13 CRHIRP Sample Supporting Documents General Requirements: Vehicle Purchase/Lease Agreement The date on the purchase/lease agreement must be within 6 months of the date of application. In some cases, the date of delivery of your vehicle may be considered your date of purchase/lease. A purchase/lease agreement confirms the sale or lease of a vehicle and may look different depending on where the vehicle was purchased/leased. You must submit a complete, clear and legible scan/photo with all details visible and all pages present. You must submit a final copy (executed and signed). Note: Review or pro-forma copies are not accepted. If you do not have the final copy, request it from your dealership or leasing agency. You can submit one of the following documents as a complete purchase/lease agreement: o Sample 1: Purchase agreement, if purchased o Sample 2: Lease Agreement, if leased o If the vehicle was purchased in a private-party sale, a copy of the current Certificate of Title listing the new owner s information will be sufficient. Alternate Document Requirements: If your dealership provided you a Final Price Sheet and a Vehicle Configuration instead of a multi-page purchase/lease agreement, please submit ONE of the following document combination options (A or B) as your purchase/lease agreement: o Option A: Sample 3: Motor Vehicle Purchase Agreement: Vehicle Configuration (with order date and VIN present) AND Sample 4: Motor Vehicle Purchase Agreement: Final Price Sheet o Option B: Sample 3: Motor Vehicle Purchase Agreement: Vehicle Configuration (with VIN present, but no order date) AND Sample 4: Motor Vehicle Purchase Agreement: Final Price Sheet AND Signed Delivery Declaration or signed Terms & Conditions 3 13

18 CRHIRP Sample Supporting Documents Proof of Vehicle Registration Your vehicle must be registered to your current, residential SCE address and your registration document must be valid. o If your registration lists an out-of-date address, you may also submit a Department of Motor Vehicles Change-of-Address Form showing that your address is now changed to your current, residential SCE address. Your document must be a complete, legible scan or photo with all details visible. Documents that can be used as Proof of Vehicle Registration include: o Sample 5: Permanent Registration (Registration Card) o Sample 6: Temporary Registration (New or Used Vehicle Dealer Notice) o Sample 7: Temporary Registration (Application for Registration of New or Used Vehicle) Sample 1: Permanent Registration (Registration Card) Sample 2: Temporary Registration (New or Used Vehicle Dealer Notice) This document is typically found taped to your windshield by your dealer. 28 MUTUAL OPERATIONS PHYSICAL PROPERTY AMEND DRAFT Unit Fire/Safety Inspection Biennial Inspection WHEREAS, The Occupancy Agreement between each Stockholder and the Corporation provides that the Board may make an inspection of the dwelling unit at any reasonable hour of the day, and WHEREAS, It is the desire of this Board that such situations be discovered and rectified before harm can come to shareholders or to the structure; now, therefore, be it RESOLVED, That the Golden Rain Foundation Facilities Director is authorized and instructed to institute a biennial Fire/Safety Inspection of the dwelling units of this Corporation, using appropriate City and County Health, Fire and Building Codes as a basis for developing a checklist of possible violations, and assigning staff members from Physical Property, Security and/or other appropriate departments to work with members of this Board in conducting such an inspection. At the time of Fire Health Safety and Sanitation inspection, the Building Inspector will verify that the smoke detectors are within the 10-year maximum life and in working condition. If the Mutual has made improvement to the unit at a cost of $1,000 or more the responsibility of maintaining all batteries in all smoke detectors is with the Mutual. Otherwise, improvements made by the shareholder at a cost of $1,000 or more, will make only the non-standard smoke detectors a shareholder responsibility. RESOLVED FURTHER, that violations of City or County Codes shall be reported by the Golden Rain Foundation Facilities Director to the appropriate City or County agency for such remedial action as is provided under the Code. 29 MUTUAL OPERATIONS RESIDENT REGULATIONS ADOPT Co-Occupants, Qualified Permanent Residents and Health Care Providers The community facilities of the Golden Rain Foundation are maintained for the use of