Updated July 31, 2018 Case Number 18 9342 07/29/2018 at 4:30 am Rte. 41 and Deerpath Driving Under the Influence of Alcohol, Driving While License Suspended, Illegal Transportation of Alcohol, Aggravated Driving Under the Influence of Alcohol Subject(s) Flores Damian, Hugo Victor, male, age 31, 913 Commonwealth Ave. Waukegan, IL 60085 $40000 08/2/2018 at 1:30 pm T 110 Waukegan Lake Forest Police responded to the area of Rte. 41 and Old Elm Road after receiving a 911 call concerning a red SUV that was swerving all over the road. Officers located the vehicle driving north on Rte. 41 and after observing several driving issues, conducted a traffic stop on the vehicle. When officers approached, they could smell a strong odor of alcohol coming from the vehicle and observed, in plain view in the center console, two clear plastic cups containing an unknown red liquid. Flores Damian was also demonstrating signs of alcohol impairment and it was determined Flores Damian was driving on a Suspended Driver s license. Officers asked Flores Damian to exit the vehicle and perform field sobriety test, which he did not pass. Flores Damian was subsequently arrested for Driving Under the Influence of Alcohol, Driving While License Suspended, and Illegal Transportation of Alcohol. Flores Damian was transported to the Public Safety Building and processed. The Lake County States Attorney approved an Aggravated Driving Under the Influence of Alcohol charge based on Flores Damian s past driving record and convictions. Flores Damian was transported to Waukegan for a bond hearing in C 120. He was held on $40,000 bond. Case Number 18 9303, 9304, 9307, 9311 07/28/2018 at 7:00 am Subject(s) Timber Drive, Onwentsia Road, Basswood Road, Forest Hill Road unknown Theft of Motor Vehicle (2) and Burglary to Motor Vehicle (2) Lake Forest Police responded to two separate addresses on July 28 after a resident on Timber Drive reported the theft of a 2015 BMW 328 xi that had been left unlocked in the driveway overnight with the keys in the vehicle, and a resident on Onwentsia Road reported the theft of a 2011 Land Rover that had been parked over night with the doors unlocked and the key fob in the center console. Two other residents in the same areas reported unknown persons had entered unlocked vehicles in the driveways and stole miscellaneous items from the vehicles. The 2011 Land Rover was recovered in Calumet City after being in a vehicle pursuit with Chicago Police. Calumet City PD located the vehicle abandoned with no occupants and a gun was found in the car. The 2015 BMW was recovered in Dalton, IL. No arrests have been made and no suspects have been identified. In one incident, the offenders used a garage door opener to access a garage an effort to steal an additional car from one of the locations. LAKE FOREST POLICE again want to remind residents to remove the keys from the cars and lock their cars when not in use. These are crimes of opportunity and are very preventable with just a little effort. N/A N/A

Case Number 18 9300 07/28/2018 at 12:51 am Rte 41 and Gage Lane Driving While License Suspended (Class 4 Felony), Driving Under the Influence of Alcohol (Class 4 felony), Endangering the Life of a Child, Operating an Uninsured Motor Vehicle Subject(s) Hilliard, Steven D., male, age 29, 9876 Michigan Blvd, Beach Park IL 60099 $75000 08/02/2018 at 1:30 pm T 110 Waukegan Police on patrol in the area of Old Elm Road and Rte. 41 observed a white Honda Civic that was being operated in a hazardous manner and conducted a traffic stop on the vehicle. When officers approached the vehicle they located a driver, front passenger and three children in the back seat not in child restraint seats as required. As officers identified Hilliard as the driver, they determined his driver s license was suspended for DUI and he did not have insurance on the vehicle. Hilliard was also demonstrating signs of alcohol impairment at the time of the traffic stop. Hilliard admitted to drinking and was removed from the car for field sobriety testing. Hilliard was subsequently arrested for Driving While License Suspended and Driving Under the Influence of Alcohol. He was transported to the Public Safety Building where has was processed and held. Officers consulted with the Lake County States Attorney and based on previous DUI convictions, Hilliard s charges for Driving While License Suspended and Driving Under the Influence of Alcohol were upgraded to felonies. He was also cited for Endangering the Life of a Child and Operating an Uninsured Motor Vehicle. Hilliard was held overnight pending transport to bond court in Waukegan. Case Number 18 9266 07/27/2018 at 2:00 am Rte. 41 and Rte. 60 Leaving the Scene of an Accident, No Vehicle Insurance, Driving While License Revoked (Class 4) and Driving Under the Influence of Alcohol (Class 2) Subject(s) Koopman, Brian H, male, age 56, 129 S. Central Ave. Highwood, IL 60040 $75000 08/02/2018 at 9:00 am T 110 Waukegan Police responded to the area of Rte. 41 and Rte. 60 after receiving 911 calls concerning a white van that had gone off the roadway into some trees. Police received multiple calls concerning the crash and on arrival could not locate the driver of the vehicle. After calling for a K9 and searching the area, Koopman was located, passed out in the yard on a house on Onwentsia Road. The K9 followed a track from the crash to the same address helping to confirm Koopman as the driver. The Lake Forest Fire Department transported Koopman to Condell Hospital. Upon his release from the hospital, Koopman was charged with Leaving the Scene of an Accident, No Vehicle Insurance, Driving While License Revoked (Class 4), and Driving Under the Influence of Alcohol (Class 2). Koopman was processed and transported to Lake County bond court for a bail hearing.

