6 General Information CORDIS RETURN POLICY Return Authorization Prior to return of any Cordis Corporation product for credit consideration, approval for the return needs to be requested by phoning Johnson & Johnson Health Care Systems Sales Support Center at option 2, or by logging on to and selecting the Customer Resources link. Consignment returns must be coordinated with the Cordis Sales Representative. The customer must provide all of the following information for products to be considered for a return authorization approval and for credit processing: Customer number, catalog number, lot number, and quantity of each item to be returned Acquisition method of item (such as: a purchase, consignment, or evaluation) Invoice number and purchase order number (if available) Reason for return Return Goods Condition All products, which are in saleable condition, may be returned for credit. Note: These products must be in their original packaging integrity (original ship unit). If any of the following conditions are true the products are generally not considered saleable: Discontinued from sale Modified products ("Specials") Opened packaging Modified product/trays Modified instruments Damaged Used Within six months of expiration date Less than full shipping unit Stickers or markings on blister package A-4

7 General Information CORDIS RETURN POLICY Credit Policy for Unused Products Credit will be given only after product has been received and inspected. Full Credit for merchandise in saleable condition: Shipped in error by Johnson & Johnson Health Care Systems Inc. Returned at the request of Cordis Corporation or Johnson & Johnson Health Care Systems Inc. Full Credit, less a 10% restocking charge (of the invoice price), on returns of any value: Returned in saleable condition within 90 days from date of purchase (determined by invoice date) Full Credit, less a 30% redress fee (of the invoice price) on merchandise: Returned in saleable condition within 90 days from date of purchase (determined by invoice date) but requiring reprocessing (repair, repacking, reboxing, etc.). Disposition on need for redress is determined by Cordis Quality Assurance. No Credit on merchandise: Returned after 90 days from date of purchase (determined by invoice date) In unsaleable condition Return Location All products should be returned with freight prepaid by the customer within five business days of receiving Return Authorization approval.
