Prescribed form can be obtained from this office, Bus owner holding Permanent/Temporary Permit in any route, should surrender the permit held by him/her at the time of making application to consider his application for any other route of his/her choice as per this advertisement. Any application made without surrendering existing Permanent/Temporary Permit shall be summarily rejected. The applicant shall also file self-declaration along with application stating therein that no tax dues and penalty are outstanding against the operator, no VCR pending, no court cases pending against any of his vehicle, year of manufacture and make of bus, type of bus and facilities provided in bus (Non-Deluxe, AC Deluxe, AC Deluxe Sleeper etc.) and the vehicle in question is not off-road at the time of making application and till the date of consideration of application for grant of permit. In-complete application and application received beyond this specified date shall not be entertained.
