17 STATE OF ILLINOIS SPECIAL PROVISIONS The following Special Provisions supplement the Standard Specifications for Road and Bridge Construction, Adopted April 1, 2016, the latest edition of the Manual on Uniform Traffic Control Devices for Streets and Highways, and the Manual of Test Procedures of Materials in effect on the date of invitation of bids, and the Supplemental Specifications and Recurring Special Provisions indicated on the Check Sheet included herein which apply to and govern the construction of Section PV, and in case of conflict with any part, or parts, of said Specifications, the said Special Provisions shall take precedence and shall govern. DESCRIPTION OF PROPOSED IMPROVEMENT The work shall consist of the reconstruction of County Highway 34 (Dee Bennett Road/N. 27 th Road) for a length of 2,250 feet (0.43 miles) approximately ¼ mile west of Naplate, IL. The work shall also consist of the reconstruction to the entrance roads to the LaSalle County Highway Department and the Ottawa Barge Terminal. The work shall consist of Earth Excavation, Special; Furnished Excavation; Aggregate Subgrade Improvement; Bituminous Materials (Prime Coat); Hot-Mix Asphalt Binder and Surface Courses; Storm Sewer and the related structures; Paint Pavement Markings; Erosion Control; and Seeding Class 2 (Special). The construction shall be staged according to the Staging Plans provided in the Drawings. RESPONSIBLE BIDDER The Contractor shall have on file, at the LaSalle County Highway Department, a STATEMENT OF COMPLIANCE establishing adherence to the LaSalle County Resolution titled AMENDING RESOLUTION DEFINING RESPONSIBLE BIDDER. The Contractor shall comply with the conditions of the resolution for the duration of the work. PREVAILING WAGES This contract calls for the construction of a public work, within the meaning of the Illinois Prevailing Wage Act, 820 ILCS 130/.01 et seq. ( the Act ). The Act requires contractors and subcontractors to pay laborers, workers and mechanics performing services on public works projects no less than the current prevailing rate of wages (hourly cash wages plus amount for fringe benefits) in the county where the work is performed. The Illinois Department of Labor publishes the prevailing wage rates on its website at: The Illinois Department of Labor revises the prevailing wage rates and the contractor/subcontractor has an obligation to check the Illinois Department of Labor s website for revisions to the prevailing wage rates. For information regarding current prevailing wage rates, please refer to the Illinois Department of Labor s website. All contractors and subcontractors rendering services under this contract must comply with all requirements of the Act, including but not limited to, all wage requirements and notice and record keeping duties. Certified payrolls shall be required per Check Sheet LRS 12. 1

(815) Revise Article of the Standard Specifications as follows: Change the second and third sentences of the first paragraph to: Co. The minimum acceptable insurance company rating shall be A as rated by A.M. Best Add the following to Article : The Contractor and Subcontractors shall waive subrogation on workers compensation. A statement concerning the waiver shall be shown on the insurance certificate. County of LaSalle shall be listed as primary additionally insured on all policies. County of LaSalle shall be listed as the Certificate Holder on all policies. The minimum umbrella liability limit shall be $1,000,000. The County of LaSalle and their officers, employees, and agents shall be indemnified and held harmless in accordance with Article Certificates of insurance shall be mailed or faxed to the LaSalle County Highway Department with a copy to the LaSalle County Board Office at 707 Etna Road, Ottawa, IL Fax Phone NOTE: The insurance requirements as described herein are illustrated in the Sample Certificate of Liability Insurance included in this proposal. It is understood that the sample certificate is for information purposes only and includes only the minimum insurance coverage required. SUBCONTRACTORS AND MATERIAL SUPPLIERS Prior to execution of the contract by the awarding authority, the Contractor shall provide the names and addresses of all Subcontractors and Material Suppliers for approval. No changes to the approved list shall be made without written consent of the Engineer. Final payment will be withheld until the Contractor, all Subcontractors, and Material Suppliers have provided Affidavits stating that all sums claimed on account of this improvement are paid. 2