Case Number 18 9223 07/26/2018 at1:50 am Rte. 41 and Deerpath Driving While License Revoked and Speeding 74/55 Subject(s) Brown, Quinton I. male, age 29, 1715 Longview Rd. Apt 10. Waukegan, IL 60087 09/04/2018 at 1:30 pm Police on routine patrol conducted a traffic stop on a 2007 Hyundai after observing the vehicle speeding 74 mph on Rte. 41. When officers approached the vehicle and spoke to the driver, Brown, they determined his driver s license was revoked. Brown was arrested, transported to the Public Safety Building, charged with Driving While License Revoked and Speeding 74/55, processed, and released on bond with a September court date. Updated July 25, 2018 Case Number 18 7663 07/17/2018 at 10:00am 400 Hastings Violation of a Public Park Zone Child Sex Offender (Class A Misdemeanor) Subject(s) Costello, David R., male, age 47, 525 W. Everett Road, Lake Forest IL 60045 08/07/2018 at 8:45 am C 150 Waukegan Lake Forest Police investigated an incident involving Costello after receiving information that Costello was observed on several occasions attending softball games in the public park area behind Deerpath Middle School and adjacent to the Lake Forest Recreation Center. Police determined this action to be in direct violation of restrictions placed on Costello as a registered child sex offender. Based on the subsequent investigation by Lake Forest Police detectives, Costello was arrested and charged with Violation of a Public Park Zone by a Child Sex Offender. Costello was processed, released on bond, and given an August court date. Case Number 18 9083 07/23/2018 at 2:15 pm 840 S. Waukegan Road Driving While License Suspended and No Vehicle Insurance $2000 09/04/2018 at 1:30 pm Subject(s) Miranda, Leslie, female, age 27, 2027 W. Glen Flora, Waukegan, IL 60085 Police on patrol conducted a traffic stop on a 2007 Volkswagen and after speaking to the driver, Miranda, determined she was driving on a suspended license. Miranda also did not have insurance for her vehicle. Miranda was arrested, charged with Driving While License Suspended and No Vehicle Insurance. She was transported to the Public Safety Building where she was processed, released on bond, and given a September court date.

