The driver will complete the first six columns of Form # 8.01A before departing for the destination. 02. After the destination is reached, the driver must complete the remaining three columns on Form # 8.01A. Regardless of the destination, Form # 8.01A must be filled out completely by the bus driver. 03. Form # 8.01A will stay on the bus at all times. At the end of each pay period the driver will turn in Form # 8.01A to the Transportation/Facilities Coordinator. 04. The completed forms will be maintained in the Transportation/Facilities Coordinator s office.

The bus driver will conduct each Pre-Trip Inspection using Form # 8.02A and begin by approaching the vehicle to check for (use a flashlight if necessary): a. Damage to body, roof, windows, vehicle leaning to one side, anything unusual b. Fluids leak (coolant, fuel, oil, grease, etc.) c. Hanging items (wires, tailpipe, drive shaft) 02. The bus driver will open the hood and check: a. Oil level b. Coolant level by reservoir or radiator (Hot engine-do not remove radiator cap!) c. Power steering fluid d. Brake fluid e. All belt for snugness (up to ¾ play at center of belt), cracks, frays, or misalignment f. Alternator (secure, belt tension) g. Battery (connections, corrosion, cracks, leaks) h. Steering box (secure, leaks, belt tension) i. Pitman arm (secure, cracks) j. Drag link (secure, cracks, bent) k. Spring mounts (secure, cracks, broken) l. Leafs (secure, cracks, bent) m. U bolts (secure, cracks, broken) n. Shocks (secure, leaking) o. Brake hoses (bulges, cracks, leaks) p. Slack adjuster (more than 1 play or 90 degree angle) q. Brake drums (cracks, fluid) r. Brake liners ( at least ¼ remaining) 02. The bus driver then enters the vehicle and: a. Make sure parking brake is set b. Start engine-listening for unusual noises (knocking, etc.) c. Check oil pressure gauge first, then all gauges (top to bottom, left to right) d. Check steering wheel - turn it both ways - too much play? Does it bind or catch? e. Check function of all controls and switches (top to bottom, left to right) f. Activate headlights - low beams, marker or clip lights, sign light and master flasher switch if present, and left turn signal.

5 Procedure # 8.02 Page 2 of The bus driver then exits the vehicle and starting at the front ride side walk counterclockwise around the vehicle and: a. Inspect the tires and wheels - inflation, tread depth, nails or punctures, side wall cracks or cuts, valve stem, cracks between studs, lug nuts, powdered rust or bare metal present, grease exiting front seal dust cap or rear center hub, grease visible inside the rims or tires, broken or misaligned springs, leaking or broken shocks, dents in wheel rim, objects between rear wheels b. Inspect marker or clip lights - corners, front and rear, sides c.. Inspect headlights - low beams d. Inspect running lights and taillights e. Inspect left turn signals (including side mid-vehicle signals) f. Ensure mirror brackets are secure and mirrors are clean g. Inspect hood latch h. Check overhead red flashers if applicable i. Check antenna(s) j. Check sign arm & light k. Check stop sign arm l. Ensure that battery door and luggage compartment doors are secure m. Check to see if reflectors are intact n. 2 nd check for fluid leaks (coolant, fuel, oil, grease, etc.) o. 2 nd check for damage to body, roof, windows p. 2 nd check for hanging or loose wires, drive shaft, muffler, etc. q. Inspect emergency doors, buzzers, safety catch (check emergency reflector triangles r. Check to see that exhaust pipe is secure - push end of pipe with foot (not hand - it may be hot) - does pipe extend proper length beyond muffler? s. Check to see that fuel cap(s) are tight 04. The bus driver then re-enters vehicle and: a. Inspect step well light b. Inspect fire extinguisher (charged, secure in bracket) - sign present c. Inspect seat belt cutter(s) - sign present d. Inspect first aid kit (stocked, secure in bracket) - sign present e. Inspect body fluid spill kit (stocked, secure in bracket) - sign present f. Insure vehicle registration and certificate of insurance is present g. Inspect emergency reflector triangles h. Activate high beams and right turn signal 05. The bus driver then re-exits vehicle proceeding to front and then back along right side of vehicle and: a. Check high beams b. Check right turn signal 06. The bus driver then re-enters the vehicle and: a. Closes door b. Activates 4-way hazard flashers and amber flashers

