4 Quality Deficiency Report The Quality Deficiency Report (QDR), SF-368, is the primary tool used for receiving feedback from vehicle users that are experiencing chronic defects & failures that prevent the safe & efficient operation of a vehicle 4

5 The QDR Form is available on the AutoChoice web site 5

6 QDR Background The primary purpose of a QDR is to: Report recurring non-conformances Initiate steps to effect corrective and preventative actions 6

7 QDR Qualifiers Defects or nonconforming conditions which limit or prohibit the vehicle from fulfilling its intended purpose Deficiencies in design, specification, materials, manufacturing & workmanship 7

8 Reporting Requirement Discrepancies or Deficiencies with respect to Government vehicles occur in two broad categories Quality Deficiencies Recurring Defects Shipping and Transportation Discrepancies are not QDRs and are to be reported to 8

9 What is NOT a Defective Product? Routine Maintenance Items: Flat tires Worn out items such as Brakes, Batteries, Wiper Blades Collision Damage Wear & Tear Abuse IF you aren t sure as to routine maintenance, check with your vehicle s owner s manual, MCC, Fleet Operations, FSR or Vehicle Engineering Representative 9

10 What IS a Defective Product? Recurring failure of a feature or system on your vehicle that occurs at delivery or during normal use within the factory warranty period PLUS 1 year thereafter. A repair that doesn t stay repaired Equipment that continues not to operate as promised Significant defects upon delivery of the vehicle o Gaps in sheet metal o Misaligned glass o Improper or incomplete assembly o Mold, dirt, cuts & stains in fabric surfaces o Missing equipment 10

11 Discrepancies and Deficiencies Here s the Statute - Code of Federal Regulations, Title 41, Volume 2, Chapter 101, Part , prescribes uniform system for reporting discrepancies or deficiencies in materials 11

12 Types of QDRs Category I Critical defect which may cause death, injury, or severe illness; would cause loss or major damage critically restricting an agency s mission Category II Major or minor defect which does not meet the criteria set forth in Category I Vehicle QDRs are typically Category II 12

13 Reporting QDRs Prompt reporting of quality deficiencies are essential to maintain an acceptable quality level for the Government s vehicles A QDR can help to repair or replace the defective vehicles Also, QDRs can document contractor performance files for use in future contract awards 13

14 I think I might have a QDR, what should I do (step 1)? Light duty vehicles - GM, Ford, FCA, RP Automotive- end user customer contacts the dealer and has the vehicle inspected and repaired under warranty Medium/Heavy vehicles - end user customer contacts Prime Contractor (or their authorized service center as directed by the prime) and has the vehicle inspected and repaired under warranty If my vehicle still is not fixed, then what? Fleet customer? Contact your FSR Purchase customer? Contact vehiclebuying@gsa.gov for additional guidance 14
