RECOMMENDATION: Adopt Resolution. BACKGROUND: The Fire Department is currently in the process of acquiring three new ambulances and placing them into service. As part of the ambulance replacement process we have identified a need for improvement in our patient gurney and transportation system. The primary concern is patient management; we must have high quality patient gurneys for safe reliable patient care and transportation. Staff is also very concerned with prevention of injuries to firefighter/paramedics as a result of handling manual lift patient gurneys that are currently used to transport medical patients. And, our current patient gurneys used in the old ambulances are in need of replacement.. Patient gurneys are used at the scene of medical emergencies to move medical patients to the ambulance where the patient gurney is loaded by firefighter/paramedics into the ambulance for transport to a medical facility. Upon arrival at the medical facility firefighter/paramedics unload the patient gurney and transport the patient on the gurney into the medical facility. The Fire Department is very conscious of the health, safety and welfare of our firefighter/paramedics and the medical patients we serve. We are continuously examining our processes, procedures, equipment and emerging technologies to identify areas where improvements may be made. As part of this improvement process, we have identified that our firefighter/paramedics are incurring injuries associated with the lifting, loading, unloading and lowering of our existing patient gurneys. In addition to ongoing staff training for back care injury prevention, we believe that one solution to this concern is to upgrade our three new ambulances and existing patient gurneys to a new combined patient gurney and patient gurney power lift assembly. SAN RAFAEL CITY COUNCIL AGENDA REPORT / Page: 2 Fire Department staff, including firefighter/paramedics, has examined patient gurney and patient gurney power lift assemblies of several manufacturers. Based on the performance, quality and cost, staff is recommending that we select Stryker Sales Corporation as our provider. The Stryker combined patient gurney and patient gurney power lift assembly is a complete integrated system, which automatically lifts, loads, unloads and lowers the patient gurney into and out of the ambulance. This Stryker system eliminates the need for firefighter/paramedics to manually lift, load, unload and lower the patient gurney, thereby minimizing the risk of injury associated with patient transportation. It's always regrettable when one of our firefighter/paramedics is injured on the job. It becomes a hardship for the Fire Department when a firefighter/paramedic incurs a loss of time injury, as other members of the Fire Department must work overtime to backfill the vacant position. ANAL YSYS: As the Fire Department is currently in the process of bringing three new ambulances online, this is the best time to upgrade the existing patient gurneys to the new Stryker combined patient gurneys and patient gurney power lift assemblies. The upgrade to new Stryker combined patient gurneys and patient gurney power lift assemblies is time sensitive as the three ambulances are currently under construction and the patient gurney power lift assembly infrastructure may be included now at a substantial savings in the cost of installation. As a result of the timing, we're asking for an expedited purchasing process to ensure that we may place the new ambulances into service as soon as possible. Any delay in purchasing the new combined patient gurneys and patient gurney power lift assemblies simply extends the potential for an injury to one of our firefighter/paramedics. The attached Resolution authorizes the waiver of the competitive bidding process and authorizes the City Manager to execute a purchase order with a sole source provider for the purchase of three new Stryker combined patient gurneys and patient gurney power lift assemblies. FISCAL IMPACT: There is a significant cost to the City associated with firefighter/paramedics loss of time injuries. Presently the Fire Department has firefighter/paramedics that are off duty with loss of time injuries associated with patient gurney lifting, loading, unloading and lowering. Nationally, this is an issue for the fire service that has left some firefighter/paramedics permanently disabled. The cost associated with just one severe back injury could be substantially more for the City than the cost of the three new Stryker combined patient gurneys and patient gurney power lift assemblies. The cost for the purchase of three new Stryker combined patient gurneys and patient gurney power lift assemblies is 143,396.89, which is available in the Vehicle Replacement Fund. OPTIONS: The City Council may choose not to approve the Resolution but staff does not recommend this option. The injuries to firefighter/paramedics associated with the existing patient gurneys have impacted Fire Department's medical response operations. Staff feels strongly that the financial transaction described herein will help solve the Fire Department's concern for firefighter/paramedic injuries associated with patient gurneys. RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SAN RAFAEL APPROVING STRYKER SALES CORPORATION AS A SOLE SOURCE PROVIDER FOR THE PURCHASE OF THREE NEW PATIENT GURNEYS AND PATIENT GURNEY POWER LIFT ASSEMBLIES FOR NEW AMBULANCES. WHEREAS, the Fire Depaliment is acquiring three new ambulances, which must have patient gurneys for patient transpoliation; and WHEREAS, the Fire Department desires to safely transport patients at the scene of a medical emergency on a patient gurney and to safety load and unload patient gurneys in and out of ambulances and to safely transpoli patient gurneys in the ambulances; and WHEREAS, the Fire Depaliment desires a combined patient gurney and patient gurney lift system that minimizes the risk of injury to patients and firefighter/paramedics; and WHEREAS, the Fire Department has identified a single source (Stryker Sales Corporation) for the purchase of new combined patient gurneys and patient gurney power lift assemblies that will minimize risk, enhance safety and save valuable assets; and WHEREAS, the patient gurneys and power lift gurney assemblies manufactured by Stryker Sales Corporation are complete assemblies with new cots and all required palis and hardware; and WHEREAS, there are sufficient funds in the Vehicle Replacement Fund for the transactions described above; and

WHEREAS, the Fire Depatiment has reviewed the specifications, pricing and time lines from Stryker Sales Corporation and have determined that they offer financial value and immediate operational benefits. NOW, THEREFORE BE IT RESOLVED, the City Council of the City of San Rafael does hereby authorize for the waiver of the competitive bidding process for the purchase of three Stryker Sales Corporation combined patient gurney and patient gumey power lift assemblies; and NOW, THEREFORE BE IT FURTHER RESOLVED, the City Council of the City of San Rafael does hereby authorize the City Manager to execute a Purchase Order in the amount of One hundred forty three thousand, three hundred ninety six dollars and eighty nine cents (143,396.89) for the purchase of three Stryker Sales Corporation combined patient gurney and patient gurney power lift assemblies.
