4 Page 4 of PURPOSE 1.1 This document provides instructions for safe operation of motor vehicles for personnel and equipment/material transport at the Portsmouth Gaseous Diffusion Plant (PORTS), in order to minimize hazards to employees and property. 1.2 This document implements requirements of Federal and State (Ohio) laws for Fluor-B&W Portsmouth LLC (FBP) and contractor requirements. 2.0 SCOPE AND APPLICABILITY 2.1 This Level 2 procedure applies to all vehicle operations for personnel and equipment/material transport at PORTS. 2.2 This procedure applies to all FBP and contracted labor resources personnel who use privately-owned/ leased, company owned, or government-owned/leased vehicles, including utility-type vehicles (e.g., golf carts, gators, and mules), during the performance of FBP project work. The accomplishment of official business is the sole reason for operating a government-owned/lease vehicle. 2.3 FBP and contracted labor resources personnel are encouraged to use governmentowned/leased vehicles, when performing on-site work that is designated as part of a work package. This includes secondary tasks associated with support or performance of a work activity such as movement of equipment, tools, and materials to and from a job site. Exceptions: This procedure does NOT apply to powered industrial trucks (PITs), or non-motorized construction equipment. 3.0 GENERAL INFORMATION None 4.0 USE REFERENCES A. 5 Page 5 of RESPONSIBILITIES 5.1 Supervisor Ensures that employees maintain proper licenses and knowledge of safety requirements established by this procedure Documents vehicle accident information. 5.2 Occupational Safety and Health (OS&H) Professional Assists in accident investigations Assists in establishing adequate facility postings and traffic patterns. 5.3 Vehicle Operator Knows and adheres to procedure requirements for safe operations and proper use of all defined vehicles Maintains applicable state driver's license(s) Reports any vehicle problems or potential problems to supervision immediately upon identifying the existing or potential vehicle-related deficiency Reports all vehicle accidents. 5.4 Pedestrian Knows and adheres to procedure requirements for safe pedestrian travel. 5.5 Design Engineer Assists in reviewing and approving vehicle modifications. 5.6 Facility Manager Establishes and maintains safe traffic patterns for vehicles and persons (refer to Appendix A, Controls for Establishing Safe Traffic Patterns). 5.7 Leased Equipment Manager Provides oversight to the leased equipment program. Receives weekly inspection forms for all leased equipment. 5.8 Garage Supervisor Schedules and performs maintenance on Government-owned equipment Receives weekly inspection forms for all Government-owned vehicles.

6 Page 6 of ACTIONS 6.1 General Requirements Occupational Safety and Health Professional Supervisor Discuss vehicle safety issues at safety meetings, as necessary Ensure that motor vehicle operators are properly trained and qualified prior to operating any vehicles. 6.2 Vehicle Inspection Vehicle Operator Check all vehicles in use prior to first use each shift, to ensure that the following parts, equipment, and accessories are in safe operating condition and free of apparent damage that could cause failure while in use: Service brakes, including trailer brake connections, parking system (hand brake), and emergency stopping system (brakes) All manufacturer-installed safety devices (e.g., back up alarm and/or sensor) Lights Tires Horn Steering mechanisms Coupling devices, if applicable Seat belts Operating controls Safety devices not installed by manufacturer, if applicable Condition of the windshield and other glass Windshield wipers Other conditions that could cause the vehicle to be unsafe to operate

