Nijhaan Premkumar (Report No. 7007) Applicant for the Renewal of Tow Truck Driver's Licence No. D Tow Truck Driver's Licence will be renewed, subject to the following conditions: submitted to the satisfaction of Municipal Licensing and Standards within 30 days of the Tribunal hearing, failing which the licence may be (2) Immediately upon being renewed, Tow Truck Driver's Licence will be placed on probation for a period of two(2) years; (3) Prior to each of the next two (2) renewals of the licence, Nijhaan Premkumar must provide to Municipal Licensing and Standards, at his own expense, an original up-to-date abstract of his driving record; has concerns with any new charges or convictions against Nijhaan Premkumar, those matters and report no. 7007, and any updating Ontario Inc. (Report No. 7011) o/a Galaxy Fresh Foodmart Liang Wen Fang, Director En Wei, Director Holder of Retail Store (Food) Licence No. B Retail Store (Food) Licence will have the following conditions placed on it: (1) Retail Store(Food) Licence will be suspended for a period of four (4) days to take place on July 31 st, August 15 th, August 22 nd and August 29 th, 2018; (2) Retail Store (Food) Licence will be placed on a probation for a period of two (2) years to commence on the date this Proposed Resolution is approved by the Toronto Licensing Tribunal; (3) The licence will be suspended for ten (10) days, according to the following terms: i. The suspension of the licence is suspended ii. During the probationary period, if the licensee is convicted under a charge under the Holiday Shopping Bylaw (other than the offences of March 30 and April 1, 2018), Ontario Inc. must notify Municipal Licensing and Standards in writing within 3 business days; iii. During the probationary period, if the licensee is convicted of a charge under the Holiday Shopping bylaw, the 10 day suspension will take effect automatically upon notification to Municipal Licensing and Standards; 2

Caulfield, President Holder of Pedicab Licence Nos: V , V , V , V , V , V , V , V Pedicab Driver's Licence and Pedicab Owner's Licences will continue, subject to the following conditions: Condition No. 1 has been added as an amendment to the Proposed Resolution dated June 13, (1) Pedicab Owner's Licences will continue without conditions. (2) Mr. Caulfield's Pedicab Driver's Licence will be suspended for a period of ten (10) days, every Monday, commencing on July 9, 2018 up to and including September 10, 2018; (3) Mr. Caulfield's Pedicab Driver's licence will be placed on probation for a period of two (2) years; has any concerns with any new charges or convictions against Clayton Alexander A. Caulfield or against Ontario Inc., those matters and report No. 6744, and any updating material may be brought back before the Tribunal for a full hearing. 3

Arukshan Vijayakumar (Report No. 7006) Applicant for a Tow Truck Driver's Licence (Application No. B814138) The Tribunal dismissed Arukshan Vijayakumar's application for a Tow Truck Driver's Licence without a hearing. (Mr. Vijayakumar failed to attend today's hearing) 6. Rajveer Singh (Report No. 7002) Applicant for a Vehicle-For-Hire Driver's Licence (Application No. B806457) Rajveer Singh requested that his application for a Vehicle-For-Hire Driver's Licence be cancelled. Report No was withdrawn by Municipal Licensing and Standards. (Mr. Singh did not attend today's hearing) Ontario Inc. (Report No. 6771) o/a Sunny Convenience Mr. Zubayer Rifat, President Holder of Retail Store(Food) Licence No. B Retail Store Licence No. B is cancelled, therefore Report No was withdrawn by Municipal Licensing and Standards. (Mr. Rifat did not attend today's hearing) 8. Ayaaz Nathalia (Report No. 7022) Applicant for a Vehicle-For-Hire Driver's Licence (Application No. B818822) Vehicle-For-Hire Driver's Licence will be issued to Ayaaz Nathalia, subject to the following conditions: 4

5 submitted to the satisfaction of Municipal Licensing and Standards within 30 days of the Tribunal hearing, failing which the application may be (2) Immediately upon being issued, Vehicle-For-Hire Driver's Licence will be placed on probation for a period of four (4) years; (3) Prior to each of the next four (4) renewals of the licence, Ayaaz Nathalia must provide to Municipal Licensing and Standards, at his own expense, his original up-to-date criminal record; has concerns with any new charges or convictions against Ayaaz Nathalia, those matters and report no. 7022, and any updating material, may be brought back before the Tribunal for a full hearing. 5
