12 I have been actively encouraged and supported to undertake ICM training as a LTFT ICM was my first rotation after returning as a new LTFT trainee. I think this was a useful rotation to return to after a period of time on maternity leave as ICU tends to be a well supported, protective environment I have had many positive experiences and feel that training can be better as a LTFT trainee. For example I see far more cases and develop a greater depth of experience There have been lots of advantages in addition to better work life balance including more time to complete lengthy projects or audit loops I think its very doable being LTFT in both specialities and I wish I'd applied sooner to ICM because I love it and am more fulfilled. I didn't because I was intimidated by the reputation but I shouldn't have been!
13 Attitudes to LTFT training...you re just being lazy / difficult / shouldn t have a training number* *I have personally heard all the above descriptions being used about other LTFT trainees in anaesthesia and ICM I was exposed to some very negative perceptions...i did find it hard to stomach some of the comments and implied criticism I got for not being in on days I was not contracted to work Real problem with culture on the shop floor though. Vastly outnumbered by men which I think leads to perception that women with families and ICM don't mix I have also had very difficult experiences with hostile consultants and other trainees, challenging my commitment to the specialty
14 Access to LTFT training in ICM I was asked to be available FT for ICM blocks but with some difficulty I have managed to organise block LTFT after some disagreement with health board I will not be doing my ICU training as a LTFT. I am an 80% trainee who by contract need to go full time for ICU and Obstretrics for service provision..it does not appear to be a very viable career choice. I was very interested in icm but felt I would not be appointable as a LTFT trainee I have applied for dual training twice but was not successful it became apparent that a LTFT would not have the time to do the extracurricular work necessary to obtain the points for the interview process

16 I fully believe that to make the specialty sustainable, part time working, whether long term of for shorter periods, needs to become a normal part of an ICU career. The specialty needs to be more honest about burn out and the prospects of ICU careers spanning 40+ years. I feel that ICM is a speciality that should be encouraging LTFT working both at trainee and consultant level, since being LTFT I was able to cope much better than my colleagues with the stresses of working on ICU (psychological, high intensity shifts, frequent out-of-hours working, etc). I think that this would help reduce burn-out in ICM trainees/consultants
