failure of the Back Angle Sensor, due to vibration, can cause the engine to shut down while riding. If this happens whilst in operation there is potential for the rider to lose control of the motorcycle resulting in an accident. DEFECTS The unique vibration characteristics of this particular model can cause resonance that may affect the operation of the Back Angle Sensor. If the sensor is affected, it can cause the engine to shut off during normal riding conditions and the engine cannot be re-started until the ignition switch has been turned off and back on. Owners of affected vehicles will be contacted by mail. Owners are advised to present their machine at their nearest Honda dealer for modification. The Dealer will carry out the fitting of a new Back Angle Sensor at no charge to the owner. AUSTRALIA Volvo C30 DRIVe Vehicles PRODUCT DESCRIPTION Volvo C30 DRIVe vehicles MY 2009 to MY2011, fitted with a 1.61 diesel engine. IDENTIFYING FEATURES VIN range: to Campaign number: R59192 TARGET NUMBER: 54 SUPPLIER Volvo Car Australia Pty Ltd HAZARDS Increased brake pedal force to be used by the driver. DEFECTS The cover panel on top of the engine can chafe against the vacuum pump and the brake booster, and in some cases this condition may lead to a vacuum leak in the brake booster system. Owners of affected vehicles will be contacted by letter. Owners are advised to contact their authorised Volvo dealer to have the brake vacuum hose replaced, and a protective mesh collar added on the vacuum line for future protection. If this chafing condition should occur, depending on the size of the hole on the vacuum hose, it may lead to a gradual reduction of brake booster assistance while braking i.e. a hard pedal brake. OF ORIGIN: Japan INJURIES The product poses a risk of injuries because if the steering wheel is repeatedly and strongly turned to the full-lock position, there is a possibility that the nuts securing the pinion shaft in the steering gearbox assembly may come loose. If the vehicle is continuously operated in this condition, the pinion shaft could become unstable and power generated by the electric motor may not be fully transmitted. Misapplication of this part could lead to separation of the ball joint from the lower control arm and collapse of the front suspension, which could potentially cause a vehicle crash. Affinia is working with Napa to identify any owners that have had this part mistakenly installed on to an Acura tl vehicle, and will replace the part free of charge. The supplier of the rear springs rolled the spring eye diameter out of tolerance on a limited number of parts in its production, and then corrected the diameter without heating the parts. VEHICLE MAKE / MODE sikatack / asap+ PRODUCTION DATES: NA sika corporation COMPONENT: equipment 1,152 Sika is recalling certain aftermarket auto glass adhesive cartridges, branded sikatack asap+ with product code packaged on February 28, 2011, as lot number Due to a production anomaly, a limited quantity of product was contaminated during packaging. As a consequence, the product may not cure or harden, thereby preventing the glass from remaining secure to the vehicle. If this condition is undetected, the glass may shift or fall out unexpectedly, causing injury to the vehicle occupant, property damage, or a vehicle crash. Sika will notify owners and will remove the windshield or other affected glass and reinstall the glass (or, where necessary, new glass) with new adhesive product. A potential exists for a small fracture within the line delivering gas to the oven cavity burner. Such a fracture may cause propane gas to leak from the line. A propane leak in the presence of an ignition source could result in a fire and/or explosion causing property damage and/or personal injury. Dealers will isolate and perform a drop pressure test on the oven supply tube and if the tube leaks it will be replaced free of charge. The safety recall has started. The adapter fittings have been identified as being defective in that they may cause a piece of the fitting to shear off or may result in a hydraulic leak. 16 keystone / residence-2012 : keystone rv company COMPONENT: electrical system: wiring 44 Keystone is recalling certain model year 2012 residence travel trailers produced from May 6, 2011, through May 18, The wiring connection to the 120 volt light on the ceiling of the slide room has the incorrect wiring connectors. The incorrect wiring connectors could cause a loose electrical connection leading to an increased risk of fire, property damage and/or personal injury. The safety recall is expected to begin on or before July 15, Owners may contact Mazda customer assistance center at Mazda is recalling certain model year mazda3 and mazdaspeed3 vehicles manufactured from January 7, 2008, through November 28, The ground terminal of the windshield wiper motor may have been inadvertently bent during assembly. If this condition exists then over time the electrical resistance of the motor circuit may increase up to a point that the windshield wipers would not work. Page 16

