Owais Anwar Ali Ladhiwala (Report No. 6578) Applicant for a Taxicab Driver's Licence (Application No. B542777) Owais Anwar Ali Ladhiwala s licence will be issued, effective immediately, subject to the following conditions: (2) Mr. Ladhiwala must provide to Municipal Licensing and Standards proof of for a period of two (2) years to commence on ; (4) Prior to each of the next two (2) renewals of the licence, Mr. Ladhiwala expense, an original updated abstract of his criminal record; (5) During the probationary period, if Mr. Ladhiwala incurs any charges and/or convictions under the Toronto Municipal Code or the Highway Traffic Act, and any new charges and/or convictions under the Criminal Code, he must notify Municipal Licensing and Standards, in writing, within three (3) business days. Goldenberg for a period of six (6) months to December 31, Michael Tommy Welna (Report No. 6577) Applicant for a Taxicab Driver's Licence (Application No. B645945) Michael Tommy Welna s licence will be issued, effective immediately, subject to the following conditions: within 30 days of the Tribunal hearing, failing which the licence will be cancelled; (2) Mr. Welna must provide to Municipal Licensing and Standards proof of for a period of four (4) years to commence on ; (4) Every six (6) months for the next four (4) yearly renewals of the licence, Mr. Welna must provide to Municipal Licensing and Standards, at his own expense, an original updated abstract of this driving record, and original updated abstract of his criminal record to be submitted yearly. The notification shall include his ML&S licence number and the ticket number(s). Mr. Welna can notify Municipal Licensing and Standards in one of the following ways: report No. 6577, and any updating material, shall be brought back 8.
