3 12. When reversing on TJ or third party sites with a reversing assistant what must I do? Not reverse until instructed to do so by the reversing assistant Reverse quickly and safely to get the job done Never allow the reversing assistant to direct me from behind my vehicle on the camera for example. Ensure that the reversing assistant can always be seen in the mirrors of my vehicle Only reverse at tick over speed, slowly and carefully 13. If my vehicle breaks down on the highway what must I do? Carry out a mental risk assessment of the dangers to myself and other road users, pedestrians and cyclists. Pull off the road if safe to do so. Phone my Manager immediately to tell them where I am Make a cup of tea and switch my tacho to rest Call the Police if you believe other peoples lives may be in danger Turn my vehicles wheels into the verge 14. Tachograph Symbols, write the letter next to the appropriate symbol A: Periods of Availability B: Driving Symbol C: Other Work D: Rest 15. What are the 5 principles of Risk Assessment? 4 16. What should I do if a fire is discovered? Raise the alarm and ensure that the site managers and fire marshalls are informed Rush in and tackle the fire Assess the fire to whether it can be extinguished safely without putting yourself or anyone else in danger If I have put the fire out don t tell anyone Proceed to the nearest fire assembly point and ensure fire doors are closed Grab the nearest available fire extinguisher and spray on the fire Call the emergency services 17. Before, tipping, loading or un-loading you must... Ensure that I have taken my allocated rest breaks Check for any overhead obstacles such as cables, pipes, buildings and trees etc Ensure that the travelling height is safe for the route I am taking Ensure the lorry is a safe distance from overhead cables before sheeting and un-sheeting Check my load is sheeted safely and load secured appropriately and correctly Try and get an extra ton on my lorry to save the client an extra journey 18. If I have a road traffic accident and there is personal injury, damage to property, injury or death of certain animals such as dogs, horses, cows etc or damage to property adjoining the road. What must I do? Keep driving back to the yard and report it then Report it immediately to my manager Write the information down about the accident on the job sheet Use the company form to record all the information Stop at the scene Personal Injury will be reported to the police within 24 hours 19. What are the principles of Manual Handling Only lift an object if it is under 15kg Examine the object Plan the task Keep the load being lifted at arms length Decide in advance how best to hold the load Do not lift more than you can easily manage 20. When MUST I torque up the wheel nuts on the vehicle that I am using? Never Once a week, if it s my regular vehicle When instructed to do so by a tyre fitter or engineer (usually a written instruction) When it is not raining 4

5 21. When I complete the work allocated to me, what should I do? Drive straight back to the yard Call the office for further instruction Have a break, I deserve one! Start driving back towards the yard and call in after 30 minutes for the next job Park around the corner and wait for the next driver to finish 22. What should be completed when filling out a TJ Transport ticket book? Date and vehicle registration Customer name and loaded at address EWC code and delivery address Customer signature and disposal signature All of the above 23. What documents do I need to hand in at the end of the day? Anything that I find in the cab Most tickets and a day sheet All TJ tickets and material/tip tickets and a completed drivers day sheet Just the tickets Just a day sheet 24. If I have not been issued my work for Monday by the close of play Friday what must I do? Turn up at anytime Call in anytime Saturday Call in Saturday before lunchtime Start at 6am Start at 8am 25. Leaving the engine running is far better for the engine Thirty seconds of idling can use more fuel than turning off the engine and restarting TJ policy is to limit idling to the shortest possible time and turn off the engine if possible. One hour of idling can burn half to a full gallon of diesel Reducing idling can help TJ reduce its carbon footprint and be more efficient. 26. When I am doing my daily defect sheets what should I do with the cab and vehicle cameras if fitted? Nothing as they are pre-set Move the cab camera so that it is facing forwards out of the windscreen Make sure that the camera is facing the driver of the vehicle Check all cameras for damage Report on my defect sheet if the camera has been tampered with 5

6 27. What are the key things I must think of whilst tipping? I must ensure that nobody enters the danger zone whilst tipping The danger zone is the length of my lorry The danger zone is the tipping height plus 2 meters all around my vehicle If anyone enters the danger zone whilst I am tipping I must continue quickly tipping. If anyone enters the danger zone whilst I am tipping I must stop and politely ask them to wait safely away from my vehicle until I have finished tipping. 28. When should I use the Auto Sheet System? It s an automatic system I can use it at anytime I can only use it when parked on the weighbridge I should only use when my vehicle is stationary and in agreed safe areas I have checked for overhead obstructions like cables etc When working on the road planer to prevent material from falling onto the cab if allowed to by the Banksman. 29. When do I need to wear the chin strap on my hard hat? They are not required as we are not builders When walking around on site in case I trip over When working at height When climbing onto the body or working on a platform on my lorry When climbing into the cab of your lorry 30. What checks to the tailgate or rear doors of my lorry must I make before loading/unloading or joining the highway? It s an automatic tailgate therefor there is nothing to check I must check that the tailgate has correctly locked visually If security chains are fitted I must make sure they are used and only close one door at a time If securing rings or bolts are fitted they must be used I must check all security locks and wires are fitted and secure 31. I am concerned about contracting Tetanus or Hepatitis A or B as I am in contact with waste at TJ, what could I do about this? Speak to my manager for further advice Ensure that I always wear the correct PPE and wash my hands before going to the toilet or eating. Speak to my G.P. and request a Tetanus Booster Speak to my G.P. and request a Hepatitis A and B injection and ask TJ to cover the cost of my vaccination Do nothing 6

7 32. I am feeling unwell whilst at work or overnight what should I do and what will happen when I return to work? Telephone my manager as early as possible in the morning to alert them that I am unable to work I can call up to lunchtime on the day I should be working I can call no later than 30 minutes after the time I am expected to be at work to notify them that I cannot work. I don t need to notify anyone as there are loads of other staff who can do my job I will be asked to attend a back to work interview to ensure I am fit to return to work. 33. MY SPACE?
