1 Procedure Type: Creation Date: Revision Date: Critical 12/22/ /06/2015 Purpose: Ensure that the Isolation of Hazardous Energy is done effectively and to maintain the safety of personnel working on those systems. Scope: This SOP is intended to be used as a guideline for Brown's Backhoe Service, Inc. employees who oversee and perform any Isolation of Hazardous Energy activities. Sub-Contractors that perform Isolation of Hazardous Energy activities for Brown's Backhoe Service, Inc. shall have procedures in place that meet or exceed the details listed in this procedure. Requirements: Before commencing any work that would require a shutdown of any system, the site shall be thoroughly inspected by the Work Team to determine if special safety measures must be taken. Applicable Documents: JSA (Job Safety Analysis) Materials and Tools: Locks Hasps Tags Personal Protective Equipment (PPE): Hard Hat Safety Glasses Flame Resisitant Clothing H2S Monitor Gloves Steel Toe Boots Page 1 of 6
2 1) Prepare for Shutdown a) A 360 degree walk around of the pumping unit is required to be completed before shutting down or working around a pumping unit. b) Verify key components are in safe and proper working condition by visual inspection, including: -unit guards are in place -equalizer bearing is functioning without restriction and properly lubed -saddle bearing is functioning without restriction and properly lubed -crank/wrist pin is functioning without restriction and properly lubed -counter weights are tightly fastened and that the bolts/nuts appear to be tightened down. Identify ALL isolation points (JOG/AOJ Switch, Electrical Panel Box, Unit Brake). Document these on your isolation log on the JSA. A Supervisor must be called when shutting down an in service unit so that the lease operator may be notified of shutdown. ALL employees and personnel must have individual locks and keys for the procedure to continue. Page 2 of 6
3 2) Equipment Shutdown (Standard) a) Notify all employees that shutdown is going to begin. b) Approach the JOG/AOJ Switch and either verify that the switch is off or turn switch from ON/AUTO to OFF. c) If pumping unit is currently pumping, wait for unit to slow down before applying unit brake. Apply unit brake. d) Approach the Electrical Panel Box and remove all conductive items on your person. If orange or red blinking light is flashing, STOP WORK and contact your Supervisor before proceeding. Always approach panel box from the hinge side to avoid being in line of fire of an arc flash. e) Remove glove from the hand closest to the panel box. f) While facing away, touch the panel with the backside of your hand in order to test for static energy. g) Replace glove, face away, and turn the panel boxes main switch from ON to OFF using the finger tips. h) Apply locks and tags with required information (Employee name, Company, Supervisor and Phone number, Work scope, Location, and Date of application) to the hasp once the switch is in the OFF position. Remember, each person must have their key on them at all times. i) Once all personnel have attached the required safeguards, return to the pumping unit. j) Release the unit brake. k) Test energy isolation by turning JOG/AOJ switch to the JOG position 2-3 times in order to VERIFY isolation. l) Return switch to the OFF position and apply unit brake. 4 3) Equipment Shutdown (ASD) a) This step is to be completed if an ASD is present on unit to be shut down. Notify your Supervisor and DO NOT proceed until instructed. b) Once ASD has been shut down by your Supervisor or a Chevron Representative, proceed to next step. c) Approach JOG/AOJ switch and turn switch from ON/AUTO to OFF. d) If unit is pumping, allow for unit to slow before applying brake. Apply unit brake. e) Approach electrical panel box and remove all conductive items on your person. If orange or red blinking light is flashing, STOP WORK and contact your Supervisor. Do NOT proceed. Always approach panel box from the hinge side to avoid being in line of fire of an arc flash. f) Remove glove. g) While facing away test electrical panel box for static energy with the back of your hand. Replace glove. h) While facing away, use the finger tips and switch the electrical panel box from ON to OFF. i) Apply locks and tags with required information (Employee name, Company, Supervisor and Phone number, Work scope, Location, and Date of application) to the hasp once the switch is in the OFF position. j) Once ALL personnel have attached the required safeguards, return to the pumping unit. Release unit brake. k) Test energy isolation by turning switch to the JOG position 2-3 times to Verify energy isolation. l) Return switch to the OFF position. Re-apply the unit brake. 4) Job Completion a) Complete the job on that location. b) Once job is complete, then the removal process can begin. ONLY if ALL personnel have completed their work. c) IF this condition is satisifed, begin removal procedure. Page 4 of 6
5 5) Lock and Tag Removal a) Once the job has been completed, ensure that all tools and equipment are in a safe location. b) If any pumping unit guards were removed, put them back in place. c) If well head guards were removed, put them back in place. After these items are verified, proceed to remove locks and tags. ALL personnel must be present before removing locks and tags. d) Remove glove and test for static energy before removing locks and tags. e) Replace glove. f) Remove the locks and tags. Each person must remove their own lock and tag. g) After all locks and tags have been removed, continue to next step to return the pumping unit back to service. 6) Returning Equipment To Service a) Once locks and tags are removed, face away from electrical panel box and using finger tips flip the switch from OFF to ON. b) Return to pumping unit and release unit brake c) Switch the JOG/AOJ switch and JOG the pumping unit for at least one full rotation to ensure it is operating properly. d) To do this, hold the switch to the JOG position. e) After JOG rotation confirms the proper operation of unit, turn JOG/AOJ switch to the ON/AUTO position. Keep in mind, the unit may not start immediately upon switching the JOG/AOJ switch to AUTO. f) Inform your Supervisor that the unit has been returned to service. Page 5 of 6