19 Section PV County Highway 34 LaSalle County MOBILIZATION This contract contains no provisions for Mobilization. Therefore, Section 671 of the Standard Specifications is deleted. WORKING DAYS The Contractor shall complete the work within 75 working days. Work shall commence in accordance with the Prosecution of Work requirements and continue uninterrupted unless conditions listed in Article (a) or (b) are encountered. PROSECUTION OF WORK Revise the first sentence of Article of the Standard Specifications to read: After award of the contract, the Contractor shall begin work to be performed under said contract within ten (10) days of a written Notice to Proceed. TRAFFIC CONTROL PLAN Traffic control shall be in accordance with the applicable Sections of the Standard Specifications for Road and Bridge Construction, the applicable guidelines contained in the Illinois Manual on Uniform Traffic Control Devices for Streets and Highways, the Standard Specifications for Traffic Control Items, these special provisions, and any special details and Highway Standards contained herein and in the plans. Standard Specifications for Road and Bridge Construction and the following: (1) Highway Standards, and (2) other Special Provisions relating to traffic control. 1. Highway Standards: , , , , , , , , B.L.R. 21-9, and B.L.R Signs shall be new or in like-new condition. 3. Traffic control and protection will not be paid for as a separate item, but the cost will be considered as included in the contract unit price for the construction items involved and no additional compensation will be allowed. PRECAUTIONS FOR UTILITIES The Contractor shall take whatever precautions which may be necessary to protect the property of the various public utilities which may be located underground or above ground, at or adjacent to the site of this improvement. These facilities shall be saved harmless and care shall be exercised so as not to disrupt or destroy the services provided by these utilities. The Contractor will be required to repair or replace any public utility property which has been damaged through his/her efforts. The procedure and specifications of repair will be in accordance with the regulations and/or policy of the utility. 3

20 Section PV County Highway 34 LaSalle County PRECAUTIONS FOR UTILITIES (cont d) THE CONTRACTOR SHALL CONTACT AND COORDINATE HIS ACTIVITIES WITH THE UTILITIES BY CONTACTING: JULIE When the plans or special provisions include information pertaining to the location of underground utility facilities, such information represents only the opinion of the LaSalle County Highway Department as to the location of such utilities and is only included for the convenience of the bidder. EARTH EXCAVATION (SPECIAL) Description. This work shall consist of the excavation and transportation of suitable excavated material from the designated borrow area to embankment locations throughout the limits of the Contract. Equipment. Equipment shall meet the requirements of Article 202 of the Standard Specifications. Construction Requirements. This work shall be in conformance with the applicable provisions of the following Articles of the Standard Specifications: General Excavation Articles Method of Measurement. Earth Excavation (Special) will be measured for payment as outlined in Article of the Standard Specifications by the cubic yard. Basis of Payment. This work will be paid for at the contract unit price per cubic yard for EARTH EXCAVATION (SPECIAL). Removal and disposal of unstable and/or unsuitable material will be paid for at the contract unit price per cubic yard for REMOVAL AND DISPOSAL OF UNSUITABLE MATERIAL. EMBANKMENT Revise Article of the Standard Specifications. Add the following paragraphs after paragraph 2: Reclaimed asphalt millings may be used as embankment in that area which will serve as the subgrade directly beneath the proposed road bed. The depth of millings shall not exceeds 12-inches once compacted. The density of the compacted material shall be to the satisfaction of the Engineer. 4

21 Section PV County Highway 34 LaSalle County EMBANKMENT (cont d) The top 4-inches, minimum, of placed embankment shall be topsoil quality material meeting the requirements of Article The top 4-inches of embankment shall be prepared meeting the requirements of Articles 212 and 214. HOT-MIX ASPHALT SURFACE REMOVAL, SPECIAL Description. This work shall consist of the removal of the existing asphalt surfaces to variable depths via cold milling in the locations indicated on the Plans. Unless a specific depth is outlined on the Plans, the cold milling shall be performed to a depth that removes the entirety of the hot-mix asphalt pavement. It is estimated that the average depth of hot-mix asphalt on County Highway 34 is approximately 8-inches and that the average depth of hotmix asphalt on the County Highway Department Entrance is approximately 6-inches. Equipment. Equipment shall meet the requirements of Article of the Standard Specifications. Construction Requirements. This work shall be in conformance with the applicable provisions of the following Articles of the Standard Specifications: Cold Milling Articles Method of Measurement. Hot-Mix Asphalt Surface Removal, Special will be measured as outlined in Article of the Standard Specifications by the square yard. Basis of Payment. This work will be paid for by the contract unit price per square yard for HOT-MIX ASPHALT SURFACE REMOVAL, SPECIAL. PIPE CULVERTS, CLASS D All Pipe Culverts shall be Precoated Galvanized Corrugated Steel Pipe of the type and diameter indicated. STORM SEWERS, CLASS A All Storm Sewer shall be Reinforced Concrete Culvert, Storm Drain, and Sewer Pipe of the type and diameter indicated. 5