17 With the incorrect tpms sensors, drivers will receive a constant warning from the tire pressure monitor system. The system alerts the driver but, due to incorrect tpms sensors, the system cannot determine if the vehicle tires are properly inflated. This increases the risk that the vehicle will be driven with one or more underinflated tires, increasing the risk of a tire failure that may lead to a crash. Volkswagen will replace the TPMS sensors with the proper sensors to make the TPMS system of the vehicles compliant with fmvss 138. As of June 1, 2011, Volkswagen has contacted all affected customers to schedule the repairs free of charge. Owners may contact Volkswagen at Volkswagen / cc-2012 Volkswagen of America, inc COMPONENT tires: pressure monitoring and regulating systems 4 Volkswagen is recalling certain model year 2012, cc vehicles, for failing to comply with the requirements of federal motor vehicles safety standard no. 138, "tire pressure monitoring system (TPMS)." The factory installed wheels and tires were replaced at the port with accessory wheels and tires that were not equipped with the proper TPMS sensors. Spartan / gladiator-2011 Spartan / metrostar Spartan chassis, inc. COMPONENT air bags: frontal: sensor/control module 10 Spartan is recalling certain model year metrostar and model year 2010 gladiator fire apparatus chassis/cab, manufactured from April 9, 2010 through April 25, 2011 and equipped with rolltek supplemental restraint system (SRS) side air bag. These vehicles fail to comply with the requirements of federal motor vehicle safety standard no. 208, "occupant crash protection." The SRS warning indicator may not function, and if a fault occurs the vehicle's operator may not be warned of a problem with the side air bags. If an owner or operator is not warned of a problem with the side air bag system, they will not take steps to address and remedy the problem, thereby increasing the risk of injury or the severity of injury to vehicle occupants in the event of a crash necessitating deployment of the side air bags. Dealers will inspect the side air bag warning indicator light and connect the wires as necessary for the SRS to function properly. This service will be performed free of charge. The front axle outer wheel bearing cone rollers may not have been properly heat treated. The suspect bearing cones have the date code "zj" on the major face of the inner bearing race just following the "Canada" stamp. peterbilt / : Paccar incorporated COMPONENT: power train: driveline 72 Paccar is recalling certain model year 2012 peterbilt 587 tractors, manufactured from January 12, 2011, through May 20, 2011, equipped with meritor's maxton driveline assemblies. The tubing may have a seam weld with insufficient bonding. The affected driveline assemblies are rpl25 coupling shafts and slip shafts (main) driveline assemblies. This can result in seam weld fracture when torque is applied. Once the tube seam weld is fractured, the driveline may become inoperable which may result in a disabled vehicle or debris from the driveline may fall on the roadway, both of which can increase the risk of a crash. DESCRIPTION The recalled vehicles are model 2011 DS golf cars and hospitality, utility and transport vehicles used for short-distance transportation. The Club Car has various models, colors and sizes including 2 to 6 passenger seating vehicles. The vehicles can be identified by the serial number, which is above and to the right of the accelerator pedal. A list of models and serial numbers included in the recall are listed below. The brake pedal mounting blocks can crack and separate, resulting in a loss of braking ability. This can result in a crash. INCIDENTS/INJURIES Club Car has received no reports of brake pedal mounting blocks breaking in the field. No injuries have been reported. COMPONENT equipment: mechanical: carrier/rack 9,122 Yakima is recalling certain model flipside hitch mount bicycle carriers, p/n , used to transport up to 4 bicycles, sold as aftermarket equipment for use on various vehicles. The flipside has a feature that allows the mast to be lowered to the side of the vehicle to allow access to the vehicle's trunk, hatch, or truck bed. The pull pin that allows this feature can "walk" out of its position and allow the unit to fall to the side while bikes are loaded. The unit can fall to the side during use. This could lead to a bicycle hitting the ground and ejecting from the mount onto the road, creating a road hazard to other vehicles which could possibly result in a crash or injury to persons outside the vehicle.
