1 SECTION 2 DESCRIPTION OF CLAIM The following pages describe all the entries used in filing a claim to DDC. Some data entry fields are required in generating a claim and are designated by an asterisk (*) next to the field description on the claim entry screens. 2
2 * * * * 1) Enter the number which identifies the nature of the claim being submitted. The following describes the various claim types.refer to section E for detailed information on each claim type. Click on List for more information. These codes will be entered automatically on electronic claims.in instances where a claim administrator is responsible for multiple locations, a drop down menu will be available to choose the appropriate code for a specific claim. 3) RO Number Enter the number which corresponds to the actual repair order number used by the DST/DLR to reference the repair. The repair order number is necessary to support the claim for payment and audit purposes. 4) RO Date This is the date the repair order was opened. Enter in day,month, year sequence (using all numbers,i.e. 03-JUN-2004). This date is used to verify warranty eligibility as related to the engine s delivery date. It also establishes the extent of warranty coverage, and applicable parts and labor prices.click on the date box for complete list. 3
3 5) DDC Engine Serial Number Enter the complete serial number of the engine on which the claim is being submitted. Enter the ten (10) digit serial number on the claim. The serial number can be found on the option label and the emissions label located on the valve cover, and also stamped on the block. Examples of the various new DDC engine serial numbers are shown below: 06R D VA VF G R A VA VF L A VA E D A VA E D A VF E D VA VF E Detroit Diesel manufactured Series 2000 and 4000 engines also require all ten (10) digits of the serial number to be entered on the claim. A change point exists where the fourth and fifth positions of the serial number have been switched.these are as follows: OLD NEW 8V V V V V A) Serial Numbers for Engines not Manufactured by Detroit Diesel(or by DDC in North America) DDC also warrants various engines supplied by other manufactures. 5 5B) DDC Marine Transmission Serial Number Enter the complete serial number of the marine transmission on which the claim is being submitted. The serial number can be found on the instruction plate which is located on the transmission main housing. Example of an identification plate and serial numbers are shown below: Identification Plate Serial Numbers 05AG AS BC AP S FH ) Model Number Enter the complete DDC model number on which the claim is being submitted.the engine model number can be found on the option label,emission label or block along with the engine serial number. The marine transmission model number can be found on the transmission identification plate or main housing. Example model numbers are shown below. An explanation of the model number can be found in the General Information section of the engine service manual Series Series V Inline GK60 Series Series GK28 Series Series 149 DD5111A Marine Transmission G067GK40 Series 55 R1627K02 Series 2000 T1237K11 Series MT60 MBE MT60 MBE900 7) Application Code Enter the application code for the engine being repaired. Refer to Section 10 for a list of the application codes. This entry is used to determine the applicable warranty coverage. Click on "List" for more information. 8) Delivery Date Enter the day, month, year sequence (i.e. 03-JUN-2004) when the engine was delivered to the first retail purchaser.if the engine has not been delivered to the first retail purchaser, leave this space blank and make a note in the "Comments" section of the claim, i.e. stock unit", not delivered" or OEM stock". If the engine has been used as a demonstrator refer to Section D, of the Service Policy Manual for details. On Replacement Parts claims (CT 05 & 53), the date used should be the date the failed part was installed in the engine by an authorized DDC service outlet, or the date of sale on "over the counter" purchases. This date is used to establish the effective date of the applicable warranty. 9) Complaint Code Enter the code which best describes the complaint made by the customer ( refer to the list of Complaint Codes, Section 11 of this manual ). Click on List " for more information. 10) Failure Code Enter the code that best describes what was wrong with the primary failed part ( refer to List of Failure Codes, Section 12 ).This code is used to define the failure mode of the primary failed part or reliabilt components and assist in making evaluations of product reliability and durability. Click on " List " for more information. 6
6 11) Primary Failed Part Number Enter the part number of the specific part determined to be the primary cause of the failure. Do not enter the part number of service kits or assemblies which contain the failed part. This entry is used by DDC in making evaluations on product reliability and durability. If a Detroit Diesel reliabilt remanufactured component ( such as a remanufactured turbo, ECM or water pump ) is the primary cause of the failure, then show the complete part number of the failed reliabilt components, including the leading "R". Examples: Remanufactured Turbo R Not , Water Pump R Not ECM R Not , Blower R Not & Not ) Requested Amount Enter the total amount requested for payment from DDC. This total must reflect the entire cost of parts, mark-up, labor, net items, travel time and mileage less any applicable "P-3" deductible. * * 13) Equipment Make Enter the name of the OEM who made the piece of equipment/ vehicle the engine is powering. This information is required on all claims. Click on " List " for more information. Examples of various Equipment Makes are shown below: Freightliner American LaFrance Sterling Kenworth Link Belt MCI NABI OBI Oshkosh Western Star 14) Enter the "Model Number " of the piece of equipment/vehicle the engine is powering. This information is required on all claims. Click on " List" for more information. Examples of various Models for the Equipment Makes are shown below: (Freightliner) C120 (Sterling) Acterra (Freightliner) CST 120 (Sterling) L Series (Freightliner) CL120 (Western Star)
