May 11, 2001 ADVICE 2316-G/2116-E (Pacific Gas and Electric Company ID U 39 M) Subject - Revisions to PG&E Forms Entitled General Terms and Conditions (Form 79-716) and Declarations (Form 62-0980). Public Utilities Commission of the State of California Pacific Gas and Electric Company (PG&E) hereby submits revisions to its forms for General Terms and Conditions (Form 79-716) and Declarations (Form 62-0980). The affected forms are enclosed Attachment I. Purpose The purpose of this filing is to propose revisions to two forms, that pertain to Gas and Electric Rules 15--Distribution Line Extension. The forms, General Terms and Conditions (Form 79-716) and Declarations (Form 62-0980) have been changed to reflect changes to Rule 15 approved by the Commission on March 21, 2001. Background D. 99-06-079 ordered PG&E to file changes to line extension rules deleting provisions known as Option 1 and the irrevocable option selection requirement. On March 21, 2001, the Commission approved PG&E s Advice 2209-G-B/1963-E- B implementing the required changes to Rule 15. In compliance with direction from the Commission s Energy Division, PG&E did not modify its existing Distribution and Extension Agreement Declarations and its General Terms and Conditions at the time it filed Rule 15 revisions. With the approval of the Rule 15 changes, PG&E is now proposing changes to the related forms.

May 11, 2001 2 Advice 2316-G/2116-E Proposed Changes PG&E proposes revisions to its General Terms and Conditions and Declarations as follows: General Terms and Conditions Section B.1. Reference to the Installation Option Selection Agreement is deleted since this agreement serves no purpose with the elimination of Option 1. Section C.1.b. This section is deleted since its sole purpose is to refer the Installation Option Selection Agreement, which serves no purpose with the elimination of Option 1. Section C.1.c. This section is deleted since its sole purpose is to refer the Installation Option Selection Agreement, which serves no purpose with the elimination of Option 1. Declarations Section 2. This section is revised to remove discussion of Option 1 from the construction options. In addition, the statement: Once the Applicant selects either Option 1, Shared Construction or Option 2, Competitive, the option selection is final and cannot be changed is deleted. Effective Date This filing will not increase any rate or charge, cause the withdrawal of service, or conflict with any other rate schedule or rule. PG&E requests that the form changes proposed in this filing become effective June 20, 2001.
