SAVE 10 % Captain s Choice Commissioning Service (You only need to complete the first and last page of this form) When selecting this option we will perform the services that specifically apply to your boat. Your boat will be unwrapped, sacrificial anodes replaced, the onboard systems commissioned, the motors and generator (if equipped) will be serviced, the exterior will be cleaned, waxed, compounded if needed and bright work polished. In addition, we will inspect test and repair any minor issues found with the following systems. Bilge pumps, navigation lights, motor compartment exhaust ventilation system, intake sea cocks and hoses, sound devices, shaft packing glands and steering system. With Captain s Choice any repairs will be billed at 10% off the posted labor rate of $135.00 per hour. Required parts will be billed at 10% off list. For a detailed description and cost of each commissioning service, refer to the services offered on the following pages. Our Service Department will contact you and provide a cost to repair any significant issues found with the systems listed above or if we discover problems with any of the other remaining onboard systems or components. 2

NOTES 1. All accounts must be paid in full prior to launch, including your Spring Launch work orders. 2. Boats were put on land according to the date you requested to be re-launched on your winter storage contract. Please make sure you have completed any work that needs to be done prior to this date. If you change this date, there may be a charge for relocating your boat. We will do our best to accommodate you. Due to uncontrollable forces (i.e. weather) Clarks Landing Marina cannot guarantee the exact date your boat will be launched. 3. It is the responsibility of the boat owner to inform Clarks Landing Marina if the boat was previously painted with Micron. Micron paint must be used again since it is the only paint that will adhere to previously painted Micron bottoms. 4. Boat bottoms will be painted the same color as the existing color with Interlux bottom paint. If you would like a different color please indicate in the space provided under bottom paint. A half hour free scraping charge is included. If additional time is required, you will be notified. 5. Sandblasting is not permitted in Clarks Landing Marina. 6. Clarks Landing has been recognized by the NJ Department of Environmental Protection as a New Jersey Clean Marina. This requires the following procedures be adhered to: A. Boat owners are not permitted to scrape, sand, or bottom paint their boats while on marina property. The waste material is considered hazardous waste and must be disposed of through licensed haulers. B. Waste oil disposal. Clarks Landing provides a waste oil tank for customer use via the service department. Oil filters are to be drained completely on the rack provided. Under no circumstances shall oil filters be disposed of in the trash bins. C. Ethyl glycol permanent anti-freeze must be recycled. It cannot be discharged onto the ground. D. The trash bins at Clarks Landing are for trash generated while at the marina. Please do not bring your trash from outside and dispose of it in these receptacles. You will be responsible for the clean up costs of any hazardous material deposited in these receptacles or on marina property along with any fines or penalties. E. Clarks Landing has the right, but not the obligation to clean up any violations and bill the responsible party accordingly. F. Customers removing their own shrink wrap must remove the material from marina property. 7. Does not include installation of Bimini tops or enclosures. 8. Consists of adding some water to the tank and test that system pressurizes and pump works. 9. With the outdrive and outboard painting, if necessary, the first half hour of scraping and sanding will be FREE. However, any additional time will be billed out as time and material. 10. The tune-up price includes adjusting carburetor/set timing/installing spark plugs. The price does not include launch and test run. Therefore, if needed, customer will be billed for labor only on test runs. 11. We will not commission any engine not winterized by Clarks Landing Marina. 12. Temporary slip tie-up: Customer does not have to be present at time of launch. Clarks Landing Marina will notify owner when boat is launched and moored in slip. Owner to secure lines within 24 hours of notification. Mooring lines must be in boat prior to launch date. If lines are missing or in questionable condition, new lines will be installed (2 bow, 2 stern, 2 spring lines) and billed to the customer. 13. A minimum of three (3) days prior to tentative launch date, boat owner must drop off a spare set of keys to ignition and all locks onboard. 14. Clarks Landing assumes no responsibility for damage to canvas, isinglass, zippers tracks, stitching or fasteners, during the installation process as this is generally an indication of aged or degraded material. The customer will be contacted in the event the enclosure is found to be in need of repair. 15. Due to space limitations it is imperative that you remove your boat after the services have been completed. Boats remaining on land or in the water for more than 5 days after completion of the work will be subject to a storage fee of $2.00 per foot per day. 7

Authorization below allows Clarks Landing Marina to apply payment to your invoice. You and your employees may operate my boat for the purpose of testing, inspecting or delivery at my risk. An express mechanic s lien is acknowledged on boat to secure the amount of repairs thereto.