7 Page 7 of 22 Slow Moving Vehicle sign, if the vehicle s maximum speed is 25 MPH or less and operated outside the Controlled Access Area (CAA)/Limited Access Area (LA) A documented inspection must be performed weekly and the completed -F02, Vehicle Inspection Form, forwarded to the Leased Equipment Manager, or Garage Supervisor. This requirement does not apply to privately owned vehicles (POV) Vehicles with a Gross Vehicle Combined Weight Rating (GCWR) in excess of 26,001 lbs. (defined as a Commercial Motor Vehicle) or vehicles which carry hazardous materials requiring a placard require a commercial driver s license (CDL) to operate and the drivers of such vehicles must be part of the driver program defined in FBP-WM-PRO-00052, PORTS Motor Carrier Operations. In addition to the daily checks in Section above, commercial motor vehicle operators shall complete the daily, and pre-move inspections and checks listed in FBP-WM-PRO For carts operated at PORTS, prior to first use each shift, complete an inspection, and complete -F01, Operator Checklist for Fossil Fueled/Electric Carts Ensure that any carts operated at PORTS meet the following requirements: A. Carts outdoors in daylight shall have at least one working tail/brake light. B. Carts outdoors after dark or inside buildings shall have the following: Minimum one headlight Minimum one tail/brake light Minimum one flashing light Take vehicle out of service immediately if vehicle cannot be operated safely; ensure vehicle is tagged out, according to FBP-OS-PRO-00014, Accident Prevention/Equipment Control Tags Notify Supervisor of any items that do not satisfactorily pass inspection. 6.3 Vehicle Operations Supervisor Ensure employees do not operate motor vehicles when alertness is impaired by fatigue, illness, alcohol, or drugs, or when they are otherwise unfit for duty. Drivers with CDL have additional requirements for drug testing and fitness for duty in accordance with FBP-WM-PRO

8 Page 8 of Approve any modifications made to vehicles before use with guidance from Design Engineering or the manufacturer. Vehicle Operator NOTE Operators and passengers need to be conscious to keep body parts inside of the carts to prevent contact with objects such as columns when inside buildings FBP does not have a permit to transport hazardous material off-site except for certain Materials of Trade used to perform work at an offsite location. Therefore, do not transport hazardous materials offsite without contacting the Transportation Department On-site transportation of hazardous materials shall be conducted in accordance with the DOE-approved POEF-FBP-010, Transportation Safety Document for the On-Site Transport of Hazardous Material at the Portsmouth Gaseous Diffusion Plant Piketon, Ohio, as implemented in procedure FBP-WM-PRO-00272, On-Site Transportation of Hazardous Materials IF Operator cannot sit comfortably in operator s seat with extremities inside the cab of the cart during operation (except when signaling), THEN notify Supervisor. Do NOT operate cart Ensure pedestrians are always given the right of way Ensure Vehicle Operator has a valid driver s license prior to operating a motor vehicle. IF state driver s license is suspended or revoked, THEN immediately notify supervision Do not operate any type of motorized cart unless issued a cart license, following completion of training and Job Performance Measure (JPM) Follow all traffic sign postings and speed limits Do not exceed the posted plant site speed limit Report any deficiencies and any operational defects to Supervisor immediately IF vehicle is equipped with seat belts, THEN ensure that all occupants wear them Clear obstructions (e.g., ice, snow, sleet, dust, etc.) from windshield and windows prior to operating vehicle.

9 Page 9 of Perform a 360 degree walk-around of the vehicle prior to movement in order to: Verify no personnel are in the cargo compartment Ensure the load is properly secured The vehicle has no obvious defects The vehicle can be moved safely Identify if a spotter is required. Utilize spotters WHEN tight spaces and/or obstacles have been identified by the 360 degree walk-around inspection Do not operate a motor vehicle within the affected PROCESS AREA or Immediate Evacuation Zone (IEZ) of a Category 2 nuclear facility during loss of the Criticality Accident Alarm System (CAAS) coverage, except for vehicles required for CAAS operability restoration Observe the area behind the vehicle prior to backing up Sound the horn two (2) times prior to backing up any vehicle if such vehicle is not equipped with a backup alarm Take special precautions when driving in congested or high-hazard areas, such as the area around a process building, track alleys, inside buildings, and around construction sites Do not drive over unprotected air hoses, electrical cords, cables, etc Do not block driveways, access to parking lots, or emergency vehicles access; or park within designated zone of a fire hydrant (within 10 feet of hydrant), marked or unmarked Yield the right-of-way to all emergency vehicles when they are responding to an emergency Emergency vehicles, when responding to an emergency, shall utilize both emergency lights and audible signals. IF emergency vehicles are not equipped with or displaying emergency lights and audible signal, THEN all traffic rules should be obeyed Take special precautions when driving in congested or high hazard areas, such as round the process building track alleys.

