1 SILICON VALLEY ENERGY PROGRAM POWER PLAYERS REBATE PROGRAM ELIGIBILITY The POWER PLAYERS Rebate Program is part of the Silicon Valley Energy Program, administered by Pacific Gas and Electric Company (PG&E) in collaboration with the City of San José. POWER PLAYERS is available to PG&E small commercial, industrial or agricultural electric customers taking service on A-1 or A-6 rate schedules who have a facility located in Gilroy, Los Gatos, Milpitas, Mountain View, San José or Sunnyvale within one of the following zip codes: The total rebate limit for this program is $200,000 per customer per year. This rebate cap applies to customers with single or multiple meters. Customers are encouraged to apply to PG&E s Express Efficiency rebate program if they have reached their rebate limit or if they would like rebates for measures not offered by POWER PLAYERS. REQUIREMENTS Rebate items must be properly installed and operating successfully on the PG&E electric service account listed on the rebate form. All paid invoices or receipts must be dated between January 1, 2004 and December 31, All rebate items must be purchased and installed prior to submitting a rebate form. Rebate forms must be received by PG&E s Central Processing Center or postmarked no later than December 31, Rebates cannot be combined with other Public Purpose Program surcharge-funded rebate or incentive programs for the same measure. Equipment must meet the requirements as stated on the next page. If you have questions, please call before purchasing products. PG&E reserves the right to verify installation and deny payment for products that do not meet program requirements. A Joint Project of: Rebate payments are the property of the customer. The customer may make their rebate payable to a third party, such as a vendor or landlord, in which case a completed Payment Release Form is required before payment can be made. The selection, purchase and ownership of the equipment are the sole responsibility of the customer. PG&E and the City of San José make no representation as to the safety, reliability and/or efficiency of the equipment selected or components thereof. PG&E and the City of San José make no warranty, whether expressed or implied, including warranty of merchantability or fitness for any particular purpose, use, or application of the equipment. HOW TO APPLY 1. Reservations are required for rebates of $1,000 or greater. Although reservations are not required for rebates under $1,000, customers are encouraged to make reservations in order to ensure fund availability. You must reserve funds before you purchase and install equipment. Call to reserve your funds. 2. Calculate your rebate by multiplying the Quantity Purchased (Column A) times the Rebate/Unit (Column B) and write the rebate amount in Column C. Your rebate for each product cannot exceed the price you paid for it. 3. Add all the rebate amounts in Column C and write the rebate amount in the space provided at the bottom of the rebate form. 4. On the paid, itemized invoice(s) or receipt(s), circle the energy-saving product(s) for which a rebate is requested and label each one with its rebate category letter. Be sure that the make and model number of each item appear in the invoice. 5. Fill out and sign the Customer Information page. Do not use correction fluid on the rebate form. 6. Attach the original, itemized invoice(s) to this rebate form and mail to: Pacific Gas and Electric Company Central Processing Center 3480 Buskirk Avenue, Suite 150 Pleasant Hill, CA The POWER PLAYERS Rebate Program may be modified or terminated without prior notice. 2 A. SCREW-IN COMPACT FLUORESCENT LAMPS (CFLS) Must replace incandescent lamps. No rebates will be paid for fixtures that have previously received a rebate in this category (exceptions may apply). If a facility has received rebates in the past for this measure, it is subject to pre-inspection if requesting rebates for additional CFLs. Replacement CFLs may be self-ballasted, modular (lamp with ballast adapter), or cord and plug retrofit kits. All self-ballasted lamps must have electronic ballasts and be ENERGY STAR -qualified. Modular units 15 Watts must have electronic ballasts. For all units, lamp/ballast efficacy must meet these requirements: Lamp Power & Configuration Minimum Efficacy (Lumens Per Watt, Based on Initial Lumen Data) Bare Lamp Power < Lamp: Lamp Power Covered Lamp Lamp Power < (no reflector): Lamp Power 15 and < Lamp Power 19 and < Lamp Power Covered Lamp Lamp Power < (with reflector): Lamp Power In addition, when replacing the following wattages of incandescent lamps, all replacement Bare Lamp CFLs must have the following Minimum Lumen Output (shown on the manufacturer s specification sheet): Wattage of A-Shaped CFL Minimum Lumen Output Incandescent Bulb (based on 100 hr. initial values) 40 Minimum of Minimum of Minimum of 1, Minimum of 1, Minimum of 2,600 B. COMPACT AND LINEAR FLUORESCENT FIXTURES Only complete new fixtures or modular retrofits with hardwired ballasts qualify. New fixtures must replace an incandescent fixture or mercury vapor fixture. All ballasts must be electronic. CFLs/ballasts must meet the same efficacy requirements as in category A. CFL ballasts must be Programmed-start or Programmed Rapid-start with PF 0.90 and THD 20%. Linear fluorescent lamps/ballasts must meet the specifications defined in category C, below. Fixtures in this category are not eligible for additional rebates under categories A, C or D. C. T8 OR T5 LINEAR FLUORESCENT LAMPS WITH ELECTRONIC BALLASTS Rebate applies to existing T12 lamps and magnetic ballasts that are either delamped or replaced by T8 or T5 lamps with the electronic ballasts described below. Delamping is the permanent removal of existing T12 lamps/ballasts and unused lampholders from existing fixtures without replacing the lamps. To receive credit for delamping, customers must remove not more than half of the existing lamps/ballasts (along with lampholders) from each fixture. The total number of lamps claimed for delamping may not be more than the number of replacement T8 or T5 lamps