3 Crushed Finger Incident Activity Activity at Time of Incident Transfer of a power pack from outside of the engine room to the offloading storage area Starboard deck using a two wheeled trolley. Power pack secured in transport case 264kg & top heavy Power pack transport case

4 Crushed Finger Incident Background Atwood Eagle has 3 main diesel engines, 16 cylinders each Engine cylinder, piston and associated parts are supplied in a unit called a power pack The task of changing out a power pack is conducted 3-5 times per year A power pack had been changed out on #3 engine the previous day. Housekeeping was being conducted in the engine room which required the removal of the worn power pack from the engine room A number of I beams were available in the engine room for mechanical handling of the power packs, but some areas still required manual handling Once outside the engine room power packs were typically moved using a 4 wheeled cart

5 Crushed Finger Incident Description Toolbox meeting held to discuss movement of power pack through a water tight door and to the storage point. The weight of the unit was discussed, but the transfer method was not Motorman & two roustabouts successfully move power pack out of engine room through watertight door manually using a two wheeled trolley

6 Crushed Finger Incident Description With the power pack and trolley now outside the engine room, a roustabout (IP) promptly took hold of the trolley and lifted the power pack back on the trolley At this point the IP was unable to control the load and it continued coming down past the point of equilibrium The IP attempted to catch the load and prevent it from falling and in doing so caught his right hand little finger between the trolley handle and deck resulting in a compound fracture

9 Crushed Finger Incident Key Findings Lack of mechanical lifting points made use of ideal trolley (4 wheel) difficult Internal I beam & trolley Outdoor Lifting point Preferred Method: Mechanical Lift through doorway with 2 x lifting points and beam trolleys onto 4 wheel flatbed trolley.

10 Crushed Finger Incident Key Findings Lifting powerpacks vertically out of engine room not considered feasible due to a lack of suitable hatch area above A JSA had not been developed previously, or on this occasion for handling power packs Personnel involved had not completed this task before Instructions provided to personnel & level of supervision was inadequate Injured person was eager to get the job done, new to the rig Injured person attempted to prevent load from falling rather than letting it go

11 Corrective Actions Install mechanical lifting / trolley securing arrangement for this task Conduct hazard hunt across rig to identify areas where mechanical lifting points are required Develop a JSA for handling power packs and other similar activities as identified from the hazard hunt Educate crew on the suitability of various trolleys and hand safety I beam required for upper platform to eliminate manual handling Example of doorway lifting point Existing lifting points in engine room,