22 INLETS, SPECIAL Section PV County Highway 34 LaSalle County Description. This work shall consist of constructing special inlets with frames and grates per the Details provided on the Plans. Materials. In addition to those requirements outlined in the Details in the Plans, materials shall meet the requirements of Article of the Standard Specifications. Construction Requirements. This work shall be in conformance with the following Articles of the Standard Specifications: and Method of Measurement. This work shall be measured for payment by each, complete unit installed. Basis of Payment. This work will be paid for by the contract unit price per each for INLETS, SPECIAL. CHAIN LINK FENCE Description. This work shall be in accordance with Section 664 of the Standard Specifications and this Special Provision. Work will include 3-strands of barbed wire added to the top of the chain link fence and the removal and disposal of the existing chain link fence. Materials. Add the following to Article : Item Barbed Wire Arm-3 Strand Article/Section (e) Construction. Add the following to Section 664. In lieu of post tops, all posts shall be fitted with a barbed wire arm which shall fit around the outside of the posts and secured in place. Barbed wire shall be pulled tight, according to standard practice and attached to the arm on each fence post according to the arm manufacturer s recommendations. Splices in barbed wire and connections to existing barbed wire shall be by wrapped splice not less than six complete turns or by use of a compressed sleeve meeting the approval of the Engineer. Basis of Payment. The work shall be paid for at the contract unit price per foot for CHAIN LINK FENCE, 6 which shall include barbed wire arms, barbed wire, and the removal and disposal of the existing chain link fence. 51 La Salle County Prevailing Wage for July 2015 Explanations LASALLE COUNTY ELECTRICIANS (NORTH) - Townships of Mendota, Meriden, Earl, Adams, Troy Grove, Ophir, Northville, Freedom, Serena, Mission, Dimmick, Waltham, Wallace, Dayton, Rutland, Miller, Manlius, Peru, LaSalle, Utica, Ottawa, South Ottawa, Eden, Vermilion, Deer Park, Farm Ridge. MILLWRIGHTS (EAST) - The Eastern 1/3 of the county (approx.). The following list is considered as those days for which holiday rates of wages for work performed apply: New Years Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, Christmas Day and Veterans Day in some classifications/counties. Generally, any of these holidays which fall on a Sunday is celebrated on the following Monday. This then makes work performed on that Monday payable at the appropriate overtime rate for holiday pay. Common practice in a given local may alter certain days of celebration. If in doubt, please check with IDOL. Oil and chip resealing (O&C) means the application of road oils and liquid asphalt to coat an existing road surface, followed by application of aggregate chips or gravel to coated surface, and subsequent rolling of material to seal the surface. EXPLANATION OF CLASSES ASBESTOS - GENERAL - removal of asbestos material/mold and hazardous materials from any place in a building, including mechanical systems where those mechanical systems are to be removed. This includes the removal of asbestos materials/mold and hazardous materials from ductwork or pipes in a building when the building is to be demolished at the time or at some close future date. ASBESTOS - MECHANICAL - removal of asbestos material from mechanical systems, such as pipes, ducts, and boilers, where the mechanical systems are to remain. 54 La Salle County Prevailing Wage for July 2015 This shall encompass laborers, workers and mechanics who drive contractor or subcontractor owned, leased, or hired pickup, dump, service, or oil distributor trucks. The work includes transporting materials and equipment (including but not limited to, oils, aggregate supplies, parts, machinery and tools) to or from the job site; distributing oil or liquid asphalt and aggregate; stock piling material when in connectin with the actual oil and chip contract. The Truck Driver (Oil & Chip Resealing) wage classification does not include supplier delivered materials. Other Classifications of Work: For definitions of classifications not otherwise set out, the Department generally has on file such definitions which are available. If a task to be performed is not subject to one of the classifications of pay set out, the Department will upon being contacted state which neighboring county has such a classification and provide such rate, such rate being deemed to exist by reference in this document. If no neighboring county rate applies to the task, the Department shall undertake a special determination, such special determination being then deemed to have existed under this determination.
