3 Indiana OSHA Cites City in Worker s Death 2013 The City of Indianapolis was issued $80,000 in proposed fines for an incident involving the death of one of its garbage collection workers. Worker died after being run over by a reversing garbage truck. He was pinned between a tree and the truck. IOSHA cited the city for four knowing violations, five serious violations and one other-than-serious violation. Knowing violations are issued when an employer knew or should have known that conditions were likely to lead to worker injuries or deaths, yet did nothing to address the hazards. IOSHA says Indianapolis Department of Public Works garbage collectors have routinely violated safety standards, including letting workers ride on the backs of garbage trucks traveling faster than 10 miles per hour, letting them ride on the backs of trucks for more than one-fifth of a mile, and allowing them to ride on the rear steps of reversing trucks.

10 Sanitation Safety Checklist and Agreement: This checklist is signed by an employee or temporary worker following review with a Department Manager. The manager reviews the PPE to be issued as well as the safety procedures for a rear loader. Included in this oral overview is Riding on the Platform, Backing Procedures, Operation of packing mechanisms, proper lifting technique and dumping at the transfer station or yard waste site. Copy of Agreement stays with the employee for the duration of the training period. On the Job Training: The employee is assigned to a competent Driver with a good safety record. The Driver physically reviews each of these procedures with the employee prior to initial route departure and monitors compliance on the route. Driver and employee sign Checklist/Agreement and return it to Supervisor. Document is stored in a separate labeled binder to be kept in the Supervisor s office. Supervisor Observation: A supervisor or crew leader will periodically observe and document compliance on the route until satisfied that all safety protocol is being adhered to. Safety Meeting: Topics include Personal Protective Equipment, Blood Bourne Pathogens, Proper Lifting Techniques, Lockout/Tagout, Trips/Slips/Falls and Hazard Communication/GHS. Documented attendance and dates conducted forms are kept in SOP manual in Supervisors office.

11 Backing. While there is a defined method for backing a rear loader (described in Procedure section of this document), the action itself remains hazardous. Visibility issues, constant focus on coworkers and surroundings, overhead lines, parked vehicles and other obstructions are challenging. To compound the matter, there are streets in Mooresville that have no cul de sac or turn around area. These require backing down the entire length of the street to service. Whenever possible the Hopper truck is utilized to service these areas due to its much improved maneuverability. Riding on the Platform. Riding on the outside of a moving truck is inherently dangerous and can result in serious injury. The Town subscribes to ANSI standard of riders allowed on the rear platform only at speeds of less than 10 mph or for a distance of less than 2/10 of a mile (5). In addition, the Sanitation department prohibits riding on the step at all while on heavily trafficked roads. Operating the Packing Mechanism. Pressure levels on a Rear Packer operate at 2,500 psi on the hydraulic side. This force, when spread across the surface area of the blade is strong enough to cause dismemberment or death if the employee comes in contact with it. Packing mechanisms also can cause debris to fall out of the hopper as well as explosions or chemical reactions emanating from disposal of prohibited items. Procedures outlined within this document limit exposure. Dumping at the Transfer Station. Dangers present at the transfer station include heavy equipment and other trucks operating in and around the dumping area as well as discharging compacted waste from the rear of the truck. Preparing the truck for dumping prior to backing into the waste area limits the need for employees to be outside of the vehicle during dumping.

12 Prior to engaging reverse on the truck, driver must stop vehicle, engage parking brake and allow worker to exit the vehicle. Worker should position himself at least 30 feet behind vehicle on the driver s side so that he can make eye contact with the driver via the driver s side view mirror. Once positioned, driver engages truck in reverse and maintains eye contact with worker. Worker ensures Back Up Alarm is functioning. Worker uses an open hand to signal driver to commence movement. 12

13 Worker scans surrounding area for obstructions or hazards while maintaining frequent eye contact with driver via driver s side mirror. Worker uses a closed fist to signal driver to stop for any hazards. When truck gets in vicinity of worker, worker uses closed fist to stop vehicle, moves at least another 30 feet from truck, and signals truck to continue backing. At the end of the sequence, driver stops vehicle, places in neutral (signaled by shut off of backup alarm) and engages parking brake. Worker may then re-approach vehicle. 13

15 Sanitation Safety Checklist & Agreement (Form A) shall be maintained by the Supervisor in a separately labeled binder. Documentation shall be retained for the duration of employment. Supervisor Safety Observation Form (Form B). Periodic Supervisor observations are maintained with the employee s Sanitation Safety Checklist and Agreement and maintained by the Supervisor. Pre-trip Inspection Form (Form C). Completed pre-trip forms are to remain in the vehicle during that work day and returned to the supervisor at the completion of the shift. Supervisor shall keep documentation of Pre-trip inspections for 1 year. Required Safety Meeting Attendance (Form D). Safety meeting attendance records are maintained by the supervisor in the SOP