members of Leisure World Seal Beach with the following exceptions: 1. Co-Occupants a. Senior citizens, as defined in California Civil Code Section 51.3 (b)(1), who are not members but are approved by the Mutuals to reside with a stockholder, shall be entitled to use all of the community facilities upon payment of a fee equal to the Amenities Fee listed in Policy (Forms 9001 and 9002) b. A Co-Occupant must meet the following requirements: i.) ii.) iii.) iv.) Is 55 years of age or older Has completed the Co-Occupant Application Has written authorization from the Mutual President, or any Mutual Officer so designated by the Mutual President, to reside in the dwelling unit. Has paid the required Amenities Fee to the Golden Rain Foundation 2. Qualified Permanent Residents a. Persons who are not senior citizens as defined in California Civil Code Section 51.3(b)(1), who can present proof that they are eligible to be classified as Qualified Permanent Residents under California Civil Code Section 51.3(b)(2), shall be entitled to use all of the community facilities upon payment of a fee equal to the Amenities Fee listed in Policy (Forms 9003 and 9004) 3. Health Care Providers a. Permitted health care residents, as defined in California Civil Code Section 51.3(B)(7), shall be required to obtain a Caregiver Pass and are not entitled to use any of the community facilities. 30 MUTUAL OPERATIONS PHYSICAL PROPERTY Flooring and Floor Covering Permits Building Permit to include all floor coverings ADOPT Effective immediately, Mutual Five requires a GRF Building Permit for all floor covering, including carpet installed by a shareholder or a contractor. Both interior and patio/porch floor covering will require a GRF Building Permit. If original flooring is disturbed this will require all abatement procedures for asbestos be followed. All testing reports and EPA/AQMD permits will be required as needed. Copies of permits and testing reports shall be filed with GRF flooring permit. Reason for this requirement includes: Assurance that no asbestos contained material is removed or compromised. No Mutual property is damaged Interior/exterior patio/porch flooring must be nonskid, with C.O.F. minimum 0.42 Shareholder understands what is and what is not allowed Shareholder understands that Mutual Five is not responsible for damage to, or failure of, flooring purchased and installed by shareholders or their successor shareholders. 31 MUTUAL OPERATIONS RESIDENT REGULATIONS Sidewalk Traffic Restriction Mutual Five 1. Gasoline-powered vehicles are prohibited from using sidewalks in this Mutual. Exceptions shall be limited to the following: a. Emergency medical vehicles belonging to the Health Care Center. b. Service vehicles designated for sidewalk use belonging to the GRF. c. Service vehicles designated for sidewalk use belonging to contractors or vendors doing business with shareholders or corporations (such as newspaper carriers). 2. The wheelbase on all electric carts (golf carts, two-seaters, gems and similar vehicles) must not exceed 68 inches. The width of the cart must not exceed 48 inches. The inflatable tires must be 8 ½ -inches wide. 3. Electric carts are restricted to traveling the shortest distance from the unit to the street. 4. In order to be parked at a Mutual Five residence, the vehicle must have a DMV-issued handicap placard and a Leisure World decal. 5. If eligible, shareholders must obtain all approvals and install a cement pad or Turfstone four feet wide next to the sidewalk at the unit. Shareholder is responsible for all costs relating to the installation and removal of the paid, including the cost of all permits, inspections, construction and removal. 6. The driver of the cart shall stop for pedestrian traffic at a sufficient distance away from the pedestrian to enable pedestrian traffic to safely navigate the sidewalk at all times. 7. Any vehicle issued a Vehicle Identification Number (VIN) by its manufacturer, or one that fulfills all of the requirements of the State of California that would allow the vehicle to be licensed for highway operation, is prohibited from using sidewalks in Mutual Five and must be parked in an authorized parking space. 8. Electric carts may not be parked on a walkway and shall not be parked in such a way as to interfere with the entry into or the exit from the apartment. 9. Electric cords for charging cannot be placed across any walkway. 10 Visitors may not park on the lawns. Visitors may use the cart parking spaces at the ends of carports. (Apr. 18) Page 1 of 2 31
