4 Introduction ShurShape is a unique and revolutionary molded seating system. It allows a seating professional to capture and manipulate a shape and then deliver that shape to the user with no waiting. The ShurShape System can do this because the seating professional actually creates the client s finished cushion. To do this, the molding bag is prepared by adding a two-part adhesive to the foam granules inside the molding bag, and distributing it throughout. By using the bag first to take an impression, then manipulating the contents of the bag, the desired shape is created. ShurShape allows the fitting to take place right in the client s mobility base. Seating and mobility are brought together to design a system that addresses all the client s needs at the same point in time. Each system uses a Carrying Structure to provide structural support to the cushions and to secure them to the mobility base, much like a linear style seating system. Seat depth, back height, and other angle adjustments are made with ease and accuracy CUSTOM MOLDING SOLUTION Ordering Information ShurShape can be ordered to fit chairs 12" wide through 21" wide. Seat depths range from 9" to 24". Back heights range from 11 to 28. Each kit has the flexibility to accommodate a range of 3" to 4" on seat depth and 3" to 4" on back height. The Carrying Structure is designed 3 narrower than the frame width of the chair. Each platform comes with a double track mounting system for ease of installation and adjustment. The depth of the seat platform on the Carrying Structure may be 2" longer than the requested seat depth to accommodate the thickness of the back. A Polartec cover is included with each kit. Mounting hardware is available in two styles Knob Release and Drop Hook. Several options are available including a bi-angular back modification, an abductor, headrest, and the unique Flexi-Feet foot positioning product. The mold takes between 30 and 40 minutes to set up. At any point, during the molding process the vacuum may be used to obtain the final shape. ShurShape recommends that the user sit in the system for one hour for best results. Molding should be done in temperatures of approximately 70 to 75 degrees Fahrenheit. ShurShape contains no measurable latex and meets the CAL117 flamability standard. ShurShape requires completion of our training session to participate in the remold policy and to ensure quality results with the product. 2 ShurShape Product Catalog

5 Remold Policy ShurShape must be contacted within 90 days of the purchase of the molding product should a remold be required. The remold kit must be of the same size and configuration as the original kit. Only one remold per order is allowed. ShurShape requires that the unused mold be returned and a credit will then be issued for the remold kit. ShurShape reserves the right to modify or change this policy at any time. This remold policy is available only to those who have received the Formal ShurShape Training Session. Training Dealers wishing to purchase and re-sell ShurShape products must receive the ShurShape Formal Training Session. This is a 6.5 NRRTS Credit Course instructing in the proper use of the product. Growth Policy ShurShape will provide a Seat Molding Bag Kit and/or a Back Molding Bag Kit for one-half of the retail price for any patient requiring a growth remold. A ShurShape system shall be allowed only one growth kit. Original orders for a Seat only or a Back only will be allowed one growth Molding Bag Kit. Original orders for a Seat AND Back will be allowed one Seat Molding Bag Kit and one Back Molding Bag Kit. The Molding Bag Kit must be used with the original Carry Structure. Should the Carrying Structure also require replacement, then the ShurShape Growth Policy shall not apply and both Carrying Structure and Molding Bag Kit will be provided at full retail price. The kit includes a Seat Molding Bag, Adhesive, Carrying Structure, Finish Cover, and Mounting Hardware. The kit is available in many sizes, as selected on the order form. This part number is Code Verified for HCPCS code E2609. Please note that a Vacuum Pump must be ordered seperately for use in creating the mold. The Vacuum Pump is not included or sold as part of the seating system. The Vacuum Pump can be reused for other fittings. ShurShape Back Code Verified E2617 This part number includes all of the items required to make a complete back mold. The kit includes a Back Molding Bag, Adhesive, Carrying Structure, Finish Cover, and Mounting Hardware. The kit is available in many sizes, as selected on the order form. The lateral guides are fully adjustable to both secure the cushion and to help achieve the proper shape. The Carrying Structure is a plastic material for durability and moisture resistance. When ordering, it is not necessary to select the Carrying Structure part number. ShurShape will select the correct Carrying Structure based on the dimensions provided for the Molding Kit. 11 Options Structural Modifications This section contains a number of options that can be added to the ShurShape system. All of the options are designed to work with any system. If you have questions please contact a Rehab Product Specialist. The SPLIT GUIDE option is an excellent way to pinpoint control both on the seat as well as the back.
