2 National Account, National Fleet Program, ERS Page 1 Bridgestone Commercial Solutions ( BCS ), a business unit of Bridgestone Americas Tire Operations, LLC ( Bridgestone ), will invoice authorized National Account, National Fleet Program, and Emergency Road Service (ERS) at US Standard Service and Labor Rates when these services are authorized by the customer and included on a Delivery Receipt properly submitted to and processed by BCS. National Account Please refer to the BCS Program Guide and/or Tire Policy for specific fleet authorization requirements. National Fleet Program All National Fleet Program transactions require a fleet authorization number. For National Fleet Program, ERS calls, please visit bridgestonenationalfleet.com or call for an authorization number. The following policies and rates apply to deliveries within the United States and are subject to change without notice. I. Policies US Holidays The following holidays and hours are observed: New Year s Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Day Hours Regular Hours After Hours 7:30 a.m. 5:00 p.m., Monday Friday 5:01 p.m. 7:29 a.m., Monday Friday Saturday (24 hours), Sunday & Holiday Sunday (24 hours), Listed Holidays Dealer Specified Pricing () Pricing supplied locally by the servicing dealer. Emergency Service Service that is needed immediately, regardless of location.

3 National Account, National Fleet Program, ERS Page 2 Non-Emergency Service Service that is scheduled or is convenient at the current time, regardless of location. Hourly Labor All EMERGENCY hourly labor rates include the following: All labor charges (including installation of valve stems, etc.) Flat repair (including standard patch and cement) Mounts and dismounts (including inside duals) Charges for additional materials may be assessed at, such as: Valve hardware Studs Tubes/flaps/etc. All Monday Friday hourly labor requires a one-hour minimum and will be billed in 15 minute increments thereafter. All After Hours and Sunday & Holiday service rates require a two-hour minimum and will be billed in 15- minute increments thereafter. All hourly labor rate charges are portal to portal, based on the servicing dealer traveling to the service site, time spent completing the service, and returning to the original start or shop location OR at the completion of service and the commencement of travel to a new service event. Tire Classification Light Truck Tire Medium Truck Tire Heavy Truck Tire All LT designated tires in the Bridgestone Medium & Light Truck Tire Data Book and 8R17.5 and cross-section or LP cross-section and 215/75R17.5 and up. Larger than a 315 cross-section or cross-section. Mileage Fee / Fuel Surcharge Policy for Deliveries and Non- Emergency Service With customer approval, mileage fees or fuel surcharges may be assessed on round trip new tire and retread deliveries and travel to perform nonemergency service work. These rates should be negotiated between the BCS servicing dealer and each customer location. The name, title and phone number of the authorized person who approved the fuel-related charge must be submitted on the DR.

4 National Account, National Fleet Program, ERS Page 3 The Suggested Mileage Fee table on page 3 may be used by BCS servicing dealers as a reference to determine a mileage fee. BCS reserves the right to reverse any mileage fee or fuel surcharge billed to a fleet that BCS determines to be excessive. Mileage Fee Policy for Emergency Service Each emergency service call is subject to a mileage fee billed at the service provider s discretion. The mileage fee should be fair and based on the additional fuel-related cost of the service call. A fuel surcharge is not allowed for emergency service. The Suggested Mileage Fee table on page 3 may be used by BCS servicing dealers as a reference to determine a mileage fee. The mileage fee code YY should be used when billing this fee. BCS reserves the right to reverse any mileage fee billed to a fleet that BCS determines to be excessive. The Suggested Mileage Fee table is based on the national average price of on-highway diesel fuel (all types).