6 Procedure # 8.02 Page 3 of 3 c. Checks front 4-way hazard flashers, and amber flashers using pedestrian mirrors and by leaning against windshield d. Walk to back of vehicle checking seat backs and all emergency exits and lights e. Open rear door to check amber and 4-way hazard flashers f. Walk to front of vehicle checking seat belts, seat cushions, seat backs, and child restraint devices 07. The bus driver then will sit in the driver s seat and: a. Turn engine off but leave key on b. Release parking brake (if vehicle is parked on level ground; otherwise must chock wheels to prevent rolling) c. Inspect the hydraulic brakes: with key off, depress brake pedal, listen for whirring sound of brake booster motor; with key on but engine off, check brake warning light/buzzer d. Restart vehicle e. Check all mirrors from normal seated position for correct adjustment (especially pedestrian crossover mirrors) f. Fasten and properly adjust driver seat belt g. Check that parking brake is set, place vehicle in reverse, checking parking brake h. Ask another staff member to check brake and back-up lights and beeper at the back of vehicle (if available) i. Check horn (honk twice and pause before backing) j. Perform a 50' service brake check 08. The bus driver will note any problems on Form # 8.02A, and contact the Transportation/Facilities Coordinator for instructions on fixing the problems. Note: Wheelchair lifts and tie-downs must be checked if vehicle is so equipped

When the bus has accumulated 4,000 miles or 7,500 miles, the driver is responsible for contacting the Transportation/Facilities Coordinator. The driver will be instructed to deliver the bus to a specific vendor for service. 02. When the bus has accumulated 20,000 miles the driver is responsible for contacting the Transportation/Facilities Coordinator. The driver will be instructed to deliver the bus to a specific vendor for the transmission/differential service. 02. The driver must get the form # 8.04A Bus Service Checklist signed by the vendor, the driver will also sign form #8.04A. When the form is completed, each driver will turn it in to the Transportation/Facilities Coordinator.

Each driver must use the participating vendors. Form # 8.05A must be filled out each time the driver purchases fuel in his/her bus. A separate form must be used for each bus. 02. At the end of every pay period, the driver will forward the original form to the Transportation/ Facilities Coordinator and provide a copy to the vendor (where required). 03. In case of an emergency, when the driver needs to purchase gas or oil using personal funds, the driver must turn in receipts to the Transportation/Facilities Coordinator in order to be reimbursed. These emergency purchases must also be documented on Form # 8.05A. 04. The driver must use a separate Form # 8.05A for each bus driven during the month.

Fire or danger of fire - If any portion of a school bus is on fire, children should be moved to a point 100 feet or more from the bus and remain there until the bus driver has determined that no danger remains. If a school bus is unable to move and is close to an existing fire or highly combustible materials, the "danger of fire" should be assumed and all children evacuated. b. Unsafe Location- If a bus is stopped in an unsafe location and is unable to proceed, the driver must determine immediately if it is safer for the children to remain on the bus or to evacuate. c. Stalled or inoperable vehicle- If the vehicle stalls or becomes inoperable, the driver must evacuate the occupants if the vehicle s final stopping location is: i. in the path of a train or closely adjacent to any railroad tracks ii. near, or in, a body of water iii. iv. where it could slide over a cliff where visibility is hindered or in danger of any collision (bus should be visible for at least 300 feet) 02. Important Factors in Evacuations a. The safety of the children is of utmost importance and must be given first consideration. The driver should never leave the children alone. b. Prior to evacuation the parking brakes should be set, transmission placed in an appropriate gear, ignition turned off, and the hazardous warning lights activated. c. If the situation allows, the driver should stay in the bus during evacuation to assist in the safe evacuation of occupants. d. Evacuations should be conducted with "deliberate" speed. A time interval of 1 1/2 to 2 seconds per child has proven to be safe. e. To insure a safe exit, children should have their hands free. Personal belongings should remain on the bus. Procedure # 8.06

12 Page # 2 of 2 f. The bus monitor will lead the children to a safe location from each door utilized for evacuation. Children should be taken to a safe point at least 100 feet from the bus, utilizing the rope, and remain there until given further directions. 03. Common Types of Emergency Evacuations a. Evacuations utilizing the front or service door should be conducted using the same techniques as a routine unloading. b. Evacuation steps utilizing the rear exit door are: i. The bus monitor should exit first through the rear door and stand ready to help children as they exit the bus. ii. The driver will instruct the children to leave personal items on the bus and assist the children as they exit the bus. iii. The bus monitor will lift each child off the bus and direct them to a safe location. iv. The bus monitor will call the roll to assure all children are present. 04. Evacuation Bus Drills a. All children should be given an opportunity to participate in evacuation drills. b. All children should be instructed in safety and procedures for emergency evacuation prior to participation in evacuation drills. c. Evacuation drills must be held monthly and held only on Head Start Premises. The driver must document the drill on Form # 8.06A. Form # 8.06A should be signed by the driver and monitor. d. At the end of each month, the driver will forward Form # 8.06A to the Operations Manager. Form # 8.06A

