CITY OF SHARON Notice to Bidders RENTAL OF CHIPSEAL EQUIPMENT WITH OPERATORS AND LABORERS Sealed proposals for Rental of Chipseal Equipment with Operators and Laborers will be received by the City of Sharon, Mercer County, Pennsylvania, at the office of the City Manager, 155 W. Connelly Blvd, Sharon PA 16146 or by mail to the City Manager, 155 W. Connelly Blvd, Sharon, PA 16146. Bids will be received until 4:30 PM, Thursday, June 19, 2014. They will be opened and read aloud at 6:00PM at 155 W. Connelly Blvd, Sharon PA in the council chambers. Specifications and Forms of Proposal may be obtained from the undersigned at the office of the City Manager. Proposals must be accompanied by a Bid Bond or Certified Check in the amount of ten percent (10%) of the bid price. Proposals must be enclosed in a sealed envelope, addressed to the City Manager and should bear the name and address of the bidder together with the words 2014 Equipment Rental clearly marked on the exterior of the envelope. DETAILED SPECIFICATIONS Bidders must be able to provide ALL equipment as requested. All equipment is to be State approved and carry current Pennsylvania State inspection and Contractor shall submit evidence of ownership of said equipment as a condition of award of the contract upon request. Equipment and labor to be utilized is described as follows: A. Labors (2) B. Self-Propelled Chip Spreader, computerized rate control, variable width capable 6 to 20 C. Three (3) Tri-Axle Dump Trucks compatible with chipper D. Two (2) Single-Axle Trucks E. One (1) Self-Propelled Steel Wheel Tandem Roller 10-14 ton F. One (1) Self-Propelled Nine-Wheel Rubber-Tire Roller G. One (1) Power Broom Tractor mounted H. Rubber-Tire Loader (3 cubic yard capacity) I. Foreman with one ton support truck DETAILED SPECIFICATIONS FOR EQUIPMENT: 1. Bidder must be able to provide ALL pieces of equipment as requested. 2. All licensed equipment is to be State approved and carry current Pennsylvania State Inspection and Contractor shall submit evidence of such approval upon request. 3. Equipment to be used approximately 40 hours at the discretion of the Municipality. 4. Successful bidder must furnish Municipality with Certificate of Insurance for Workmen s Compensation for operators. All proposals shall include a detailed description of each piece of equipment being bid, including year, make, model, size and description in the space provided for this information in the Form of Proposal. Bid security shall be ten percent (10%).

I/We hereby certify that I/We have read, understand and agree to all the conditions outlined in the Form of Proposal, Instructions to bidders, Detailed Specifications and all other parts of the Contract Documents. I am/we are the only person(s) interested in the Proposal as Principal(s). This Proposal is made without collusion with any person, firm or corporation. I/We have investigated the availability of the equipment and labor specified and agreed to furnish and deliver the specified equipment and labor at the following prices: A. The Bidder must visit the site and meet with the foreman about the job prior to bidding. Any deviation from this format will result in a rejection of your bid.
Accompanying this Proposal is a certified check/bid bond (cross out one) in the amount of dollars ($ ) payable to the City of Sharon, Mercer County, Pennsylvania, which it is agreed shall be retained as liquidated damages by the Owner if the undersigned fails to execute the Contract in conformity with the Form of Agreement incorporated in these Contract Documents and furnish any specified bonds within ten (10) days after notification of award of the Contract to the undersigned.
