12 OES Type III Engine Pre-Trip Inspection
13 The pre-trip inspection for the OES Type III engines is similar to the pre-trip inspection performed on our Type I engines. This presentation covers the unique pre-trip inspection aspects of the type III engine.

29 1. Set Maxi-brake. 2. Place transmission in neutral. 3. Switch from ROAD to PUMP 4. Confirm PUMP ENGAGED & OK TO PUMP lights are illuminated
30 5. Set chock blocks Front and back when parked on level ground. Outside duals on downhill side when parked on slope. 6. Flip up TANK TO PUMP switch 7. Select pressure or volume Up for VOLUME psi Down for PRESSURE psi 5 7 6

35 Turn Aux Pump switch to the right until engine starts then leave in the on position. 6. If Aux Pump PSI gauge fails to show pressure, activate the primer. 5

37 Auxiliary Pump (Pump and Roll)
38 1. TANK TO PUMP switch must be open (up 1 position). 2. Turn Aux Pump ignition switch to the on position. Allow lights to cycle Allow glow plug to activate 3. Turn Aux Pump ignition switch to the start position. Release switch back to the on position once engine starts. 3 2
39 4. AUXILIARY PUMP ENGAGED light should be lit, the gauge should be backlit in red and reading pressure. 5. If the AUXILIARY PUMP PRESSURE gauge doesn t show pressure, activate the primer. 4 5
40 6. Pump and roll in low gear to help control speed. *Remember, the Aux Pump runs independently from the apparatus engine and transmission. Engine speed and road speed will not affect pressure. 6

58 OES Type III Engine Forms
59 Form 662 All LACoFD units assigned to an emergency incident outside of Los Angeles County shall complete a Form 662 for all incident assignment time. The 10/2009 version shall be used. (No previous versions of the Form 662 will be accepted.) Form 662 and accompanying documents shall be submitted to FMD s Cost Recovery Unit within 14 calendar days of the end of the assignment.
60 1. The Form 662 shall be completed---including a description of the assignment in the DESCRIBE INCIDENT ACTIVITIES box at the bottom of the form---and approved with a supervisory signature. (Forms without this approval cannot be processed.) DO NOT USE STANDBY IN THE DESCRIPTION. 2. A copy of the approved paper timecard, ESS etimesheet, or IPFIRS Screen #17 for each reported employee shall be attached to the Form 662. Hours reported on the Form 662 shall be consistent with the hours reported on the employee s time record. 3. All Form 662s must include the appropriate Incident Number (e.g., CA-BDF , CA-MMU , etc.) and the resource/unit request number (e.g., O-31, A-24, C-109, E-324). Request numbers can be obtained from Command & Control Dispatch. (Do not use an LAC Incident Number on the Form 662 for out of County incidents.)
61 Additionally, because an incident is outside of LA County, all personnel shall complete the new OES Form F-42. This new NCR, multipart OES form should be available at the incident. Form should be completed, approved, and submitted at the incident. One copy of the Form F-42 should be kept and submitted with the employee s Form 662 package.
62 Enter the time (24-hour clock) and briefly describe individual notable activities. Note the date as well if the operational period covers more than one day. Activities described may include notable occurrences or events such as task assignments, task completions, injuries, difficulties encountered, etc. This block can also be used to track personal work habits by adding columns such as Action Required, Delegated To, Status, etc. Completed ICS 214s are submitted to supervisors, who forward them to the Documentation Unit. All completed original forms must be given to the Documentation Unit, which maintains a file of all ICS 214s. Always keep a copy for your records.
63 Retain Cal OES Form F- 101 in engine apparatus log book for one year after last entry.
64 OES F-42 should be submitted at the incident or mailed to the OES office. A copy should be kept and submitted with your 662 package.