The bus monitor is responsible for seeing that each child is seated and has his/her seat belt properly secured before the driver can move the bus. No child is allowed to stand up on the bus during movement. 02. The use of tobacco, food, or drink on the bus is prohibited. Animals, oversize objects, and glass containers are not permitted on the bus. 03. The bus monitor is responsible for controlling the children s conduct on the bus. 04. All aisles and step wells must be clear at all times. No unsecured objects are allowed in any area of the bus. 05. Emergency equipment required on the bus includes: fire extinguisher, first aid kit, triangle reflector markers, flashlight, blanket, rope, seat belt cutter, camera, body fluid clean-up kit, cellular phone, spare fuses (if equipped with fused electrical system), registration, and certificate of insurance. The driver will sign Form # 8.16A annually to insure the care and upkeep of the above safety equipment. 06. Dropping off the children: a. When the driver is within 50 ft. of a child's stop, the caution lights are turned on. b. When the bus comes to a complete stop and the door is opened, the red lights will be activated. c. The driver will make sure all traffic is stopped in front and rear of the bus before allowing the child to be unfastened from his/her seat belt. d. The bus monitor will proceed to the front door, step off the bus and assist the child in his/her exit. e. The monitor will hold the child's hand and lead the child to the person who has permission to accept the child. f. In the event no one is present to accept the child, refer to Education Procedure # 3.05, "Leaving Head Start Children With Anyone Other Than Parent."

Head Start Bus Drivers will be assigned a cellular telephone and will sign Form # 8.08A taking possession of a particular agency cellular telephone. 02. The main purpose for the phones on Head Start buses is to report an accident/incident, to report a problem, or to call for help. 03. The security of the cellular telephone is the responsibility of the Driver assigned the instrument. Cellular telephones must not be left unattended on the bus because of security reasons as the bus cannot be locked. 04. Cellular telephones are in no way to be construed as the operator s personal equipment and will not be used as such. a. All cellular telephone bills will be scrutinized monthly. b. Personal use of the cellular phone can lead to disciplinary action.

The Bus Drivers must complete a Daily Schedule, Form # 8.10A, at the beginning of the school year. 02. If changes are made in the classroom schedules, or the Bus Driver is moved to another classroom site, a new schedule will be completed. 03. The Transportation/Facilities Coordinator will review the schedule. a. If the Transportation/Facilities Coordinator determines the Driver s schedule can and should include Release Time (see Policy #13.26), the Driver s schedule will be revised as appropriate. b. The Daily Schedule will be reviewed and approved by the Transportation/Facilities Coordinator. 04. Once approved, the revised schedule, indicating Release Time, will be shared with the Bus Driver with instructions regarding any logs or activity reports necessary to document daily activities. 05. The Bus Driver must contact the Transportation/Facilities Coordinator to request extended hours that may result from a field trip, bus maintenance, classroom work, emergencies, or any other circumstances, and the Bus Driver will document reason for extended hours on their time sheet.

Head Start buses will be washed once a week. The Bus Driver must utilize a selfwashing facility. Under no circumstances will the buses be washed at a commercial/ professional car washing facility. 02. In order to be reimbursed for monies expended at self-washing facilities, the Driver must complete the Bus Washing Voucher, Form # 8.11A. 03. The Bus Washing Voucher, Form # 8.11A, must be turned into the Transportation/Facilities Coordinator monthly. There should be no more than four (4) washes on each voucher per month. 04. Head Start Bus Drivers should adhere to a reasonable amount of monies expended at each washing.

Bus Driver s are required to follow basic rules to insure the safety of the children at all times. 02. Bus Drivers are expected to abide by all approved safety laws governing the operation of the bus, as well as those promulgated by the State Department of Safety and the State Department of Education. 03. To insure Driver s understand these basic rules, Form #8.12A must be read and signed on an annual basis.