installed. Customers are responsible for deciding whether delamping will maintain adequate light levels. Replacement T5 lamps in low bay installations (under 15') must provide indirect lighting only. Electronic ballasts must be high frequency ( 20 khz), UL-listed ballasts that are warranted against mechanical or electrical defects for five years, and have PF At full light output, ballasts for 4-foot and 8-foot lamps must have THD 20%, while ballasts for 2-foot lamps must have THD 32%. For T8 lamp installations, Instant Start and Programmed Start/Programmed Rapid-start Ballasts, described below, all qualify. Programmed Start/Programmed Rapid-start ballasts must be used for T5 lamp installations. Submit the manufacturer s ballast specification sheet with your rebate form. The specification sheet must document the characteristics listed below for each ballast type. Ballasts earning rebates in this category are not eligible for rebates in categories B & D. Instant Start Ballasts When T8 lamps are being installed for general illumination purposes, Instant Start ballasts must be used. TERMS AND CONDITIONS Programmed Start/Programmed Rapid-start Ballasts When occupancy sensors are installed to control circuits in lamp/ballast retrofits, Programmed Start/Programmed Rapid-start ballasts are recommended. Category E Occupancy Sensor rebates may also apply. All replacement lamps must meet the following CRI and Rated Lamp Life standards: Lamp Type & Size Ballast Type CRI Minimum Rated Lamp Life (3 hours/start) T8 2 ft, 4-ft Programmed Start/ 80 24,000 hours Programmed Rapid-start T8 2 ft, 4-ft, 8-ft Instant Start 80 18,000 hours T5 2 ft, 4-ft, 8-ft Programmed Start/ 82 20,000 hours Programmed Rapid-start Replacement lamps earning rebates in this category are not eligible for rebates under categories B or D. D. INTERIOR HIGH BAY LINEAR FLUORESCENT FIXTURES Only complete new 4-lamp or 6-lamp fixtures qualify. Must use T8 or T5 lamps and ballasts as specified in categories B & C. New fixture must replace, one for one, existing incandescent, T12/PG17 fluorescent or High Intensity Discharge (HID) fixtures in interior high bay installations (over 15'). Existing fixtures must be 400 Watts each. All fixtures must have a reflector with a minimum of 90% reflectivity. Exterior installations (roadway, street lighting, outdoor area, and parking lot lighting) do not qualify. Fixtures in this category are not eligible for additional rebates under categories B & C. E. OCCUPANCY SENSORS This rebate applies to passive infrared and/or ultrasonic detectors only. Wallbox and wall- or ceiling-mounted lighting sensors must be hardwired and control interior fixtures. Self-contained wallbox lighting sensors (defined as units without an exterior switchpack or relay that are designed to replace a standard wall switch) must not control more than 500 Watts. Wall- or ceiling-mounted sensors must not control more than 1,000 Watts. F. LED EXIT SIGNS Only new exit signs that replace incandescent exit signs qualify. Non-electrified (such as tritium) and remote exit signs are not eligible. All new exit signs must meet UL-924 requirements. Exit signs must have a usage level 5 Watts and a minimum product life of 10 years or be listed as ENERGY STAR -qualified. Manufacturer s information stating the model number and ENERGY STAR qualification or other qualifying specification sheet must be submitted with each rebate form. New exit signs must meet local fire codes. Retrofit kits are not eligible. G. PACKAGE TERMINAL AIR CONDITIONERS AND PACKAGE HEAT PUMPS Package terminal air conditioners (PTAC) and Package terminal heat pumps (PTHP) are through-the-wall, self-contained units and are 2 tons (24,000 Btu/hr) or less. Only those units that have an Energy Efficiency Ratio (EER) that is 20% higher than the minimum are eligible. Minimum EER is calculated from the following equation: Min EER = (0.16 x 1,000) (capacity in Btu/hr*) *If the capacity is less than 7,000 Btu/hr, use 7,000. If the capacity is >15,000 Btu/hr, use 15,000. H. SETBACK PROGRAMMABLE THERMOSTATS Must automatically adjust the temperature setting of your heating and air conditioning system at pre-selected times to make sure energy is consumed only when needed. Must have automatic setback feature. The system fan switch must have an on feature that circulates air continuously. I understand that rebate payments are based on related energy benefits over the life of the product. I agree that if: 1. I do not provide PG&E with 100% of the related energy benefits specified in the rebate form for the life of the product or for a period of five (5) years from receipt of rebate, whichever is less, or 2. I cease to be a distribution customer of PG&E during said time period, I shall refund a prorated amount of rebate dollars to PG&E based on the actual period of time for which I provided the related energy benefits as an electric customer of PG&E. I understand that this program may be modified or terminated without prior notice. I understand that this rebate form and the paid itemized invoice must be received at PG&E s Central Processing Center or postmarked no later than December 31, 2005 and that all equipment must be purchased and installed prior to submission of rebate form. If requested, I will allow PG&E or City of San José personnel reasonable access to my facility to verify the installation of product(s) I have purchased before a rebate is paid. I agree that PG&E may release my name, address, and phone number and rebate information to the City of San José for the sole purpose of verifying equipment installation. I agree that PG&E may release my name, address, phone number, PG&E account number and rebate information to the vendor providing and/or installing the equipment under the POWER PLAYERS Rebate Program. In no case will PG&E pay more than 100% of the actual purchase price of the item(s) to be rebated. This program has a limited budget. If no reservation has been made, rebate forms will be accepted on a first-come, first-served basis, until allocated funds are spent, or December 31, 2005, whichever comes first.
