1 FLEET REPAIR SERVICES Commodity Code: Automotive/Chemical Waste Removal Services Commodity Code: Vehicle Maintenance, Repair, and Service Buyer: Janet Bradley Expiration Date: Instruction for Repairs: 1. Choose the vendor of your choice from the list on pages 2 to 5. The spreadsheet shows all the awarded vendors and the services their company performs. The services the vendors provide are listed across the top of the page. If there is a "" in the column next to the vendor of your choice, they provide that service. The vendor list will be updated as the vendors turn in their required paperwork. 2. To compare the warranty of vendors refer to pages 6 to 7. The warranty varies by the type of service you are having performed so make sure you are looking at the correct warranty. For example: has 5 different warranties, make sure you are looking at the correct one. 3. Enter a purchase requisition to the vendor of choice using a estimated amount for the repair. Enter the following information in the Description of the Requisition a. The type of repair requested (ex: Air Conditioning Service, Transmission Service, etc.) b. The Year, Make, Model and Vehicle ID Number for the vehicle being repaired. 4. When you receive the final estimate from the, enter a Change Order in MIS and send Chris Chambers a copy of the estimate. When entering the change order, add an addition 10% to the total estimate to cover any unexpected repairs that may arise. She will approve the Change Order and attach the estimate to the Purchase Order as back up. Instructions for Oil Change, Inspections, and Tire Services (repairs only, not purchases) 1. Enter the following information in the Description of the Requisition. When the funds on the Purchase Order are expended, you will need to enter a new Purchase Requisition. We will not process any increases on these purchase orders. Open Purchase Order for (enter the type of service) for the (enter your department name). Not to Exceed $. (enter the dollar amount).

One year warranty from the date of installation ANY warranties on the products sold herein are those made by the manufacturer of those products. Warranty on labor is one year or 12,000 miles whichever comes first. Warranty work has to be performed in our shop and cannot exceed the original cost of the repair. Take 5 Oil Change LLC If your oil warning light comes on (or your gauge loses pressure) stop your car immediately! Never operate your engine when the oil warning light comes on. Doing so may void your warranty. Guarantee of parts is covered by the manufacturer and is expressly limited to manufacturer specified coverage. The guarantee will be void if claims against it are submitted after 30 days or 1,000 miles, whichever comes first. Your receipt serves as your warranty and is non-transferable. We are not responsible for loss of fluid or oil through component leaks other than those items used during or replaced during our service. We are not responsible for oil consumption by your vehicle. The customer also acknowledges that he or she is personally responsible in deciding when to return for service and should consult the car's owner manual. The customer will hold Take 5 harmless from any claims resulting from advice or recommendations made by Take 5 personnel regarding oil change intervals. This warranty is valid at any 5-minute Oil Change or Take 5 Oil Change location. Unauthorized repairs or expenses performed by another repair facility concerning a warranty issue without our consent will void your warranty.