Case Number 18 9049 07/22/2018 at 5:19 pm Waukegan Road and Gloucester Driving While License Revoked Subject(s) Castrejon, Jose, male, age 36, 3020 15 th Street, North Chicago, IL 60064 08/21/2018 at 1:30 pm Officers on routine patrol observed Castrejon operating a motor vehicle on Waukegan Road while using a cell phone. Officers conducted a traffic stop and subsequently learned Castrejon was driving on a revoked driver s license. Castrejon was arrested and charged with Driving While License Revoked. He was processed and released on bond with an August court date. Case Number 18 9022 07/22/2018 at 2:11 am Green Bay Road and Vine Ave. Subject(s) Torres, Leonardo, male, age 20, 515 N. Ash St. Waukegan, IL 60085 Minor Consumption of Alcohol, Zero Tolerance While Driving, Possession of Cannabis 08/21/2018 at 3:00 pm 3:00 pm Officers on patrol conducted a traffic stop on a blue Acura after observing the vehicle commit a traffic violation. Officers approached and spoke to the driver later identified as Torres. Officers observed open alcohol in the vehicle and smelled the odor of cannabis coming from the vehicle. Officers were then able to smell the odor of alcohol on Torres s breath. Subsequent sobriety testing determined Torres had consumed alcohol while under age and in violation of the Zero Tolerance statute. Torres was arrested, charged with Minor Consumption of Alcohol Zero Tolerance, Minor Possession of Alcohol, and Possession of Cannabis. Torres was processed, released on bond, and given an August court date. Case Number 18 9021 07/22/2018 at 1:29 am Rte. 41 and Gage lane Driving Under the Influence of Alcohol, $3,000 08/24/2018 at 9:00 am C 405 Waukegan Subject(s) Bautista Hernandez, Amaro, male, age 31, 4202 Hickory Hills Dr., Waukegan, IL 60087 Police responded to the area of Rte. 41 and Old Elm after receiving 911 calls about a dark color minivan that struck the center median and kept driving north on Rte. 41, swerving in both lanes of traffic. Police were able to locate the vehicle and conducted a traffic stop on the offender. Officers approached the vehicle and spoke to the driver, Bautista Hernandez, who displayed obvious signs of impairment. Officers conducted standard field sobriety testing and Bautista Hernandez subsequently was arrested and charged with Driving Under the Influence of Alcohol. Bautista Hernandez was transported to the Public Safety Building where he was processed and submitted to chemical breath testing which resulted in a reading of 0.130 BAC. Bautista Hernandez was released on bond and given an August court date.

Case Number 18 9004 07/21/2018 at 5:29 pm Everett Road and Lakewood Driving While License Suspended, Possession of Cannabis Subject(s) Piktel, James D., male, age 19, 39333 N. McAree Rd. Beach Park, IL 60099 09/04/2018 at 1:30 pm Police conducted a traffic stop on a white 1996 Mercury after observing the vehicle commit a traffic violation. Officers spoke to the driver, Piktel, and determined his driver s license was suspended. Officers also located drug paraphernalia and cannabis in the vehicle. Piktel was arrested and charged with Driving While License Suspended and Possession of Cannabis. He was processed and released on bond with a September court date. Updated July 20, 2018 Case Number 18 8841 07/18/2018 at 1:30 pm Rte. 41 and Rte. 60 Driving Under the Influence, Driving While License Suspended, Speeding 71/55. Subject(s) Sosa Orellana, Jose, A., male, age 18, 443 N. Sheridan Road, Waukegan, IL 60085 $3000 08/24/2018 at 9:00 am C 405 Waukegan As a result of a traffic stop for speeding 71 mph in a posted 55 mph zone and driving on the shoulder of the road, officers spoke with the driver of the 2007 Ford Edge, identified as Sosa Orellana. Officers determined Sosa Orellana was driving on a suspended license and officers also observed Sosa Orellana displaying obvious signs of alcohol impairment. Subsequent field sobriety testing and field interview by officers resulted in Sosa Orellana being arrested for Driving Under the Influence, Driving While License Suspended, and Speeding 71/55. Sosa Orellana was transported to the Public Safety Building, processed, and a breath test was conducted which resulted in a reading of 0.142 BAC. Sosa Orellana was released on bond and given an August court date. Case Number 18 8725 07/15/2018 at 1:23 AM Rte. 43 and Deerpath Driving While License Suspended, Use of Electronic Device While Driving 08/21/2018 at 3:00 pm Subject(s) Abequibel, Christian P., male, age 19, 300 Pembridge, Schaumberg, IL 60193 Police on routine patrol conducted a traffic stop on a white 1996 Chevrolet after observing the driver using an electronic device while driving. Police spoke to the driver, identified as Abequibel, and determined his driver s license was currently suspended at the time of the stop. Abequibel was arrested and transported to the Public Safety Building where he was processed and released on bond with an August court date.