10 Page 10 of 22 NOTE UF 6 cylinder haulers include the following: Forklifts (when loaded with a UF 6 cylinder) Wagons (when loaded with a UF 6 cylinder) Straddle Buggies/Carriers (loaded or empty) Cylinder Stackers (loaded or empty) Any other mobile equipment transporting a UF 6 cylinder Oncoming traffic must pull to the side of the road and stop while cylinder haulers pass. All following traffic shall maintain a 100-foot clearance; do NOT pass cylinder haulers Display the appropriate signal prior to turning the vehicle Pass slow-moving vehicles with caution Do not move a vehicle until all occupants are seated and seatbelts are fastened. Operator must verbally ask if all passengers are buckled Do not exit a vehicle until it has come to a complete stop. NOTE An operator is considered to have left a vehicle unattended if he/she is more than 25 feet from the vehicle, or the vehicle is out of sight Prior to leaving a vehicle unattended, place the transmission in park (for automatic) or low gear (for manual transmission), and shut off the motor Do not permit anyone to ride in the bed of a truck, or in a trailer WHEN traveling off paved roads during dry conditions, THEN, where possible, park the vehicle in areas that are clear of weeds or other combustible materials. NOTE Setting the parking brake in extremely cold weather (5 F or less) could result in the parking brake freezing. Setting the parking brake under these conditions may not be advisable unless parking on a steep grade WHEN parking on a grade, THEN set the parking brake Do not use cellular phones while driving a vehicle or operating equipment, unless the phone is used with hands-free technology.

11 Page 11 of 22 NOTE Motor vehicles may be left running during cold weather, provided that the equipment is within sight of a qualified operator and left in a safe condition. Equipment safe condition is one that complies with the manufacturer s recommendations to ensure that the attachments would not suddenly move in the event of hydraulic failure or engine failure Before leaving unattended commercial motor vehicles, securely set the parking brake and take all reasonable precautions to prevent the movement of the vehicle WHEN operating a vehicle indoors, THEN observe the following: A. Keep all body parts inside the vehicle. B. Slow down and sound horn, when equipped, when approaching blind spots or intersections. C. Do not park vehicles in an area that would block or impede access to or exit from a building. D. Drive vehicle into building only when necessary to perform work. E. Travel designated lanes, when possible, inside of building. F. Ensure overhead doors are fully opened before entering or exiting the building through the doorway. G. Turn on headlights and flashing lights in a building, or outdoors after dark. H. Park only in approved parking areas. I. Travel at a speed no faster than a brisk walk. J. Yield to pedestrians and work crews WHEN loading a vehicle, THEN observe the following: A. Load vehicles so the operator s view of the road is unobstructed and the vehicle can be operated safely. B. Ensure all loads are properly secured. C. Ensure that material, such as gravel or demolition debris, is not heaped over the top of the side boards of the truck bed. D. Ensure that loads do not extend so high that they present a problem while passing under overhead obstructions, such as wires or piping.

12 Page 12 of 22 E. Ensure the vehicle is not loaded heavier than its designed capacity, or off-center. F. Ensure that loads that extend beyond the side or rear of the bed are secured from shifting, and that they are marked on the back and/or driver side with red flags or warning light. In some cases, loads may need to be escorted with a warning vehicle WHEN operating special mobile equipment, THEN observe the following: A. Ensure that there are to be no other passengers other than operators riding on the mobile equipment, unless there is a passenger seat provided. B. Ensure equipment is being supplied with the appropriate safety devices prior to operations (e.g., horn, back-up alarms, and lights). C. Ensure equipment which is capable of contacting an overhead power line is moved with an observer who can give a timely warning to the operator of the equipment. D. IF the vehicle s maximum speed is 25 MPH or less and is operating outside the CAA/LA, THEN ensure the vehicle is equipped with a Slow Moving Vehicle sign. For such vehicles, it is recommended that a spotter vehicle be used when traveling on roadways accessible to the general public WHEN operating non-motorized vehicles (e.g., bicycles, tricycles), THEN observe the following: A. All of the general traffic rules and signs that govern vehicle traffic also apply to non-motorized vehicles. Non-motorized vehicles should come to a complete stop at all stop signs. B. Travel on the right side of center on all roads, and follow the same traffic pattern as other vehicles. C. Yield to faster moving vehicles. D. Tandem riding on bicycles is prohibited. Only one person may ride on bicycles at a time. E. WHEN operating during hours of darkness, THEN ensure the nonmotorized vehicle has front and rear lights, and is equipped with reflectors or reflective tape. F. Do not ride non-motorized vehicles two abreast. Ride in a single file on the right half of the lane.

