6 Hurricane Nate Updates On 11/17/17, disaster declarations were made for several counties in Alabama due to the devastation of Hurricane Nate. FEMA has designated eight counties in the state of Alabama as disaster areas. The facilities located in the designated counties will be exempted from CMS ESRD QIP reporting program and granted extension for CROWNWeb clinical reporting periods as indicated. The additional affected counties designated by FEMA as of the date of this communication are as follows: Alabama Counties Autauga Baldwin Choctaw Clarke Dallas Macon Mobile Washington Please note that the August 2017, and September 2017 submission deadlines have already passed. As a result, the exceptions will be retroactively applied where applicable. CMS is closely monitoring the after effects of catastrophic events, and will update exception lists as needed in the future. 6

13 In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) ICH CAHPS Survey is the first national, standardized, and publicly-reported survey of patients' perspectives of dialysis care; it is also the first CAHPS survey focusing solely on a chronic disease. ICH CAHPS measures perceptions of patients with End-Stage Renal Disease (ESRD) receiving life-sustaining in-center hemodialysis care. ICH CAHPS is part of the ESRD Quality Incentive Program and is required as part of the value-based purchasing program for payments under the Medicare Program. 16 CROWNWeb Ultrafiltration Requirements 1. For 2018, the following data elements will be required to be reported in CROWNWeb for all hemodialysis sessions, Pre-Dialysis Weight Post-Dialysis Weight Delivered Minutes of BUN Hemodialysis 2. Additionally, the Number of Dialysis Sessions delivered by the dialysis unit to the patient, in the reporting month, will be required in CROWNWeb 16

22 CROWNWeb ICH CAHPS* Attestations Requirement Submissions for ICH CAHPS Reporting Measure due in CROWNWeb by 11:59pm ET on February 28 th. Facilities that are NOT eligible for the ICH CAHPS Reporting Measure because they did not treat a sufficient number of eligible patients, must attest in CROWNWeb that they are not eligible (i.e., treating fewer than 30 eligible patients). Facilities that ARE eligible for this Reporting Measure would leave the ICH CAHPS fields BLANK in CROWNWeb. 22 *In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems

24 CROWNWeb Clinical Depression and Pain Assessment Requirements Clinical Depression Screening and Follow-Up Facilities must submit information for each eligible patient once during the calendar year. The assessment period is from 1/1 12/31 of each calendar year. Pain Assessment and Follow-Up Facilities must submit information for each eligible patient twice during the calendar year. The assessment periods are from 1/1 to 6/30 and 7/1 to 12/31 of each calendar year. Both Sections CMS has provided due dates of 2/28/18 to complete reporting for the 12/31 assessment periods After the assessment period is closed, the reported values become read only. 24

28 Creating and Maintaining CROWNWeb Access Account creation and maintenance are through the Enterprise Identity Management (EIDM) System at Account assignment (Role and Scope) are obtained via the QualityNet Authorization and Role Management (QARM) System at Access to CROWNWeb is at 28

29 What is a CROWNWeb Role? CROWNWeb Role: - The assigned function(s) performed in CROWNWeb. 29

30 Types of CROWNWeb Roles: Security Official: Security Official Personnel assigned to approve or deny any facility user requesting a CROWNWeb role for a specific facility. Each facility must have at least one Security Official. 30

35 CROWNWeb Scope There is only one type of CROWNWeb Scope: A specific facility to which a CROWNWeb user may obtain access. An approved CROWNWeb user may have Scope over more than one facility at a time. 35

36 Accessing CROWNWeb Data Who can have access to CROWNWeb data? Acceptable Users Dialysis Facility Nurses Dialysis Facility Administrators Dialysis Facility Nephrologists Dialysis Facility Medical Director Only dialysis facility staff members who have approved access to confidential patient information Not Authorized Insurance Companies IT personnel Patients Family members Non-essential staff members If a user is found to be on the NOT Authorized List, their role(s) and scope should be deactivated or removed immediately. 38 Important CROWNWeb Reminders Keep CROWNWeb Roles and Scope up to date: Security Officials should remove all Roles and Scope access for personnel who are leaving their facility This should be done immediately upon departure Security Officials will need to assign and approve another Security Official prior to leaving a facility This helps insure that a facility still has control over CROWNWeb Roles and Scope access Good rule of thumb is to have two Security Officials assigned to each facility 38
