3M Personal Safety Division 3M Center, Building 235-02-W-70 St. Paul, MN 55144-1000 651 733 1110 DISTRIBUTOR PRODUCT RECALL NOTICE 3M Model SWSW Series Mobile SkyWalk Horizontal Lifeline Systems and Components Dear 3M Distributor: September 24, 2014 As you are aware, on September 12, 2014 3M issued a Stop Use And Product Recall Notice for Model Number SWSW Series Mobile SkyWalk horizontal lifeline system, advising users (i) that they must immediately identify, stop use, and quarantine all Mobile SkyWalk horizontal lifeline systems, and (ii) that a product recall would be forthcoming. This system was previously sold under the name Aearo Technologies Mobile SkyWalk system and or SafeWaze Mobile SkyWalk system. 3M acquired Aearo Technologies / SafeWaze in April 2008. Not all components are reflected in this diagram. 3M has chosen to voluntarily recall the products impacted by this issue and again requests that you immediately identify, stop use, and quarantine all Mobile SkyWalk horizontal lifeline systems. All Mobile SkyWalk horizontal lifeline systems sold from January 01, 2003 to September 12, 2014 are affected by this recall notice. We request that the following steps be implemented by your company: 1. Immediately identify and quarantine all 3M SWSW Mobile SkyWalk systems and components within your distributor inventory. Two primary components used in the Mobile SkyWalk lifeline system are the SWSW-08A Aluminum Stanchion Post and the SWSW-18 Power Brake Energy Absorber and are depicted in the image above. The SWSW-18 and if also purchased the SWSW-08A are required to be returned to initiate the product credit process. Component parts will be fully credited at current distributor pricing with proof of inventory and disposal. A full list of parts affected is attached to this document, along with several product images. 2.