RULES FOR HEAD START BUS DRIVERS ANNUAL FORM Form # 8.12A DRIVER=S NAME TO BE COMPLETED ANNUALLY BY BUS DRIVERS COMMERCIAL DRIVER=S LICENSE NUMBER You are being issued school bus # for the school year Your assigned center(s) are This bus must be driven with safety of the children in mind at all times. You are expected to abide by all approved safety laws governing the operation of the bus, as well as those promulgated by the State Department of Safety and the State Department of Education. General Duties: 1. Transport Head Start children to and from school as scheduled. Note: Children must not spend more than 90 minutes traveling one way. 2. Transport children that become ill en route on to the school if it occurs on the morning run or home if it occurs during the afternoon run. 3. You must transport parents to meetings, workshops, etc., as instructed by the Transportation/Facilities Coordinator, as needed. This transportation shall be scheduled so it does NOT interfere with the regularly scheduled transportation of the children. 4. Transport children for special services as necessary. 5. Transport meals to and from the classrooms as necessary. 6. Perform work in the classrooms, buildings or playgrounds as necessary. Vehicle Care and Operation: 1. Walk to the rear of the bus after completing your final morning and afternoon runs to make certain that no child is left on the bus, i.e. sleeping, sick, etc. This practice must be adhered to on a daily basis. 2. Sweep the inside of your bus daily - wash the outside once a week. 3. Conduct Pre-Trip and Post-Trip daily.

23 4. Keep scheduled maintenance dates and turn in all required reports on time. Form # 8.12A Page 2 of 4 5. Report all malfunctions as soon as possible. Do not wait for the scheduled service date. 6. Notify the Transportation/Facilities Coordinator before making changes in your route. 7. KEEP AN UP-TO-DATE ROUTE/ALTERNATE SHEET ON YOUR BUS AT ALL TIMES! 8. Stay on the fixed route, except when road conditions dictate a change of plans. Example: ice on a steep hill, an accident blocking the street, or standing/rushing water in the roadway. 9. Maintain a regular schedule of arrival times for schools in A.M. and P.M. 10. Refuel their vehicles while no children are in the vehicle. 11. You are responsible for the safe overnight parking of your bus. (See Over-Night Parking Procedure # 8.17). Personal Practices: 1. Practice personal cleanliness. 2. Do not eat or drink anytime while driving the bus. Water can be carried on the bus by the driver, but it must be in an unbreakable container, out of sight of children and consumed when children are not present. 3. Do not use tobacco products while on duty or allow parents to use them on the vehicle. 4. Do not use alcoholic beverages or narcotics while on duty or to excess while off duty. 5. Do not transport persons other than regularly enrolled children unless authorized to do so by the Transportation/Facilities Coordinator. 6. Sign in and out of your designated work station. 7. Do not make personal stops at stores or other businesses during the bus runs. 8. Do not install radios, stereos, etc. without the approval of the Transportation/Facilities Coordinator. 9. You must have and maintain a valid Commercial Driver s License (CDL). The license MUST be in your possession when you are on duty. 10. You may not bring other children or adults with you to work.

24 Form # 8.12A Page 3 of 4 Pick-Up and Drop-Off: 1. Children 90 pounds or less must not be transported without being properly secured in a child restraint device. 2. Come to a full stop before any child is permitted to get on or off the bus. 3. Pull up to the curb or side of the road to load and unload children. The parent, the driver, and the staff all share responsibility for helping the child to board and exit the vehicle safely. 4. If you need to get up from your seat while loading or unloading children, you MUST remove the keys from the ignition to avoid the possibility of an accident. 5. The school bus sign (where appropriate) must be in the upright position whenever children are being transported. The flashing lights must be used whenever children are loading or unloading. 6. Anytime the vehicle is not being used for child transportation, the sign must be in the lowered position and the red lights are not to be used. 7. Do not permit children, accompanied by the Monitor, to leave the bus until approaching traffic from both directions has come to a complete stop. Keep the service door open and the STOP SIGN activated until all children who must cross in front of the bus are completely off the road. Children must not be allowed to cross without the Monitor or a Parent with them. 8. Children not required to be placed in a child restraint seat must always wear a seat belt. The seat belt must be in place before the vehicle is in motion. Each seat belt must be used by only one child. Belts must be checked before the bus departs. 9. Make sure that all departing children are at a safe distance from the bus before moving. Vehicle/Passenger Safety: 1. Drivers must always buckle their own seat belts for safety and because it is the law. 2. Come to a full stop at all railroad crossings and arterial highways. 3. Follow all traffic rules and regulations, adhering to speed limits at all times. 4. Report every accident, regardless of its severity, to the Transportation Dept. immediately. 5. Do not allow children to sit or stand in the Driver s area at any time or to handle the controls of the bus.
