3 5. Life Saving Rules. From July 1, 2009 Shell's Life-Saving Rules have been fully enforced. Shell s Life-Saving Rules set out clear and simple do s and don ts covering activities with the highest potential safety risk. They are not new. Most people comply with them every day but there are still breaches taking place. The Life-Saving Rules help to make sure that rules are followed and people are protected. Compliance with the Life Saving Rules is mandatory, and the Life Saving Rules are applicable to all operations under Shell s operational and / or governance control. Each reported non-compliance will be investigated. And failure to comply will result in disciplinary action, up to and including termination of employment for Shell employees, or for employees of contractors or sub-contractors, removal from the site and disqualification from future Shell work. Supervisors are held accountable to communicate and ensure compliance. If you choose to break the rules, you choose not to work for Shell.
4 The 12 Life-Saving Rules are: Work with a valid work permit when required Conduct gas tests when required Verify isolation before work begins and use the specified life protecting equipment Obtain authorisation before entering a confined space Obtain authorisation before overriding or disabling safety critical equipment Protect yourself against a fall when working at height Do not walk under a suspended load Do not smoke outside designated smoking areas No alcohol or drugs while working or driving While driving, do not use your phone and do not exceed speed limits Wear your seat belt Follow prescribed Journey Management Plan
5 Gantry Top Loading Instruction Drivers entering the Terminal must meet the following requirements: 1. Have the valid certifications from Accredited Training Providers, approved by the Ministry of Manpower. a. Apply Workplace Safety and Health in Process Plant (formerly Oil/Petrochemical Safety Orientation Course for Workers) or Supervise Workplace Safety and Health in Process Plant (formerly Oil/Petrochemical Industry Safety Course for Supervisors). b. Work-At-Height Course for Workers. 2. Have gone through the Shell SIT (Site Induction Training). 3. Ensure that his vehicle and tank height do not exceed 4.2m. Important Instruction Iso-tank used for the collection of IPA must come with its hatch cover air vent, bottom valve sealed. The seals must not be broken. Before loading, the metal cover for the hatch opening shall be deployed. Arriving at the Terminal: 4. Driver will park his vehicle in an orderly manner, outside the depot s gate. 5. He will proceed to the main guardhouse and surrenders his ID card in exchange for a Shell visitor s pass. 6. Drivers shall wear proper PPE before loading the product. a. Long Sleeve Shirt. b. Safety Goggles. c. Safety Shoes. d. Safety Harness. e. Hand Gloves. f. Safety Helmet with chin strap. 7. Driver shall comply with Shell life Saving Rules. 8. He will then report to the despatch office with the following Documents: Carrier advice / Notification, Haulier/Driver HSE Checklist duly filled, signed and stamped.
6 9. Once the order is confirmed, the driver will be given the i-buttons (Personnel and equipment), seals and instructions to which bay to load. Driver will counter sign on the loading instruction to ensure product & quantity are accurate. 10. Driver will also fill in his name, IC. Vehicle no., date and time into the register book at the security. 11. Driver will drive his tanker through the gate and tap at the card reader, located beside the despatch office window. 12. A few meters after the barrier, the driver will stop his vehicle for the inspection. 13. The following items will be checked by the Security Officer. a. Tanker must be visually well maintained. b. 2 x 9kg. & 1 x 2kg. fire extinguishers, fully charged and in good working condition. c. All light covers must not be broken. d. Appropriate Hazchem signs (3 pcs) must be displayed on the Left, Right and Back of the tank. e. Tires must have treads with depths of not less than 3mm for the steering axle and 2mm for the rest. f. All discharge valve(s) must be fully closed and with dust cover(s) in placed. 14. Upon successfully passing the inspection by the Security Officer, the truck will then be allowed to proceed to the loading bay. 15. When driving into the bay, the driver will align his truck along the yellow line, drawn on the ground. 16. After stopping, driver to apply the brakes, switch off the engine/master-switch and deploy wheel chock. 17. Driver will then seal the bottom valve(s) before proceeding up to the loading platform. 18. On the platform, the loading assistant (LA) will lower the draw-bridge, hook his safety harness onto the fall protection line before crossing over to the tank. On the tank, the LA will hook up the bonding wire, check for debris around the hatch cover(s) before opening it. 19. Upon opening the hatch cover(s), the LA will check the tank s internal for any contaminants. 20. Driver will inform LA of the product and quantity that he is taking. The LA will check and ensure that the tank capacity is more than the quantity that he is collecting. 21. The LA will insert the correct loading arm into the compartment and ensure that it is fully inserted. The arm shall be firmly secured with the rope or chain provided. Note: For IPA loading, the LA will first cover the top hatch with the metal cover before inserting the arm into the tank, through the opening made for that
7 purpose. 22. The LA shall then hook up the portable High-level sensor/ Overfill protection device at the rim of the compartment. 23. Once done, LA will inform the driver to start with the loading. 24. Driver will activate the loading by: a. Tapping the Red i-button (Personnel) at the RDI followed by the Yellow i- button (Tanker) b. Key in the trip number c. Select loading arm and Compartment number. d. Press the Start button. e. Standby at the Dead Man switch. When the light comes on, driver will have to depress the switch till it goes off. Failure to clear the light will trip the loading. f. When loading is completed, driver will inform the LA to remove the arm. g. Before parking the arm, the LA shall ensure that it has been drained dry before hooking up the drip bucket onto the arm. h. The LA shall close the hatch cover and secured it with the seal(s) provided. i. The LA will return the portable High-level sensor/overfill protection device to its original position. j. LA will withdraw the drawbridge and secured it by checking the lights. Green light is when drawbridge is secured and Red light shows that the drawbridge is down. 25. Before driving out of the bay, driver and LA shall perform a final check to ensure all equipments are parked and placed in their original positions. 26. Driver to start the engine and slowly drive out of the bay. Park the truck at the designated holding area before proceeding to the dispatch office. 27. At the despatch office, the driver shall return the 2 i-buttons before collecting the Delivery Note. Next, he will sign and return the 4th copy and hand over the Carrier copy to the security guard. 28. The guard will do a final check before driving out. 29. The driver will park his tanker safely outside the depot and return his visitor s pass to the main guardhouse. Version 4 Apr 2016 Pandan Site Requirements