13 Page 13 of 22 G. Do not ride non-motorized vehicles inside buildings at speeds greater than a brisk walk or at a speed greater than needed to maintain safe operation. H. Rider should wear a bright colored safety vest or high-visibility clothing. NOTE During an emergency evacuation of a building, non-motorized vehicles should not be walked through a pedestrian doorway If operating non-motorized vehicles, they must travel in and out of buildings through the vehicle passageways with the overhead door fully opened. 6.4 Pedestrian Safety Pedestrian EXCEPTION: Bicycles (2-wheeled only) may enter and exit through pedestrian doorways provided that the operator is completely dismounted and pushing the vehicle Walk on sidewalks, where provided, and cross street and roads at designated crosswalks, when available. WHEN sidewalks are not available, THEN walk on the left side of the road, facing oncoming traffic Look in both directions prior to crossing any roadway Do not walk through roll-up doors, when possible. 6.5 Traffic Control Devices Facility Manager Evaluate and establish safe work zone by using traffic control devices, as necessary Ensure traffic control devices are highly visible and spaced so vehicle operator will not deviate from the assigned traffic flow Establish and maintain safe traffic patterns (refer to Appendix A). 6.6 Vehicle Accidents Vehicle Operator Do not move the vehicle unless the vehicle configuration constitutes a safety hazard Do not leave the scene until authorized to do so by investigating officials.

14 Page 14 of 22 Supervisor Notify the Plant Shift Superintendent ( , or 911 on a site landline) of the accident Notify immediate Supervisor or manager of the accident In the event of a motor vehicle accident, initiate First Notification and/or Problem Report, according to FBP-QA-PRO-00128, Control of Nonconforming Items, and/or FBP-QP-PRO-00020, Problem Reporting and Issues Management Arrange for a substance abuse test, according to FBP-HR-POL-00026, Workplace Substance Abuse Program Accidents involving commercial motor vehicles have additional requirements in accordance with FBP-WM-PRO Refueling of Vehicles and Equipment Vehicle Operator IF refueling vehicles or equipment at construction sites or at FBP project sites, THEN follow the Fire Protection requirements. NOTE In the event of a fire during refueling activities, personal safety is paramount. Do not refuel any vehicle inside of a building, cylinder yard, an enclosure, or within 5 feet of an opening to a building or enclosure without prior approval by the Fire Department IF a fire occurs during refueling activities, THEN perform the following: A. Relocate to a safe location. B. Notify the local fire department by dialing 911 or other designated phone number. C. Do NOT attempt to fight a fuel fire unless: 1) You are properly trained in the use of fire extinguishers. 2) The fire is small (incipient stage). 3) A safe escape pathway is present. 4) Emergency help has been summoned.

15 Page 15 of Upon arrival at the off-site fixed service station, position the vehicle to be refueled adjacent to the pump to be utilized, and perform the following: A. Park the vehicle. B. Set the emergency brake, if equipped. C. Switch the ignition to the off position. D. Follow any additional postings and/or requirements at or near the pump. E. Note the location of any emergency full shut-off switches and fire extinguisher locations, if visible. F. Do NOT lock the nozzle lever in the open position if there is no hold open latch provided. G. Do NOT smoke during refueling activities. H. Do NOT use cell phone during refueling activities. I. Pump fuel. J. Remain outside of vehicle in proximity to the pump/nozzle assembly during refueling operation, and be alert for equipment malfunctions that could result in overfilling or spillage of fuel during refueling. K. WHEN the nozzle shuts off, THEN stop filling the vehicle. L. Do NOT top off the fuel tank. M. Allow space for fuel expansion. N. Return hose/nozzle assembly to holder on pump. O. Perform a 360-degree walk-around inspection of the vehicle prior to driving away from the pump. P. Look for potential obstructions that, if struck, could cause damage to the vehicle. 6.8 Recharging of Battery-Powered Carts Vehicle Operator Do not charge equipment in hazardous areas (e.g., confined room, under stairwells, in locations where chemical products are being used, etc.).