Updated July 17, 2018 Case Number 18 8695 07/14/2018 at 2:03 pm Rte 41 and Old Elm Road Driving While License Revoked Subject(s) Munoz Salazar, Marion, male, age 45, 3448 Greenwood Ave., Gurnee, IL 60031 08/23/2018 at 3:00 pm Police conducting truck enforcement activities conducted a traffic stop on a red pickup towing a trailer that was dragging the ground. Officers spoke to the driver, Munoz Salazar, about the unsafe equipment violation and subsequently learned Munoz Salazar s driver s license was revoked. Munoz Salazar was arrested, charged with Driving While License Revoked, processed, and released. He was given an August court date. Case Number 18 8646 07/13/2018 at 12:43 pm 695 Western Ave Warrant Arrest n/a n/a Subject(s) Nicholas, Jonathan G., male, age 23, 5625 W. Fullerton Ave., Chicago, IL 60639 Lake Forest Police received a 911 call concerning a male subject at the east train depot who appeared disorientated. When officers arrived, they spoke to the subject, identified as Nicholas, to determine his welfare. As officers were speaking to Nicholas, they were informed he had an outstanding warrant from Cook County and a caution alert for armed and dangerous. Nicholas was taken into custody and subsequently transported to the Lake County jail where he was held pending transport back to Cook County. Case Number 18 8601 07/12/2018 at 6:13 pm Conway Farms Rd. and Old Bridge Driving While License Suspended 08/07/2018 at 1:30 pm Subject(s) Laarveld, Brittney F., female, age 32, 645 S Buckingham Ct., Lake Forest, IL 60045 Police conducted a traffic stop on a 2014 grey Honda after observing the vehicle disobey a traffic control device. When officers spoke to the driver, Laarveld, they determined her driver s license was suspended. Laarveld was subsequently arrested, charged with Driving While License Suspended and Disobeying a Traffic Control Device. She was processed, released on bond, and given an August court date.

Jackson Street, Danville, IL 61832 Burke, Rashaid A., male, age 29, 613 Commercial St., Danville, IL 61832 09/04/2018 at 1:30 pm Police conducted a traffic stop on a white Buick sedan and when they spoke to the driver, Tyler, officers determined Tyler was driving on a suspended driver s license. Officers on scene also smelled the odor of cannabis and, after asking Tyler and the passenger Burke to exit the car, officers located a small bag of cannabis in Burke s possession. Police also learned Burke had a valid warrant for his arrest outstanding from Peoria, IL. Tyler was charged with Driving While License Suspended, released on bond, and given an August court date. Burke was processed and released on bond and given an August court date in Peoria, Illinois for his warrant. Case Number 18 8449 07/09/2018 at 1:47 pm McKinley Road and Mills Ct. Driving While License Suspended, Operate Uninsured Motor Vehicle and Use of Electronic Device While Driving Subject(s) Soto, Ana V, female, age 38, 252 Hebron Avenue, Zion, IL 60099 $2000 08/07/2018 at 3:00 pm Police conducted a traffic stop on a 2010 Chrysler van after observing the driver talking on her cell phone while operating a motor vehicle. When officers spoke to the driver, Soto, they determined her driver s license was suspended and she did not have insurance on the vehicle. Soto was arrested, charged with Driving While License Suspended, Operate Uninsured Motor Vehicle, and Use of Electronic Device While Driving. She was processed, released on bond and given an Case Number 18 8368 07/07/2018 at 8:31 pm Deerpath and Western Ave. Driving While License Suspended 08/21/2018 at 1:30 pm Subject(s) Garcia, Rebecca E., female, age 22, 230 Valley Rd Lindenhurst, IL 60046 Police on routine patrol conducted a traffic stop on a vehicle that displayed an expired registration. When police spoke with the driver, Garcia, they determined that Garcia was driving with a suspended license. Garcia was arrested, transported to the Public Safety Building, and cited for Driving While License Suspended and Expired Registration. He was released on bond and given an August court date.

Updated July 5, 2018 Case Number 18 8044 07/01/2018 at 9:53 pm 580 Bank Lane Driving Under the Influence of Alcohol $3000 08/24/2018 at 9:00 am C 405 Waukegan Subject(s) Ori, William M., male, age 32, 668 Homewood Ave., Highland Park, IL 60035 Police on patrol in the area of Bank Lane conducted a traffic stop on a silver Saturn after observing the vehicle driving the wrong way on a oneway street. Police stopped the vehicle and spoke to the driver, identified as Ori. While officers were speaking to Ori, they detected the strong odor of alcohol coming from his breath and observed he was demonstrating signs of impairment. When questioned, Ori stated he had consumed 2 beers at a local bar but he had also been drinking beer all day. Ori was requested to submit to field sobriety testing and, as a result of the test, was subsequently arrested for DUI. Ori was transported to the Public Safety Building, processed, and released on bond with an August court date.