7 15) Equipment VIN Enter the Serial Number or VIN of the piece of equipment/vehicle the engine is powering. This information is required on all claims. 16) Miles / HRS Miles/Kilometers are to be entered for on-highway type applications. Hours are to be entered on all other applications. Enter the total miles/kilometers or hours the engine has operated. Enter the miles/ kilometers or hours on the failed part for Replacement Part warranty and Replacement Part policy claims (measure only from when the part was installed or purchased). All mile/kilometer or hours entries should be rounded off to the nearest mile/kilometer or hour. DO NOT SHOW TENTHS OR USE PUNCTUATION. This is one of the elements used to establish warranty coverage on the engine or part. Example: 110,526.8 Enter ) Owner Name Enter the owner s name, using standard abbreviations where possible. In the case of individuals, place the last name first, followed by the first and middle intials. Examples: Ajax Mfg. Corp. Jones, P.M. 18) Owner Zip Code Enter the zip or postal code of the engine s operating location. 19) CMCS This is an information box only, CMCS is used for tracking failed parts to Reman and DDC RM Room. This is a 7 digit number. 20) Modifications Enter the modification/campaign number that you have completed to identify special claims activity or programs by DDC. 21) Exchange Rate This is a system generated field and no entries are to be made in this area. 22) Policy Number The appropriate E-policy number for the claim that you are requesting is automatically entered when E-policy is created. 23) Authorization Number Enter the number given for ECM/Injector change out or the /Field Authorization Number given by DDC Field Personnel to authorize the repair. 24) Authorization Amount This box is for factory use only. No entries are to be made in ths box. 8
8 25) Add l. Claims Line Items Enter the number of the additional blank lines that you will need. By entering a new value and saving more lines will appear. 26) Enter New Line Items Enter the item that you are adding, such as part, labor, net item. The dropdown menu allows you to select the type of item to enter. Note the travel hours and travel time are located here. 27) Line Item Code Enter the part number or labor code in the code box of the line item. Click on " List " for more information. List the part numbers of the Detroit Diesel parts used in the repair. If reliabilt parts are used precede the part number with a "R". 9
9 Example: Reman Turbo Show R Not Part number are not to be duplicated on the claim (except CT 06). Show the total quantity used on one line. Part numbers are used in conjuction with repair order open date to compute the amount service outlets are reimbursed for parts used in the repair. Labor Operation List each labor operation in this column Applicable labor operation number can be determined by referencing the applicable Labor Time Guide. A labor operation must only be listed once, but can be added together. General labor operations (i.e ) must be completely explained in the "Comments" or narrative section of the claim. 28) Requested Quantity Enter the total quantity of each Detroit Diesel part used in the repair or the amount of labor ( in tenths of hours i.e. 1.7 or 1.0) that you are claiming that match the item code on the list. This quantity is used to compute the total reimbursement for each part or labor used. Actual Time Enter the ACTUAL REPAIR time required for each operation in this column, except for "flat rate operations" in which case the servicing outlet should enter the time shown in the Labor Time Guide. All " flat rate operations" are designated in the applicable Labor Time Guide. If additional time is needed to be claimed in excess of SRT Times in the labor time guide it should be listed as a general labor operation on the claim, and also shown in the claim narrative section, with a complete explanation. This entry is used to compute the total amount that service outlets are reimbursed for labor used in performing the repair. NOTE: Lack of time documentation may result in a chargeback of all or a portion of the claim. 10
10 29) Net Item Enter the item used, for claiming the following items: Duty Code D Sales Tax Code X Freight Code F Sublet Code S Towing Code T Overtime Code O Complete Engine Code U Special Labor Code L Skids Code P Handling Fee Code P Non DDC Items Code P The total dollar amount of each net item should be shown under requested quanity. All Net Items must be explained in the "Comments" section of the claim. NOTE: Reference Section 4 for a complete explanation of Net Items. 30) Travel Hours Enter in hours and tenths, the actual travel time claimed for the serviceman traveling to and from the repair site. THIS ENTRY MUST BE IN HOURS AND TENTHS, and is used to compute the amount of reimbursement for travel time (refer to section J of the Service Policy Manual, Schedule of Credits, for when eligibility of travel hours can be claimed). Example: 1 Hour Enter 1 2 and Half Hours Enter ) Travel Distance Enter the number of miles or kilometers claimed by the serviceman for traveling to and from the repair site. This entry is used to compute the amount of reimbursement for miles or kilometers. (refer to Section J of the Service Policy Manual, Schedule of Credits, for when eligibility of miles/kilometers can be claimed). 11
11 32) Narrative Text Enter the complaint, cause, and correction. Enter the complaint made by the owner or operator, primary cause of the failure, and correction required to accomplish repair. Any additional comments/ explanations pertinent to the repair. Description/explanation of net items( Refer to Section 4 ). List any General Labor operation used as well as explanation of work performed. Explanation of any labor times which are in excess of the Labor Time Guide guidelines. Save the claim after you have completed your narrative. You can than click " Submit" to send the claim to DDC. 12
