Mitek Sports Medicine Scope-Only Program MITEK SPORTS MEDICINE offers comprehensive Service and Repair coverage (See Table 2) for all of your scope needs. With set pricing per scope, keeping your pool of scopes in pristine working order is easy and hassle-free. As part of the annual scope replacement program, your scopes are replaced as often as you need at a predetermined, set price. It's easy. Here is how it works for scopes: 1. You determine the number of scopes you wish to cover. 2. A per-scope replacement price will be established based on your scope volume. 3. A PO is opened and billed against as your scopes are replaced.

6 Pay As You Go Mitek Sports Medicine Repair for Capital Equipment Available when you need it most With Repair Service, no service contract is required so it is available to you, when you need it. Your repair fee is based on the extent of damage no more and no less. And to ensure continuous OR uptime, certified loaner replacements are available for the entire duration your equipment is out for repair. (See Table 3) Here is how it works: 1. You: Contact Service and Repair, describe the damage and receive an RMA #. Send in your damaged equipment to Service and Repair. 2. Service and Repair: Overnights a Certified Loaner Replacement at your request. 3. Service and Repair: Evaluates returned item to determine if repair is Minor or Major. Sends you a quote for approval. 4. You: Receive repaired component within two to three weeks and return the Certified Loaner Replacement to Service and Repair. Mitek Sports Medicine Repair for Scopes With Repair Service, no service contract is required so it's available for your scopes when you need it. (See Table 4) Here is how it works: 1. You: Contact Service and Repair, describe the damage and receive an RMA #. Send in your damaged equipment to Service and Repair. 2. Service and Repair: Evaluates returned scope to determine if repair is Minor or Major. Sends you a quote for approval. 3. You: Receive repaired scope within two to three weeks. Minor Repair A minor repair is defined as a repair that does not involve the opening of the internal optical system. Minor Repair Examples Removal of stuck adapter Polishing of the objective lens Polishing of the fiber cone Scope Repair Service Major Repair A major repair is defined as a repair that involves opening of the internal optical system to restore the scope to its original condition. Major Repair Examples Replacement of: Damaged objective lens system Damaged rod lenses Damaged optical spacers Internal cleaning w/replacement of rod lenses Resealing of optical tube assembly Complete scope cleaning Severely damaged or bent scopes
