2 General Information Arrowhead General Insurance Agency, Inc. (AGIA) has designed this program to meet the insurance needs for the Transportation & Delivery, Towing, Automotive Services, Newspaper & Publishing, Life Sciences & Bio-Technology, Healthcare & Human Services, Hospitality, and Municipality Industries. This manual is designed to give basic information to the producer to determine risk eligibility in this program. This is not a rating or a quoting tool, but as a reference to attract the better than average risk. All completed submissions must be sent to AGIA for evaluation and pricing. Eligibility Risks must be in business with Work Comp history for a minimum of two year and must have current insurance coverage in force. New ventures will not be written. All risks over $500,000 EAP will be pre-inspected by loss control for acceptability. Applications must in our office no later than 1 month prior to the expiration date. Any risk pre-inspected and not approved by loss control will not be written. Risks over $150,000 will have post-binding inspections performed by loss control. Telephone inspections may be performed at underwriter discretion or randomly by AGIA management on any size risk. Risks over $350,000 EAP and/or risks with an average loss ratio of 60% or higher, or an experience modification in excess of 1.50 or less than.60 will have extraordinary underwriting analysis done. Audited financials or a Company generated financial statement must be included in the submissions for risks generating $350,000 EAP or greater. Risks must provide written description of any individual loss greater than $50,000, including nature of the injury, cause and current status. All loss data provided should be valued within the last ninety (120) days. If a Company renewal was referred and approved the previous year, and there have been no material changes in exposure and/or loss results have not changed/improved, then the risk does not require referral to the home office. The following constitutes a material change: 1) Increase of Payroll by 60% 2) Increase or decrease of Experience Modification by 60% 3) Governing Class Change that now contemplates a class that requires a referral 4) Any change that pushes the risk outside the scope of our assigned authority 5) Accounts over $1,000,000 in manual premium or any risk with more than 100 employees Risk Management must be: 1) Stable for the past three years with no material change in operations. 2) Experienced in operation s industry. 3) Safety-conscious and compliant with any and all loss control recommendations. Exclusions The following risks do not qualify and are not to be submitted for consideration as a part of the program: Any exposure involving: Pollution, Asbestos, Lead, Radon, Nuclear Accident & Health (when written as such) Any risk involving Financial Guarantee Nuclear risks as defined in the Nuclear Incident Exclusion Clause Liability Reinsurance Attached to and forming part of this Contract 1

3 Liability as a member or subscriber of any Pool, Syndicate or Association All liability of the Company arising by contract, operation of law, or otherwise, from its Participation or membership, whether voluntary or involuntary, in any insolvency fund. Insolvency Fund includes any guaranty fund, insolvency fund, plan, pool, association, fund or other arrangement, however denominated, established or governed, which provides for any assessment of or payment or assumption by the Company of part or all of any claim, debt, charge, fee or other obligation of any insurer, or its successors or assigns, which has been declared by any competent authority to be insolvent, or which is otherwise deemed unable to meet any claim, debt, charge, fee or other obligation in whole or in part. 4 Classifications The Classification that most accurately describes the Insured s operations is to be used. In the case of multiple operations, the classification for each operation will be used. Only those governing classes listed in the Eligible Classifications section below qualify. A governing class is determined by the largest payroll amount assigned.
